Abstract [sv]

Bakgrund: Datortomografi är en utmärkt undersökningsmetod vid diagnostik av lungsjukdomar och är överlägsen i jämförelse med konventionell lungröntgen. Undersökningsmetoden ger emellertid högre stråldos och antalet undersökningar ökar i Sverige. Balansen mellan en bra bildkvalitet och en låg stråldos till patienten utan att missa viktiga strukturer är en utmaning. 

Motiv: Tidigare studier har jämfört stråldos och bildkvalitet mellan lågdos- och standarddosundersökningar men utan att ta hänsyn till kön och BMI. Därför vill författarna undersöka hur bildkvalitet och brusnivå skiljer sig mellan lågdos- och standarddosundersökning även utifrån kön och BMI.

Syfte: Att jämföra brusnivå och bildkvalitet mellan lågdos- och standarddosundersökningar vid genomförande av DT-lungor, samt undersöka om det finns någon skillnad i bildkvalitet beroende på kön eller BMI.

Metod: Kvantitativ studie med retrospektiv design. Två grupper jämfördes där ena gruppen har undersökts med lågdos och den andra med standarddos. Totalt inkluderades 66 undersökningar i studien.

Resultat: Lågdosundersökning ger en sämre bildkvalitet jämfört med standarddos. Det fanns ingen signifikant skillnad i poängbedömningen gällande bildkvalitet mellan kvinnor och män i båda undersökningsgrupperna. Brusnivån var signifikant högre i lågdosgruppen vid mätningarna i aorta och trachea men ingen signifikant skillnad i lungorna. Kvinnor tenderar att få en sämre bildkvalitet vid ett högre BMI jämfört med män i standarddosgruppen. I lågdosgruppen uppfyllde, sammantaget i samtliga BMI-grupper, 43.7 % av männen och 35.3 % av kvinnorna de diagnostiska kraven.

Diskussion: Kön och BMI bör vägas in som felkälla vid datortomografi av lungor. Möjligen kan SAFIRE-styrkan anpassas efter BMI och organmodulering användas för att minska stråldosen och förbättra bildkvaliteten till kvinnor.

Konklusion: Lågdosgruppen erhöll lägre poäng vid bedömningen av bildkvalitet. Bildkvaliten blir generellt sämre hos kvinnor. Undersökningsprotokollen för lungor på datortomografi bör individanpassas utifrån kön och BMI.

Abstract [en]

Background: Computer tomography is an outstanding examination method for diagnosis of lung diseases, superior to conventional lung x-ray. The study method gives however a higher radiation dose and the number of surveys is increasing in Sweden. The balance between a good image quality and a low radiation dose to the patient without missing important structures is a challenge.

Motive: Previous studies have compared radiation dose and image quality between low dose and standard dose surveys, but did not take gender and BMI into account. Therefore, the authors wanted to study how image quality and noise levels differ between standard dose and low dose research, also analysing gender and BMI.

Aim: To compare noise levels and image quality between low dose and standard dose surveys using computed tomography of the lungs and investigating whether there is any difference in image quality depending on gender or BMI.Methods: Quantitative study with retrospective design. Two groups were compared where one group was examined with low dose and the other with standard dose. A total of 66 studies were included.

Result: Examinations using low dose resulted in a reduced image quality compared to the standard dose. No significant difference was found in the score assessment of image quality between women and men in both study groups. The noise level was significantly higher in the low dose group in the aortic and tracheal measurements, but no significant difference was found for the lungs. Women tended to get a reduced image quality at a higher BMI than men in the standard dose group. In the low dose group, 43.7% of men and 35.3% of women met the diagnostic requirements (average for all BMI groups).

Discussion: Gender and BMI should be weighed in as a source of error in computerized tomography of lungs. Possibly, the SAFIRE strength can be adjusted for BMI and organ modulation used to reduce radiation dose and improve image quality for women.

Conclusion: The low dose group received lower scores in the assessment of image quality. Image quality was generally worse for women patients. Attempts should be made to individually adjusted the examination protocol for lungs on computer tomography based on gender and BMI.
