    Abstract [en]

    The aim of the article is to analyse and problematize care managers’ argumentation of individual needs when assessing migrant customers within Swedish public elderly home care services. According to legislation, need for assistance should always be assessed individually. In recent decades of service decline, standardization of needs has been part of the distribution of elderly care services. Simultaneously, the stratum of older people has become more diverse, which means that a wider spectrum of needs is presented to the care managers. Ten interviews with care managers were analysed with a focus on their arguments regarding the care needs of elderly people with migrant backgrounds seeking assistance. The theoretical conceptions of the two perspectives of care and services were used to analyse the understanding of care managers’ arguments. Our analysis shows that the service perspective with assistance and support that are standardized for Swedes is poorly suited to people born outside Sweden and socialized into other expectations of family and society. The meaning of individual needs was imprecise and contradictory when confronted with diverse cultural codes. When general standards were used in the arguments by referring to regulations and local guidelines, immigrants’ individual needs were not prioritized. Our analysis suggests that the tension between national standards, local guidelines and individual needs has to be discussed further, where theoretical tools related to care and services may be useful in guiding care managers’ professional arguments of individual needs and cultural sensitivity in the needs assessment.

    Abstract [en]

    This article aims to explore and analyse how good and dignified care is perceived and expressed at night-time within elder home care services, in which night-time care represents a knowledge gap. Dignity has become a legislated value in Swedish eldercare, aiming to increase the quality of care and to clarify the ethical values of everyday care practice. The data presented here come from a qualitative case study with in-depth interviews with six care unit managers and 14 care workers in four municipalities. The analysis of the interviewees’ perceptions and expressions of good care were informed by Nodding’s concepts: responsiveness, receptivity, and relatedness. The results showed that there was a relative unawareness of the new goals of the dignity policy and there was no specific guidance regarding dignity during night-time care. The care unit managers’ perspective was mainly administrative and related to the policy level and the staff’s ability to care. The care workers’ view of good and dignified care included aspects of ideal characteristics and user-centredness with a focus on older people’s individual needs. However, good care was conditioned by time. The dignity policy, as described in national documents, was perceived by the interviewees as vague and with unreachable goals constructed on the structural level. In social care practice, however, expressions of good and dignified care were already found in care ethics, regardless of the dignity policy. By bringing relationality to the dignity discourse on the structural policy level, recognition of care may be emphasized.

    Abstract [en]

    The aim of this study was to describe and analyse structural frames for choice, as well as characteristics of a free-choice market, implemented in community mental health services in Sweden. Day centres were focused. A case study was conducted. Documents were collected and semi-structured interviews were conducted with twenty-eight agents involved in the implementation process. Content analysis of data showed that users’ freedom of choice was influenced by detailed regulation. Freedom of choice was extended, but it was also substantially reduced. The gain or loss of freedom of choice depended on which aspects of the system were emphasised. Features, which could be interpreted as paternalistic, were once again structurally embedded, despite a system addressing individual freedom of choice. Further, the market did not seem to be adjusted for the average user of community mental health services.

    Abstract [en]

    The aim of this study was to examine how employed descendants of immigrants in Sweden perceive that interactions with public officials have benefitted their occupational aspirations and attainments. We conducted qualitative semi-structured interviews with twelve female and nine male descendants of immigrants, followed by an abductive thematic analysis. Applying a theoretical framework of social capital, we found three main influences of public officials from respondents’ perspectives: 1) connectedness, 2) supporting personal goals and focusing on possibilities, and 3) mediation of knowledge and information. We discuss and analyse the symbolic resources deriving from these influences, e.g. increased motivation and self-belief, and conceptualize these resources as social capital contributing to the occupational aspirations and attainments of immigrants’ descendants.

    Abstract [en]

    This article presents an outline of a theory of stigmatization, consisting of theoretical propositions and a graphic model, which can help describe and explain stigmatization processes in the personal social services. The building blocks of the theory originate from existing theories and previous empirical research including our own study of stigmatization processes among social workers in Sweden and the United States. Pattern matching methodology was key to identifying a need for a new theory. Contributions to existing theory and potential implications for policy and practice are discussed, especially in relation to social work in the personal social services. The outlined theory contributes to existing stigma theory in several ways, of which three key findings are highlighted. The theory suggests: 1) that stigmatization processes can go in several directions, sometimes simultaneously; 2) that stigmatization processes can involve mutual reinforcement which can develop into a negative spiral; 3) that stigma can be placed upon social work as an institution. The outlined theory can be used as a starting point for analysing stigmatization processes in social work in several contexts, such as different institutional or organizational settings. We argue that existing stigma theory does not always provide sufficiently deep or specific explanations that are appropriate for the uniqueness of different settings. Thus, there is a need for a theory that is more precise and adapted to such a setting. The framework can be useful for researchers and university students that need a theory when studying stigma processes in social work practice.

