    Abstract [sv]

    Syftet med fördjupningsarbetet har varit att via en pilotstudie öka kunskapen om ungdomar med funktionsnedsättning i Umeå, med avseende på psykisk hälsa, och på hur de upplever sin vardag i skolan, med kamrater och i familjen. Psykisk hälsa undersöktes genom att ungdomar med funktionsnedsättning som går i specialklasser (grundsärskolans högstadium, högstadium för ungdomar med Asperger, högstadium för ungdomar med rörelsenedsättningar), fick besvara en enkät under skoltid. För att kunna relatera till den undersökning som Statens Folkhälsoinstitut genomförde hos ungdomar i Sverige 2009 har samma enkät använts.

     

    Resultatet visar att de flesta ungdomar har angett att de mår bra, trivs i skolan, har bra kontakt med sina lärare och med sina föräldrar. Majoriteten mår lika bra som de flesta ungdomar i landet och t.om bättre än de ungdomar i Folkhälsoinstitutets undersökning, som angett att de hade någon funktionsnedsättning.

     

    Inom några områden visades dock lägre resultat. Ungdomarna i vår undersökning uppgav att deras svårigheter påverkade deras vardag i familjen och vid fritidsaktiviteter. De umgås mer sällan med kamrater, motionerar mindre och har färre fritidsaktiviteter. Ungdomar med Asperger skiljer ut sig mest i vår undersökning, de är mindre nöjda med livet och med sig själva, känner sig mer ensamma,  umgås mer sällan med kamrater och har mindre kul, jämfört med ungdomar i grundsärskolan.

     

    Det finns områden där stödet ifrån Barn- och ungdomshabiliteringen kan förbättras, som att utveckla metoder för socialt samspel och stärka ungdomarnas identitet och i samarbete med andra aktörer öka delaktigheten till en mer aktiv fritid.

    Traffic and drowning incidents with emphasis on the presence of alcohol and drugs2014Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    Worldwide, fatal traffic injuries and drowning deaths are important problems. The aim of this thesis was to investigate the cirumstances of fatal and non-fatal traffic injuries and drowning deaths in Sweden including analysis of the presence of alcohol and drugs, which are considered to be major risk factors for these events. Data where obtained from the database of National Board of Forensic Medicine.

    In the first study, we investigated 420 passenger deaths from 372 crashes during 1993-1996. There were 594 drivers involved. In total, 21% of the drivers at fault were alcohol positive compared to 2% of drivers not at fault (p<0.001) (Paper I). During 2004-2007, crashes involving 56 fatally and 144 non-fatally injured drivers were investigated in a prospective study from Northern Sweden (Paper II). The drivers were alcohol positive in 38% and 21%, respectively. Psychoactive drugs were found in 7% and 13%, respectively. Benzodiazepines, opiates and antidepressants were the most frequent drugs found in drivers. Illict drugs were found 9% and 4% respectively, with tetrahydrocannabinol being the most frequent of these drugs (Paper II).

    We investigated 5,125 drowning deaths in Sweden during 1992-2009 (Paper III). The incidence decreased on average by about 2% each year (p<0.001). Unintentional drowning was most common (50%). Alcohol was found in 44% of unintentional, 24% of intentional, and 45% of undetermined drowning deaths. Psychoactive substances were detected in 40% and benzodiazepines were the most common substance. Illicit drugs were detected in 10%. Of all drowning deaths, a significantly higher proportion females commited suicide compared with males (55% vs. 21%, p<0.001). Suicidal drowning deaths (n=129) in Northern Sweden were studied further in detail (Paper IV). of these, 53% had been hospitalized due to a psychiatric diagnosis within five years prior to the suicide. Affective and psychotic disorders were the most common psychiatric diagnoses. Almost one third had performed a previous suicide attempt. One fourth had committed suicide after less than one week of discharge from hospital. Alochol was found in 16% and psychoactive drugs in 62% of these cases, respectively. 

