Abstract [en]

Aims: Several studies have explored the relationship between risk of stroke and texture characteristics of the atherosclerotic plaque in patients asymptomatic carotid stenosis. It is, however, unknown if this concept could be extended to the short-term risk of recurrent stroke in patients with symptomatic carotid stenosis. We hypothesised that the plaque texture can be useful in predicting recurrent ipsilateral ischemic cerebrovascular events (CVE)  in patients with symptomatic carotid stenosis waiting for carotid endarterectomy (CEA).

Method and results: We investigated 162 consecutive patients with > 70 % internal carotid artery stenosis. Twenty-nine patients (17.9%) were not included because of poor B-mode image quality and shadowing artifacts over the plaque. A total of 327 plaques from 133 patients were analyzed.  Thirty- four patients had a recurrent ipsilateral CVE before CEA. Five different ultrasound texture characteristics were tested in the analysis for risk stratification. After univariate analysis, except for total cholesterol (p=0.024), no variables were related to recurrent ipsilateral ischemic CVE. Furthermore, when using a multivariate model, still no predictors of recurrent CVE were observed.

Conclusion: Ultrasound based plaque texture analysis appears not to be useful in predicting the short-term risk of recurrent ipsilateral ischemic CVE before CEA in patients with symptomatic carotid stenosis
