    Changing patterns of addiction and increased psychiatric comorbidity pose new challenges for social services. As part of systematizing the work of assessing and investigating the need for help for people with alcohol or drug problems, use of the Addiction Severity Index is recommended. This descriptive quantitative study examines and compares how social workers and clients esti-mate the extent of a client’s problems in three areas: mental health, alcohol and drugs. Based on interviewer and client estimates, the study also examines whether the problems are estimated dif-ferently over time, from 2003 to 2017. The study includes ASI material for 15,005 people whose need for help for alcohol or drug problems has been investigated and assessed by the social services in 65 Swedish municipalities. The study shows that both clients and social workers estimate an increased problem level for the ASI domains of mental health and drugs. For alcohol, the trend is the opposite or is unchanged. Overall, interviewer as well as client estimates indicate an increased need for help and support for problems related to mental health and drug use, while the need for help and support for problems related to alcohol is unchanged or decreases over time among social service clients.

    A transformed landscape of care – The debate on e-health in Swedish specialist press 

    This article examines the debate on e-health in the Swedish medicine specialist press. E-health is not an entirely new phenomenon, yet digital care such as video meetings and chat via apps with medical personnel has increased rapidly in Sweden during the last years and the government’s goal is to provide the “world’s best e-health” by the year 2025. Implementation of digital technology in the public health care has transformed the ways in which the Swedish health system works, including what it means to be a patient and a provider of care. These transformations of care have led to an intense debate in the specialist press. The aim of this article is to examine how e-health as a concept is attributed different meanings in the Swedish medical professional press and to analyze the conflict lines that shape this formation of meaning. The method used is discourse analysis of articles published in two journals: Dagens Medicin and Läkartidningen. The study identifies three organizing metaphors in which meaning is established: e-health as competition, e-health as economy, and marketization of e-health as an ethical problem. These organizing metaphors were all connected to possibilities and problems in the digital health care system such as more flexible and accessible care, increased costs, aggressive marketing from private e-health providers, and conflicts of interest among health providers. The analysis shows how these organizing metaphors construct not only meanings of e-health but also what it means to be a patient and a care giver and in the current public health care system in Sweden.

    School non-attendence is often a sign of a complex combination of different kinds of problems, which means that these children and young people are often in need of composite help from several different types of professions within various authorities. The purpose of the study was to examine how school authorities, the Social Services and Child and Adolescent Psychiatry (BUP) collaborate in their work with young people who do not go to school. The study comprised a thematic analysis of qualitative interviews with managerial representatives of the respective authorities. The empirical material consisted of 12 qualitative interviews with heads of units at BUP (5 individuals), section managers from Social Services (3 individuals) and principals from compulsory schools (4 individuals) in three municipalities in western Sweden. According to the results, it seems problematic to manage the positions of power that may arise in collaboration between the parties. A position of power thus implies the right to make a desicion as a profession as well as activities that are related to each other. The right to make a desicion means the mandate to determine which measures should be put in place for the young people and their families. This study also shows that the parties should develop a common knowledge base that is a combination of educational, social and psychiatric perspectives. The common knowledge base can reduce the risk of power imbalance between the parties.

Spänningsfältet mellan organisatorisk specialisering och dess motpol integrering är utgångspunkten för denna artikel, som sammanfattar och diskuterar kunskapsläget när det gäller hur olika organisationslösningar inverkar på socialtjänstens klientarbete.

    Small rural municipalities face major challenges in the area of welfare. One of these challenges is about providing knowledge and expertise within social services. As a result of its small scale, social services in rural areas have an integrated organisation where social workers are supposed to work with a wide range of social problems. At the same time, there are expectations that they should have expertise in defined areas. This article focuses on how representatives of social services in eight rural municipalities view the opportunities to provide knowledge and expertise.