    In conclusion, alcohol and psychoactive drugs are commonly detected among injured drivers and drowning victims, and probably play a role in these events. Most of the individuals that tested positive for alcohol and high blood concentrations, indicating alochol dependence or abuse. This association warrants futher attention when planning future prevention.

    Alcohol and drugs are important risk factors for traffic injuries, a major health problem worldwide. This prospective study investigated the epidemiology and the presence of alcohol and drugs in fatally and hospitalized non-fatally injured drivers of motor vehicles in northern Sweden. During a 2-year study period, blood from fatally and hospitalized non-fatally injured drivers was tested for alcohol and drugs. The study subjects were recruited from well-defined geographical areas with known demographics. Autopsy reports, medical journals, police reports, and toxicological analyses were evaluated. Of the fatally injured, 38% tested positive for alcohol and of the non-fatally 21% tested positive; 7% and 13%, respectively, tested positive for pharmaceuticals with a warning for impaired driving; 9% and 4%, respectively, tested positive for illicit drugs. The most frequently detected pharmaceuticals were benzodiazepines, opiates, and antidepressants. Tetrahydrocannabinol was the most frequently detected illicit substance. No fatally injured women had illegal blood alcohol concentration. The relative proportion of positively tested drivers has increased and was higher than in a similar study 14 years earlier. This finding indicates that alcohol and drugs merit more attention in future traffic safety work.

    Abstract [en]

    Deaths by exsanguination among various underlying causes of death were analyzed in order to expand the knowledge on the relation of extravasated blood volume to other documented parameters.

    A consecutive series of 193 cases of ruptured thoracic aortic aneurysm (n=13), gunshot wounds (n=63), stab wounds (n=28), rib fractures (n=5), and blunt injury to thoracic aorta (n=84) were investigated.

    The amount of internal bleeding into pleural cavities only varied greatly (200-3,400 ml) with a mean value of 1,174 ml, slightly higher among males. Age, body weight, cause and manner of death, external bleeding, alcohol inebriation, multiplicity of injuries, and degree of coronary heart disease did not significantly affect the amount of internal (pleural) bleeding. Also, post-mortem delay to autopsy did not correlated to the amount of extravasated blood, indicating that post-mortem bleeding is of no importance in these days.

    OBJECTIVE: Previous studies on alcohol involvement associated with fatal injury in traffic crashes have focused on the drivers, but the passenger's view is not well known. This study (1) analyzes the relationship between passenger's death and alcohol inebriation of the driver and (2) estimates the role of alcohol as the cause of a crash by examining who was at fault, sober, or inebriated.

    METHOD: The study includes all motor vehicle passengers (n = 420) who died in crashes in Sweden 1993 through 1996 and were medicolegally autopsied. Autopsy reports from the Departments of Forensic Medicine, including toxicological analyses, and police reports were studied. Presence of alcohol among drivers was based on blood and breath tests.

    RESULTS: One-fifth of the fatally injured passengers and one-fifth of the tested drivers were under the influence of alcohol. The youngest drivers had the highest prevalence of drunken driving. Drivers at fault were alcohol positive in 21% of these crashes and drivers were not at fault in 2% of these crashes. In 53% of the crashes where both the passenger and driver were alcohol positive, the passenger had a lower alcohol concentration than the driver. Children (<16 years) comprised 15% of the killed passengers. Notably, the children were riding with a driver who was under influence of alcohol in 13% of these crashes. Alcohol involvement was not tested in half of the surviving drivers.

    CONCLUSIONS: The data show that 20% of both passengers and drivers were under the influence of alcohol. Increased testing of surviving drivers regarding alcohol and other drugs is recommended.

    OBJECTIVES: Unnatural deaths among Indigenous populations, including the Swedish Sami, occur more often than among the general population. To find prevention strategies, we explored the circumstances of the unnatural deaths of members of reindeer-herding Sami families.

