Abstract [sv]

Bakgrund: Det flesta aborter i Sverige sker innan vecka 12. År 2017 utfördes mellan 35000 - 38000 aborter varav 2313 av dessa var sena aborter. Sena aborter handläggs t.o.m. graviditetsvecka 21+6 och utförs genom medicinsk abort. Det är barnmorskan som bistår vid vårdandet av kvinnan vid sen abort.

Motiv: Att bistå sena aborter som barnmorska kan vara en komplicerad situation på många sätt. Detta är ett ämne som studerats lite under det senaste årtiondet.

Syfte: Syftet med studien är att belysa barnmorskors erfarenheter av att bistå kvinnor som genomgår inducerade aborter i graviditetsvecka 18+0 – 21+6.

Metod: Kvalitativ metod med induktiv ansats. Data samlades in genom kvalitativa semistrukturerade intervjuer. Analysen skedde med hjälp av kvalitativ innehållsanalys.

Resultat: Fyra huvudkategorier bildades; Barnmorskans kompetens och ansvar är en förutsättning, Emotionell bearbetning tar tid för barnmorskor, Behov av stöd bland barnmorskor finns samt Vårdmiljön anses påverka kvinnan starkt.

Diskussion: Vi diskuterar angående barnmorskornas kompetens anses vara tillräcklig för att bistå kvinnor vid sen abort. Diskussion om att barnmorskan anser att de påverkas genom att bistå vid sen abort och när fostret visar livstecken som stämmer överens med befintlig forskning. Vårdmiljön anses inte vara optimal vid vårdandet av sen abort vilket även det styrks med den befintliga forskningen.

Slutsats: Ett medicinsk PM fanns att utgå ifrån men som inte innefattade omvårdnad till kvinnan eller bättre smärtlindringsmetoder. Ökat stöd från chefsnivå behövs vid komplicerade situationer. Vårdmiljön sågs inte vara optimal i vårdandet av sen abort.

Abstract [en]

Background: Most abortions due before week 12, 2017 were 35000 - 38000 abortions executed which 2313 were late abortions. Late abortions handles until pregnancy week 21+6 and is executed through medical abortion. It is the midwife who assists in caring for the woman in late abortions.

Motives: To assist in late abortions as a midwife might be a complicated situation in many ways. This is a subject that have not been well studied in the last decade.

Aim: The aim of the study is to illustrate midewifes experiences with caring for women that go through induced abortions in pregnancy week 18+0 - 21+6.

Method: Qualitative method with inductive approach. Data was collected by qualitative semi-structured interviews. The analysis was performed by qualitative content analysis.

Results: Four main categories formed; The midwifes competence and responsibility is key, it takes time for midwifes for emotional processing, Support among midwifes are needed and the woman is strongly impacted by the care environment.

Discussion: We discussed if the competence among midwifes is high enough to assists women during late abortions. Together with the influence on midwifes assisting with late abortions, when the fetus is showing life signs according to available research. Furthermore, the caring environment is not optimal during late abortions which also is proven by available research.

Conclusion: A medical PM was available about late abortions but did not include caring for the women or sufficient analgesics. During complicated situations the greater support from head of staff is needed. The caring environment were not suited for caring during late abortions.
