Abstract [sv]

Irritabel tarmsyndrom (IBS) är en vanlig och för många personer besvärlig mag- och tarmsjukdom. I dagsläget finns ingen enhetlig förklaringsmodell men tidigare forskning menar att både psykiska och biologiska faktorer är av betydelse. Ett sätt att förestå dessa är genom fenomenet sensitisering. Oro och hjälplöshetsreaktion vid specifika stimuli skapar en stressreaktion i kroppen som efter upprepade påslag automatiseras och sensitiserar systemet. Andra sjukdomar som föreslås bero av sensitisering, exempelvis fibromyalgi, kroniskt trötthetssyndrom och migrän, är vanliga komorbida sjukdomar vid IBS. En annan sjukdomsgrupp som misstänks bero av sensitisering men som inte har undersökts i relation till IBS är miljökänsligheter. Syftet med studien var att undersöka personer med IBS avseende miljökänslighet och modern hälsooro med teorin om sensitisering som grund. En enkät skickades ut till 8520 personer (18-79år) i Västerbotten, stratifierat för ålder och kön. Av 3406 besvarande deltagare (40%) rapporterade 80 personer att de diagnostiserats med IBS. Dessa jämfördes med 3326 personer utan IBS. Resultatet visade signifikant högre odds för att vid IBS, jämfört med utan IBS, även ha diagnostiserad lukt- och ljudkänslighet, andra medicinskt oförklarade symptom samt psykiska sjukdomar. De med IBS rapporterade även signifikant mer besvär av lukt, ljud och el samt högre modern hälsooro än de utan IBS. Teorin om sensitisering som gemensam bakomliggande orsak till både IBS och miljökänsligheter anses av studiens resultat vara fortsatt aktuell.

 

Abstract [en]

Irritable bowel syndrome (IBS) is a common, and for many persons a troublesome, gastrointestinal disease. There is no single explanation for it, but previous research argues that both psychological and biological factors are of importance. One way to understand these factors is through the phenomenon of sensitization. Worries about and reaction of helplessness to specific stimuli create a stress reaction which after repeated occasions becomes automated, and thereby sensitizes the system. Other diseases that are proposed to be caused by sensitization, such as fibromyalgia, chronic fatigue syndrome and migraine, are common comorbid disorders in IBS. Environmental sensitivities are another group of diseases suggested to be caused by sensitization, but that have not been studied in relation to IBS. The aim of the study was to examine people with IBS with respect to environmental sensitivity and modern health worries with the theory of sensitization as a basis. A questionnaire was sent to 8520 individuals (18-79 years) in Västerbotten, stratified for age and sex. Of the 3406 responding participants (40%), 80 reported that they were diagnosed with IBS. These were compared with 3326 persons without IBS. The results showed significantly higher odds of those with IBS, compared to those without IBS, to also have a diagnosed smell- and sound sensitivity, other medically unexplained symptoms and mental illnesses. Those with IBS were also significantly more bothered by smell, sound, electricity and had higher modern health worries than those without IBS. The theory of sensitization as a shared underlying cause of both IBS and environmental sensitivities are by the study's results considered to remain relevant.