    STUDY DESIGN: The number of deaths from among a cohort of 7,482 members of reindeer-herding Sami families were retrieved from the National Board of Health and Welfare for the years 1961- 2001.

    METHODS: An evaluation of the information from autopsy records at the National Board of Forensic Medicine, police reports, and available medical records identified 158 unnatural deaths. These were then analysed in detail.

    RESULTS: Transport-related deaths and suicides were the most common unnatural deaths among Swedish reindeer-herding Sami family members. Suicides contributed to 23% of all deaths, road traffic accidents to 16%, and snowmobile fatalities to 11%. The accidents generally reflected an "outdoor lifestyle" and the working conditions were characterized by the use of off-road vehicles such as snowmobiles. Half of the number of victims tested positive for alcohol and alcohol abuse was documented in 15% of all victims.

    CONCLUSIONS: The results indicate that alcohol is an important factor in preventing unnatural deaths among reindeer-herding Sami, together with increased safety of both on-road and off-road transportation.

    Suicides by drowning have received limited attention by researchers. A recent finding that almost onethird of all drowning deaths in Sweden were classified as suicide instigated this study. We identified 129 cases of suicide by drowning in Northern Sweden and analyzed the circumstances and the psychiatric history prior to the suicide. Information was obtained from autopsy, police and medical records, as well as from the National Inpatient Register. One-third of the suicide victims had previously attempted suicide and half of the victims had been hospitalized due to mental health problems. One-third of these had left the hospital less than one week before the suicide. Alcohol and psychoactive drugs were present in 16% and 62% of the cases, respectively. A history of mental disorder and previous suicide attempt (s), especially by drowning, is an ominous combination necessitating efficient clinical identification, treatment and follow-up if a complete suicide is to be prevented.

    Background: Drowning deaths constitute a significant proportion of unnatural deaths globally. In Sweden and other high-income countries, drowning deaths have decreased. This study investigates the epidemiology and current trends of unintentional, intentional, and undetermined drowning deaths with emphasis on the presence of alcohol and other drugs.

    Methods: During an 18-years period, 5,125 drowning deaths were autopsied in Sweden. Data on cases including toxicological analysis on alcohol, pharmaceutical drugs, and illicit drugs were obtained from the National Board of Forensic Medicine.

    Results: During the study period, the annual incidence of drowning deaths in Sweden was 3.1/100,000 inhabitants and decreased on average by about 2% each year (p<0.001). The highest incidence was found among males and in middle/older age groups. The incidence increased 3% for each year of age. Children/adolescents (<= 18 years) constituted 5% of all drowning deaths. Of all drowned females in the study, 55% (847/1,547) committed suicide, which was a significantly higher proportion compared with males (21%, 763/3,578) (p<0.001). In total, 38% (1,656/4,377) of tested drowned persons had alcohol in their blood and the mean concentration was 1.8 g/l. In the unintentional drowning group, intentional drowning group, and the undetermined group, the proportion of alcohol positive was 44%, 24%, and 45%, respectively. One or several psychoactive drugs were present in the blood in 40% (1,688/4,181) of all tested persons and in 69% (965/1,394) of tested persons who died from suicidal drowning. The most common drug was benzodiazepines (21%, 891/4,181). Illicit drugs were detected in 10% (82/854) of tested persons.

    Conclusion: Presence of alcohol and drugs were frequent and may have contributed to the drowning deaths. The incidence of drowning deaths significantly decreased during the study period. Males and the middle/older age groups had a higher incidence compared to females and children. Suicidal drowning was common especially among women. Alcohol and drugs are significant contributors in drowning deaths in Sweden and should be considered as part of a comprehensive prevention program.

    OBJECTIVES: To evaluate the validity of computed tomography (CT) for reproduction of the bone margins of the temporomandibular joint (TMJ).

    METHODS: Seven TMJ specimens were examined with a CT and then cryosectioned. The bone separating the TMJ from the middle cranial fossa, middle ear and external auditory canal was measured as the full width at half maximum (FWHM). Measurements were compared with the true thickness of the bone wall.