    Abstract [en]

    The aim of this study was to examine changes in an organizational field over time when implementing a freedom of choice model. Main focus was on the range and characteristics of providers and services. Our findings suggest that the expected effects of the reform in terms of variety of providers and services within the organizational field did not materialise due to a lack of market competition. Providers complemented each other rather than competed with each other, and the logics of care, choice, and advocacy challenged each other within the quasi-market. All informants described financial conditions in the market as extremely strained. Strong personal commitment characterized providers entering and exiting the market. A gap was found between policy goals and experiences among stakeholders, and efficiency was found to be the policy goal achieved most often in practice. This raises the questions of how the Swedish community mental health service market will develop in the future and what the implications are for the participants.

    Research in the field of human services has paid limited attention to the consequences of the international development toward organisational specialisation; the consequences for clients have been overlooked in particular. The aim of the article is to describe and analyse the consequences of organizationally specialised personal social services (PSS) in Sweden for clients with complex needs. Findings from a survey and an interview study addressing PSS clients show that clients primarily perceived their encounters with the specialised PSS negatively, and that they experienced several elements of service fragmentation. Implications of the findings are discussed in light of previous research and theory in the area.

    This article highlights organizational structure as a factor influencing conditions for helping relations. It is based on a survey study and an interview study, both directed at parents in families that have parallel contacts in different parts of Swedish personal social services (PSS). The aim is to describe and analyse conditions for helping relations when clients with complex needs encounter specialized PSS.

    Low system trust, people processing dimensions of work, and an organizational and a professional emphasis on formal organizational structures and boundaries were found to constitute unfavourable conditions. Conversely, an occurrence of individual trust, people sustaining and people changing dimensions of work, as well as informal organization and individual social workers’ boundary spanning efforts, constituted favourable conditions. The article concludes that greater understanding of how clients are affected by contextual service conditions can give some pointers towards how to generally improve services for vulnerable and disadvantaged populations.

    Abstract [en]

    This paper elucidates how Swedish child welfare social workers manage complexity co-occuring in the families with which they work and the organisational contexts of practice. Focus groups were held with social workers in three municipalities in Sweden who described work processes generally and in response to a fictitious vignette. The vignette was constructed as a complex family situation to explore how social workers approach complexity when faced with a family with complex needs. Findings showed that social workers are challenged in their everyday work where they are aware of the many needs in a family. They focus on immediate conditions for children while recognising that some problems are less amenable to being solved. However they try to manage complexities related to families as well as the structural conditions of work by sorting, prioritising and oscillating between a child focus and a family service orientation. This paper serves as a necessary reminder of the complexity of social work in the broader area of child welfare and raises further questions about the use of comparative typologies to explain social work practices.

    Mental health problems among young adults often lead to obstacles to completing higher education and becoming established in the labour market. It is important to offer support that can help these individuals realize educational and vocational goals in order to become self-sufficient and achieve social inclusion. Individual Placement and Support (IPS) and Supported Education (SEd) are two methods developed for supporting people with mental health problems in attaining competitive employment and reaching educational goals. These methods have not been widely implemented in Sweden, therefore making it interesting to study management perspectives in organizations offering IPS/SEd. The study aimed to explore the reasoning and strategies managers described in offering vocational and educational support to young adults with mental health problems. This study is based on 12 qualitative interviews with managers from six units in Sweden offering support through IPS/SEd. Results show a variety in interpretation of legislation and responsibility for young adults with mental health problems. The managers were positioned between policy and practice, having to navigate between the organizational restraints and their professional assessment in offering this support and pushing traditional boundaries in prioritizing long-term individual goals over short-term economic and political considerations. These managers demonstrate the possibilities for how IPS/SEd can be implemented and made to fit different organizational contexts, even within a highly sectorized welfare system as in Sweden. However, the methods might need to be adapted and adjusted to not only fit local organizational restraints, but also to fit into the Swedish welfare system.

    Over the past 20 years, the Swedish government and national authorities have organized a structure for knowledge governance in order to implement an evidence-based practice (EBP) for social services. Since the idea of a knowledge governance has not been implemented as expected, it was considered important to develop regional collaboration and support structures (RCSSs) as a link between national and local authorities. In this article, we discuss changes in ideas on knowledge governance at the national level, and also changes in expectations on RCSSs function in knowledge governance of the Swedish social service. Changes in governance also change the prerequisites for implementation of EBP. Therefore, it is important to further examine and analyse ideas on knowledge governance at the national level and whether they change over time. The empirical data consists of official documents and interviews with representatives of key actors at national level. A qualitative content analysis of the interviews was then performed and also an analysis of the official document was used to describe the changes on knowledge governance. This article shows that ideas on knowledge governance has changed over time, from an emphasis on top-down governance towards a more dialogue-based governance. This shift in governance ideals also appears to have had an impact on expectations regarding RCSSs, through an increased emphasis on their collaborative function in relation to both national and local levels. The knowledge governance and the function of the RCSSs appear to be fragile, as this collaboration is based on voluntary partnership and networks.