    Data collection was carried out within an action research project in connection with conferences and workshops. The results show that it can be challenging to develop knowledge and expertise. Since social services mainly have an integrated organisation, it is difficult to keep up-to-date on the state of knowledge in all areas. Another challenge is that it can be difficult to maintain skills in practice because certain types of cases are encountered relatively rarely.

    To deal with the challenges, different collaborative solutions are seen as a way to balance expectations on knowledge and expertise, and to balance the expectations of knowledge and expertise with the conditions that prevail in rural environments. The attitude towards different kinds of digital technology – for example web-based education and supervision – as tools for knowledge development and access to expertise is consistently positive. The results also indicate that digital technology has the potential to change the perspectives on the organisation of knowledge development and expertise within social services in rural municipalities.

    An overall interpretation of the results is that social services in rural areas must deal with different challenges in terms of knowledge and expertise than social services in larger and more densely populated areas. Therefore, the possible solutions are also different.

    Artikeln diskuterar och problematiserar de erfarenheter som unga vuxna personer med intellektuella funktionshinder ger uttryck för i relation till en organisatorisk och samhällelig kontext. En utgångspunkt i artikeln är spänningsförhållandet mellan funktionshinderpolitikens institutionella logik och de idéer, identiteter och föreställningar om framtiden som kommer till uttryck hos unga vuxna personer med intellektuella funktionshinder. Vi relaterar till ett bredare samhällsperspektiv och argumenterar för att nya livsstilsanspråk är nära förknippade med föreställningen om det senmoderna samhället och dess ökade krav på flexibilitet och ständigt återskapande av identiteter.

    This article attempts to elaborate on the scholarly discourse about normalisation in disability care. More specifically, the problem of imitation is addressed. The aim is to cast light on the tension between artificially created, “authentic” reality in activities that try to achieve nor-malisation for people with disability. The discussion is carried out by using examples from theatre for people with mental disability and home care for people with long-term mental ill-ness.

    Disability care in Swedish municipalities is described in terms of institutional logic. The logic of care is characterised by three sets of elements: regulative elements are made up of legislation, policy guidelines and organisational framework; normative elements like integra-tion and participation are grounded in national disability policy; cognitive elements are taken-for-granted values held by actors on the disability field, e.g. the value of work, not staying up all night, etc.

    When professionals undertake normalising activities, the logic of care is welded with a logic of everyday life. The underlying assumption appears to be that users benefit from lead-ing a life as similar to what is considered as normal as possible. The article probes deeper into two such logics, the logic of theatre and the logic of home. The logic of theatre stresses the artistic and creative elements of the work with disabled clients, which creates the opportunity to achieve alternative identities for users. The logic of home involves values such as a sense of control, the home as conveyor of identity, and providing a setting for important relations.

    Both theatre and home care involve institutional ambiguities. When welfare institutions attempt to imitate authentic everyday life, the assumption is that this would bring about posi-tive effects for clients. However, it is difficult to create this kind of authenticity within the therapeutic context of disability care – clients and outsiders are likely to see through the artifi-cial design of the situation. Occasionally, though, magic moments may occur when clients and staff engage in genuine activities that transcend the therapeutic purpose.

    However, given Giddens’ conception of late modernity, the problem of imitation may ap-pear less problematic. The design of normal-appearing environments in disability care seems to be similar to and no-less artificial than what is the case with normal people, in an age where individuals increasingly tend to reflexively create their identities.

    Införandet av en samlad elevhälsa har medfört att psykosocialt arbete inom skolan givits en tydligare position i skolornas elevhälsoarbete. Trots detta vittnar tidigare studier om att skolkuratorer uppfattar sitt professionella uppdrag och sina arbetsuppgifter som otydliga. Tidigare forskning har fokuserat på skolkuratorers egen beskrivning av sitt professionella uppdrag och arbetsuppgifter. Syftet med denna artikel är att studera skolhuvudmäns beskrivning av skolkuratorers uppdrag, arbetsuppgifter, organisation samt kvalifikationer och färdigheter. Studiens empiriska material utgörs av samtliga platsannonser som publicerades hos Arbetsförmedlingen under 2017 där skolhuvudmän sökte efter skolkuratorer. I studien användes kvalitativ innehållsanalys med en tematisk ansats. Resultatet visar att skolhuvudmännen i platsannonserna beskriver en tydlig skolkurativ jurisdiktion men att denna är under- eller sidoordnad den pedagogiska verksamhetens mål, organisation och styrning.