    RESULTS: There was good agreement when the bone walls were thicker than 1 mm: accuracy was influenced only by the angle of the bone wall to the scanning plane. Conversely, bone walls thinner than 1 mm were reproduced with a magnification that increased with decreasing bone thickness. The difference increased further as the inclination of the bone wall became greater.

    CONCLUSION: Measurements performed at FWHM are reliable within +/- 10% for bone walls more than approximately 1 mm thick which form an angle of less than 35 degrees to the perpendicular of the scanning plane. For bone walls thinner than 1 mm and for those thicker than 1 mm with an inclination exceeding approximately 35 degrees, partial volume effects result in a progressively increasing magnification of bone thickness.

    Bakgrund

    Morfologisk kunskap har visats gynna läsutvecklingen vilket lett till ökat forskningsintresse för hur morfologisk träning och medvetandegörande kan främja läsförmågan, framförallt bland elever med läs- och skrivsvårigheter. Hur morfologisk kunskap ter sig i typisk språkproduktion och huruvida morfologiska skillnader finns beroende på om produktionen utgörs av tal eller skrift är däremot mindre beforskat. En svårighet när det kommer till forskning om morfologisk produktion är att morfologiskt processande sannolikt sker under den språkliga förberedelsefasen vilken är svår att undersöka experimentellt. Ett sätt att fånga morfologiskt processande under produktion är att undersöka när personer möter svårigheter och hur de bearbetar dessa svårigheter. 

    Syfte

    Studiens syfte var att undersöka eventuella samband mellan morfologisk osäkerhet (dvs. pauser, revideringar och fel) i talat och skrivet språk samt undersöka om dessa osäkerheter avspeglas i ett morfologiskt kunskapstest.

    Metod

    Nitton högstadieelever fick genomföra en skrivuppgift, en muntlig uppgift samt ett nykonstruerat morfologiskt kunskapstest. Elevernas skrivprocess registrerades med hjälp av keystroke-logging för att analysera morfologisk osäkerhet under skrivande samt andel morfologiska fel och särskrivningar i den slutgiltiga elevtexten. Den muntliga uppgiften spelades in och analyserades med avseende på morfologisk osäkerhet under muntlig framställning. Morfologitestet, vilket bestod av sex deltester, genomfördes skriftligt och det sammanlagda resultatet för varje elev beräknades. 

    Resultat

    Ingen elev uppvisade morfologisk osäkerhet under muntlig produktion. Under skrivande noterades följande processer i morfemgränser på gruppnivå: pauser (10,21%), revideringar (1,31%), fel (0,49%) och särskrivningar (0,52%). Andel morfologiska osäkerheter under skriftlig produktion hade inget signifikant samband med resultatet på det morfologiska kunskapstestet. Post hoc-analys visade dock ett positivt samband mellan morfologiska pauser i skrivprocessen och antal poäng på ett deltest som undersöker morfologisk-syntaktisk kunskap. 

    Slutsatser

    Denna explorativa studie indikerar att morfologisk osäkerhet är ovanlig i talproduktion men att det kan förekomma i skrivprocessen, främst genom pauser i morfemgränser.  Resultatet antyder vidare att samband finns mellan pauser i morfemgränser under egen produktion och resultatet på ett test avsett att mäta morfologisk-syntaktisk kunskap. Resultatet bör dock tolkas aktsamt med hänsyn till det låga deltagarantalet och studiens explorativa angreppssätt.

    Två svenska pojkar, Texas Linderot och Kevin Hjalmarsson, var båda fyra år gamla med en tydlig sak gemensam. Båda är döda. När det är ett barn som blivit dödat/mördat vill vi inte tro att det är sant.  

     

    I de båda fallen med pojkarna var det inte en vuxen person eller en pedofil som var gärningsmännen, det var barn. Kevin Hjalmarssons mördare var två syskon som var fem respektive sju år gamla. Texas Linderoths mördare var en pojke som var tio år gammal.

