Associations between atherosclerotic plaque and self-efficacy, depression and anxiety in an asymptomatic general population aged 60

Abstract [en]

The majority of cases of cardiovascular disease (CVD) are caused by atherosclerosis. Carotid plaques are associated with increased risk of CVD, independently of traditional cardiovascular risk factors. Evidence of early disease is demonstrated before onset and can be assessed with ultrasound. Psychosocial stressors, together with raised lipids and smoking have been identified as the most important risk factors. Cognitive factors such as self-efficacy have been identified as important constructs for health behaviors, and emotional factors do also play a role in this context. Therefore, the present study investigated the association between general self-efficacy, health-specific self-efficacy, anxiety and depression, on the one hand, and atherosclerosis, on the other hand, in a sample of 2291 participants aged 60 year from a general asymptomatic population. Among 2291 participants, 1247 had plaque and 1044 did not have plaque. The results showed that depression (in men only) and anxiety (in participants with university education only) were associated with plaque, and health-specific self-efficacy (in men only) was associated with absence of plaque. However, when statistically controlled for snuff, smoking, harmful alcohol consumption, diabetes, myocardial infarction, hypertension, hypercholesterol and sex or education no group differences were found. Variables share biomedical factors and is therefore possible statistically over-controlled. From an epidemiological perspective, even small changes obtained in a large population have high impact. Therefore, the research question is important and needs to be adressed with interdiciplinary compentence.

Majoriteten av alla kardiovaskulära sjukdomar orsakas av ateroskleros. Plack i halsartärerna kan knytas till ökad risk för kardiovaskulära sjukdomar, oberoende av andra välkända riskfaktorer. Innan sjukdomsdebut syns tidiga tecken på åderförkalkning, vilket kan upptäckas vid en ultraljudsundersökning. Psykosociala stressorer, tillsammans med förhöjda blodfetter och rökning har identifierats som dominerande riskfaktorer. Kognitiva faktorer som self-efficacy (bedömning av egen kompetens) har identifierats som viktiga konstrukt för hälsobeteenden, och emotionella faktorer är också avgörande i denna kontext. Denna studie undersöker därför associationen mellan generell self-efficacy, hälsospecifik self-efficacy, ångest och depression å ena sidan, och å andra sidan ateroskleros i en grupp bestående av 2291 deltagare vid 60 års ålder från en asymtomatisk population. Av 2291 deltagare hade 1247 plack, och 1044 hade inte plack. Resultatet visade att depression (hos män enbart) och ångest (hos deltagare med universitetsutbildning enbart) var associerat med palck, och hälso-specifik self-efficacy (hos män enbart) var associerat med frånvaro av plack. När resultatet kontrollerades statistiskt för rökning, snus, skadlig alkoholkonsumption, diabetes, hjärtinfarkt, hypertoni, hypercholesterol och kön eller utbildning sågs dock ingen skillnad mellan grupperna. Då variablerna har gemensamma biomedicinska faktorer, är det möjligt att de över-kontrollerats statistiskt. Ur epidemiologiskt perspektiv är även små förändringar som åstadkoms i en stor population högst betydelsefulla. Det är därför viktigt att forskningsfrågan undersöks med tvärvetenskaplig kompetens.