    En klusteranalys av intervjuarskattningar från ASI-intervjuns sju olika livsområden för 12833 klienter visade att klienterna kan delas in i tre olika klientprofiler: Narkotikaprofil, Avgränsade alkoholproblem och Alkohol och psykiska problem. I artikeln jämförs de tre profilerna med avseende på missbruksmönster, demografiska faktorer, sociala och psykiska problem, hälsa och immigrationsstatus.

    I den här artikeln presenteras huvuddragen i en domänteori för organiseringen av socialt arbete i offentlig sektor. Syftet är att beskriva – ur ett nordiskt perspektiv – dels hur socialarbetares professionsutövning formas av politik och förvaltning i organisationer för socialt arbete på den lokala nivån, dels hur denna process på lokal nivå i viktiga avseenden villkoras och styrs av övergripande strukturella och institutionella förhållanden på nationell och internationell nivå. 

    Vi konstaterar att offentliga arenor för socialt arbete konstitueras av tre domäner med olika uppgifter och logiker, men samtidigt inbördes sammanbundna och beroende av varandra: politikens domän, förvaltningens domän och professionens domän. De enskilda organisationerna på lokal nivå – konstituerade av de nämnda domänerna med sina respektive logiker – villkoras i sin tur av en fjärde domän: de institutionella villkorens domän. Det handlar om styrningsideal, normer, kunskapssyner och teknologier som råder i samhället, som legitimeras och sprids av centrala institutioner och som styr hur de enskilda organisationerna fungerar. 

    En slutsats i artikeln är att forskning om organisering av socialt arbete i offentlig sektor har mycket att vinna på en flerdisciplinär ansats som länkar samman domänerna politik, förvaltning och profession, och som även beaktar de institutionella villkor i samhället som styr vad som sker på den lokala nivån.

    In earlier research about compulsory community care, two contradictions related to the legislation have been identified: First, compulsory community care (CCC) means both more and less autonomy for the patient. Second, it means that autonomous individuals are being coerced to autonomy. The aim of the article is to clarify the normative underpinning of the law in light of four perspectives of autonomy to that help understand the two contradictions.

    The first contradiction can be explained by the fact that the legislator operates with two different definitions of autonomy: liberalistic and bioethical. In a liberalistic perspective, CCC restricts the right to autonomy. From a bioethical perspective, CCC potentially increases autonomy since it may enhance or protect the future autonomy of the patient. According to this perspective, other values, like health and quality of life, can also motivate restriction of autonomy.

    The second contradiction can be explained by how the legislator confuses procedural with substantial conceptions of autonomy. Since the patient is not assumed to be incompetent, the patient is assumed to have procedural autonomy. At the same time, whether or not the patient’s will is accepted depends on the eventual outcome of the patient’s choice. If the patient refuses care, her opinion is not presumed authentic, which is an expression of a substantive conception of autonomy. The coercive care provided is supposed to re-install the patient’s substantive autonomy. When it comes to the practice of CCC, the patient is assumed to voluntarily accept the coercive elements. This means that the patient is also autonomous according to a substantive conception of autonomy. Since the objective of compulsory care is to enable the patient to accept care voluntarily and thus restoring her/his authentic opinion, the legal requisites for allowing and discharging from CCC are met at the same time.

    If compulsory community care is to be acceptable from a bioethical perspective, it must provide the patient with actual benefits or more autonomy. There is no research evidence that compulsory community works. Accordingly, it is hard to identify what benefits CCC provide that can trump the autonomy of the patient.