    Vad är det som har hänt de båda mördade pojkarna är frågor som vi ställer oss.

    Var det lekar som gått snett?

     

    Nu i efterhand vet vi svaren på frågorna och vi vet med säkerhet att det inte var en lek som gick snett. Det var barn som med avsikt mördade andra barn. I båda fallen var det rättsläkarna på Rättsmedicinalverket som kunde konstatera att dödsorsaken var strypning och dödsättet inte skett genom en olycka  utan troligt orsakats av någon utomstående.

    När massmedia berättade den hemska sanningen bakom dessa två mord var vi många som inte trodde det var sant. Vi vill nog tro att barn är goda och inte benägna att utföra hemska våldshandlingar.

    Fem psykoterapeuter, med blandad grundutbildning, på en barnspsykiatrisk klinik i Blekinge, har intervjuats. Syftet har varit att undersöka vilka erfarenheter och upplevelser psykoterapeuterna har av att möta barn, ungdomar och familjer, tillhörande Jehovas Vittnen, i barnpsykiatrin. Jehovas Vittnen är ett trossamfund som finns över hela världen. Det är en tro som dikterar tydliga och stränga levnadsvillkor för sina medlemmar och som inte deltar i övriga samhällets förekommande traditioner.

     

    I intervjuerna har samtliga terapeuter upplevelser av att det är svårare att utmana och skapa förändring i dessa fall. Utmanandet av ett system där man lever reglerad utifrån en tro som Jehovas Vittnen, upplevs bekymmersamt. Terapeuterna saknar tillräcklig kunskap om samfundet och om barns levnadsvillkor där. Samtliga terapeuter uppger också att det inte är en självklar och naturlig sak att fråga patienter om deras trostillhörighet.

     

    I intervjuerna framkommer en negativt färgad bild av samfundet och svårigheter kring att jobba terapeutiskt i en kultur som är främmande den terapeuterna själv lever och är skolade i.

    Det faktum att terapeuterna inte har någon egen trostillhörighet verkar vara försvårande, då frågor om tro inte finns som en naturlig del i terapeuternas repertoar. Rädsla för att kränka den lagstadgade religionsfriheten och den privata sfären, är något som leder till försiktighet vid dessa möten. I uppsatsens avslutande del sker ett resonemang kring och ges förslag på frågor som skulle kunna ingå som en del i terapeuters anamnesupptagning, vid möten med nya familjer

    Background: As the group of very old persons will form an increasing part of society, the study of how well-being is described and affected by specific factors will be of importance to meet the future needs of these persons. The aim of the study was to increase knowledge of well-being in very old persons by combining assessments and perceptions using the Philadelphia Geriatric Morale Scale (PGCMS).

    Method: In a mixed method, convergent parallel design, 52 persons 80 years or older were assessed and interviewed using the PGCMS to combine assessment of morale and descriptions of perceptions of well-being using a mixed method approach.

    Results: Quantitative and qualitative results converged in four areas: not feeling lonely and being included, rating and perceiving health as good, high physical function/ability and being physically active, living in own house and feeling at home. Areas perceived as important to well-being captured only in qualitative analysis were having freedom and engagement. An example of insights not achievable from the quantitative or qualitative analysis alone was that individuals with high morale expressed anxiety about losing their health due to potential ageing-related threats and that individuals with low morale struggled with acceptance. Acceptance was the key strategy for handling adverse consequences of ageing in all described areas.

    Conclusion: When using standardized assessment scales in clinical practice, it could be useful to combine quantitative and qualitative data. Acceptance was key for well-being; however, acceptance could be resigned or reorienting in nature.

