Abstract [en]

Aim. To examine nurse anestethists’ experiences of the extubation procedure.

Background. Extubation is a critical moment in the nurse anesthetists’ daily work where several factors must be considered. Despite its complexity and the relatively high risk of complications, the extubation procedure has been neglected, in education as well as in research. Research shows that nurse anesthetists are exposed to work-related stress, but there are no studies on how they experience the critical extubation procedure.

Methods. Ten nurse anesthetists with varying experience from two hospitals in northern Sweden participated in the study. During one week in November 2013 individual interviews with semi-structured questions were conducted, the interviews were analyzed using qualitative content analysis.

Results. "Experiencing support" proved to be a central theme for the nurse anesthetists’ when carrying out the extubation procedure. "Being professionally experienced", "To feel back up" and "Remaining calm" were crucial factors in the critical procedure. Their sense of security in the procedure depends on the level of prior experience of the procedure, support from colleagues and medical equipment and from their own ability to remain calm.

Conclusion. The extubation procedure is a complex moment in the nurse anesthetists’ work. Professional experience is the key factor to feel secure. The extubation procedure deserves more attention in the postgraduate programme in anesthesia nursing and we propose it is given more space theoretically as well as practically. Nurse anesthetists may need to develop their self-awareness of how they react in stressful situations and practice their ability to remain calm. We believe that support from more professionally experienced colleagues, for example through mentoring, may support professionally unexperienced nurse anesthetists. For the professionally unexperienced nurse anesthetist increased support may also be provided through a checklist.

Syfte. Att undersöka anestesisjuksköterskors upplevelser av extubationsprocessen.

Bakgrund. Extubation är ett kritiskt moment i anestesisjuksköterskans dagliga arbete där flera faktorer ska vägas in. Trots dess komplexitet och den relativt höga risken för komplikationer har extubationsprocessen hamnat i skymundan, både under utbildning och i forskning. Forskning visar att anestesisjuksköterskor känner yrkesrelaterad stress men det saknas studier på hur de upplever den kritiska extubationsprocessen.

Metod. Tio anestesisjuksköterskor med varierande yrkeserfarenhet från två sjukhus i norra Sverige deltog i studien. Under en vecka i november 2013 genomfördes individuella intervjuer med semistrukturerade frågor, intervjuerna analyserades därefter med kvalitativ innehållsanalys.

Resultat. ”Att uppleva stöd” visade sig vara ett centralt tema för anestesisjuksköterskorna i deras arbete i extubationsprocessen. ”Att vara erfaren”, Att känna support” samt ”Att behålla lugnet” var avgörande faktorer i den kritiska processen. Känsla av trygghet uppkom beroende på grad av erfarenhet av processen, tillgång till support från kollegor och medicinteknisk utrustning samt från den egna förmågan att kunna behålla lugnet.

Slutsats. Extubationsprocessen är ett komplext moment i anestesisjuksköterskornas arbete. Erfarenhet är den viktigaste faktorn till trygghet inför momentet. Extubationsprocessen förtjänar större uppmärksamhet i specialistsjuksköterske-utbildningen inom anestesi och vi föreslår att den ges mer utrymme både teoretiskt och praktiskt. Anestesisjuksköterskor kan behöva utveckla sin självkännedom för att förstå hur de reagerar i stressfyllda situationer för att lära sig behålla lugnet. Vi tror att stöd från mer erfarna kollegor, exempelvis genom mentorskap, kan stötta oerfarna anestesisjuksköterskor. Ökat stöd kan också ges genom en checklista för den oerfarna anestesisjuksköterskan.
