Detta projekt lägger grunden till att förbättra tillgången till hälso- och sjukvård för ungdomar i glesbygd genom attkartlägga tillgången till hälso-och sjukvård för unga i norra Sverigekritiskt utvärdera strategier som utvecklats för att öka tillgänglighet kopplat till ungas önskningar och behovföreslå hälsofrämjande aktioner i samklang med intressenterProjektet utgår från nationell såväl som internationell data om ungdomars ökande ohälsa generellt och tillgång till hälso- och sjukvård i glesbygd, samt att ungdomsmottagningar i stor utsträckning saknas i norra Sverige (endast i 12/35 av glesbygdskommunerna). Även om det är väl känt att tillgången till hälso- och sjukvård är ett problem för befolkning som bor i svensk glesbygd så har det inte funnits någon övergripande studie om hur olika strategier för att möta behoven hos glesbygdens ungdomar påverkar deras tillgång till hälso-och sjulvård eller hälsostatus.Vi kommer att fokusera på de fyra nordligaste länen genom att: 1) kartlägga ungdomars tillgång till hälso- och sjukvård i relation till deras vårdbehov, inklusive utvärdering av både horisontell jämlikhet (likvärdigt vårdutnyttjande för individer med likvärdiga behov; oavsett exempelvis ras, kön eller klass) och vertikal jämlikhet (olika vårdutnyttjande för individer med olika behov; exempelvis mer vård för dem med större behov), samt jämföra jämlikhet i tillång till vård mellan stadskommuner och glesbygdskommuner, och mellan glesbygdskommuner med och utan ungdomsmottagning; 2) genomföra en fallstudie för att undersöka de strategier som utvecklats (ungdomsmottagningar, internetapplikationer, hälsoprogram) för att förbättra ungdomars tillgång till hälso- och sjukvård; 3) identifiera de förhållanden som leder till god tillgång till hälso- och sjukvård genom en kvalitativ jämförande analys av alla 35 landsbygdskommuner samt 4) genomföra en kartläggning av aktörers förslag för att förbättra glesbygdens tillgång till hälso- och sjukvård för ungdomar.
Abstract [en]
This research will lay the foundation for improving youth health services in rural areas bymapping youth access to health care services in the rural municipalities of northern Sweden,critically evaluating strategies used to enhance access according to needs,proposing further actions through a participatory process.The project emerges from both Swedish and international concerns about health status and health care access of rural youth, and the relatively low use of youth clinics in Sweden?s rural north (just 12 of the 35 rural municipalities have youth clinics). While it is well known that access to health care services continues to be a problem for populations living in rural areas in Sweden, there has been no comprehensive assessment of the impact of different rural youth health service strategies on either access or health outcomes.We will focus on the four northernmost Swedish counties, mixing qualitative and quantitative methodologies sequentially in four phases. First, we will map youth access to health care according to their health care needs, including assessing horizontal equity (equal use of health care for equivalent health needs,) and vertical equity (people with greater health needs should receive more health care than those with lesser needs), as well as comparing equity in access to health care between rural and urban municipalities, and between rural municipalities with and without a youth clinic. Second, we will use a multiple case study to investigate the strategies developed (youth clinics, internet applications, public health programs) to improve youth access to health care. Third, through qualitative comparative analysis of the 35 rural municipalities, we will identity the best combination of conditions leading to high youth access to health care. Fourth, we will conduct a concept mapping study to involve rural stakeholders, care providers and youth in proposing actions to improve rural youth access to health care.