    Sammanfattning Studiens syfte var att undersöka terapeuters erfarenheter av hur ett samarbete kan skapas när utgångspunkten är att man inte känner sympati för patienten i en terapirelation. Det finns förhållandevis lite forskning som handlar om denna specifika aspekt av behandlingsrelationen. För att belysa frågan intervjuades sju legitimerade psykoterapeuter med en halvstrukturerad fokuserad intervju. Nyckelord och citat utmynnade i fem huvudteman med underteman. Huvudteman var: Utforskande/Nyfikenhet, Allians, Arbetsmetoder, Värderingar och Egen utveckling. Informanterna menade att det går att ha en samarbetsrelation med en patient trots att man som terapeut inte känner sympati men man upplever alliansen som svagare. De upplevde att det underlättar att ha en god terapeutisk allians över tid och att utfallet blir mer gynnsamt. Vi fann också att terapeutens personliga värderingar och etiska principer är det som avgör om man kan ha en behandlingsrelation överhuvudtaget. Hamnar terapeuten i alltför svåra personliga värdekonflikter upplevde samtliga att det inte gick att fortsätta. Resultatet visar att brist på sympati för patienter förekommer och intervjupersonerna hade metoder för att hantera dessa känslor. Lång yrkeserfarenhet, livserfarenhet, egenterapi, utbildning, handledning och tillgång till kolleger kan ha varit underlättande i hanteringen av de problem i terapirelation och behandling som en bristande sympati kan skapa.   Nyckelord: Allians, Sympati, Metod, Psykoterapi, Antipati

    Previous research has identified strength as a key factor for performance in cross-country skiing. The purpose of the present study was to investigate the relationship between upper body strength and performance in swedish junior cross-country skiers. 33 swedish young men and 23 young women age 17,3 ± 1,0 years volunteered to participate in specific tests during a summer training camp were they completed a test battery consisting of bench press and bench pull. The strength variables obtained were correlated with FIS-points (International Ski Federation´s ranking points for distance and sprint performance) used as the performance indicator. Correlations were performed by Spearman´s rank correlation coefficient for all variables. The results showed that FIS-points for sprint correlated significantly with both bench press and bench pull. No significant correlation was found between FIS-points for distance and bench pull. These results may predict performance for cross-country skiers and indicate a justification for increased proportion of strength training.

    In memory, familiar but no longer relevant information may disrupt encoding and retrieval of to-be-learned information. While it has been demonstrated that the ability to resolve proactive interference (PI) in working memory (WM) is reduced in aging, the neuroanatomical components of this decline have yet to be determined. Hippocampal (HC) involvement in age-related decline in control of PI is currently not known. In particular, the association between HC subfield volumes and control of PI in WM has not been examined previously. Here we investigate the associations between mean level and 5-year trajectories of gray matter subfield volumes and PI in WM across the adult life span (N = 157). Longitudinal analyses over 5-years across all participants revealed that reduced volume in the subiculum was related to impaired control of PI. Age-stratified analyses showed that this association was most pronounced in older adults. Furthermore, we found that in older adults the effect of age on PI was mediated by GM volume in the HC. The current results show that HC volume is associated with the ability to control PI in WM, and that these associations are modulated by age.

    Abstract [en]

    Background: Several risk factors can increase the risk of loneliness among the elderly population. In literature, loneliness increases the risk of illness and mortality and causes much emotional suffering. Aim: This study aimed to investigate social factors and lifestyle factors associated with loneliness among older adults> 65 years from northern Sweden. Methods: The design was a quantitative cross-sectional study using material from the Gerontological Regional DAtabase [GERDA]. A postal questionnaire sent to older adults > 65 living in northern Sweden. Results: There were significant associations between several factors and loneliness among older adults in northern Sweden. Protective factors was being married, being satisfied with life, feeling a zest for life and feeling safe. The risk factor was self-assessing yourself as depressed/dejected. Conclusion: Being depressed was a strong risk factor for loneliness. Proactive work and treatment for depression might be interventions that can be important in preventing loneliness among older adults. Screening for loneliness and depression should be a part of the general routine at health centres, in home health care and inpatient care, to find at-risk patients in time for preventively work