    This article reports on a study the aim of which was to sketch a tentative model for illuminating variations in the professional role of a social worker, based on Swedish social work students' own experiences of knowledge use and its premises in client meetings during field practice. Social work students voluntarily submitted narratives depicting problematic and unproblematic situations experienced during field practice in one of three broad occupational positions of social work: casework; counselling; and social assistance. Three different formative aspects were identified as constituting premises for knowledge use in social work. Knowledge use from differing epistemological domains are set in relation to the identified formative aspects, all together comprising a role that we label 'the Compassionate Bureaucrat'.

    Abstract [en]

    This article describes the counselling practice of school social workers (SSWs) with individual children. The work of SSWs is mainly about helping children and adolescents who for various reasons suffer from mental ill-health, school absenteeism or stress that make them unable to achieve their intended goals. SSWs preferably meet these children in individual counselling sessions. The aim of this article is to describe and analyse SSWs’ experience of counselling with children and to examine the characteristics of counselling practice. The data collection was conducted through four semi-structured focus group interviews with a total of 22 SSWs in four different regions in Sweden. SSWs provide counselling to children in order to bring about improved feelings or behavioural changes. It can be noted that SSWs put emphasis on both the counselling process and the alliance with the child. The interviews showed a common practice among SSWs regarding the structure of the counselling sessions, with certain steps and approaches being employed. However, the specific interventions differed and were characterized by an eclectic standpoint in which SSWs utilize a broad repertoire of therapeutic schools and techniques. Furthermore, a relational perspective emerged as a most prominent focus for the SSWs by re-emerging throughout the material. We believe that SSWs could benefit from theoretical perspectives on ‘contextual model’ and ‘attachment theory’ as ‘models of the mind’. Being emotionally close to the child and being able to follow their development requires a lot from SSWs, as both professional caregivers and as ‘safe havens’.

    Abstract [en]

    The increase in private shelters for women escaping men’s violence in Sweden prompts an analysis of discursive struggles on separatism, violence, and safety. The analysis of websites and interviews with representatives from private shelters and women’s shelters show that the woman-to-woman approach is important for women’s shelters whereas private shelters frame their use of male staff as a practical necessity or a way to show women that there also exist ‘good’ men. Women’s shelters articulate the importance of knowledge about men’s violence for counteracting normalization of violence and self-blame, while private shelters emphasize therapeutic knowledge and interventions. To provide safety, women’s shelters articulate the importance of a homey atmosphere and ‘inner safety’, where private shelters emphasize security; shell protection, perpetrator profiles, and risk assessments. The women’s shelters position their work within a discourse of social change, whereas private shelters emphasize their lack of political ambitions, with profit-making as their primary motivation. Should public funding continue to be funnelled to private shelters, rather than to women’s shelters, it will undermine women’s shelter’s dual role of providing refuge and contributing to social change. Should the private shelters discourse prevail it will likely alter the support provided to victims of violence.

    From the literature we learn that social workers in health care are exposed to different stressors connected to the risk for the development of burnout or traumatization. However, there is a lack of studies that focuses on the social workers’ own narratives on the burdensome, but also the rewarding aspects, of social work in health care. This study focuses on social work in oncology. Thematically structured qualitative interviews were conducted in a narrative form with 20 oncology social workers, half of whom were less experienced and half of whom were more experienced and with additional training in psychotherapy. The interviews focused on ‘pros and cons of practicing social work’ and the results were categorized by means of the similarities-differences technique. The burdensome cons concerned ‘Organizational and professional barriers’ and ‘Demanding cases’, with the latter divided into ‘Impasse because of hopelessness’ and ‘Impasse because of helplessness’. The rewarding pros categories all concerned meaningfulness including ‘Organizational meaningfulness’, ‘Meaningfulness from giving’, ‘Meaningfulness from receiving’, and ‘Meaningfulness from personal development’. Our findings indicate that the burdensome and rewarding aspects are two sides of the same coin. They are interconnected in the sense that the strain and challenge of being in situations of psychological despair also imply emotional satisfaction due to an experience of meaningfulness.

    The meaning of work is different for people in different social groups, and research exploring perceptions of meaningful work among adults with high-functioning autism (HFA) is scarce. The aim of this paper is to explore how narratives of satisfactory and meaningful future jobs are portrayed relative to two alternative discourses of work – the 'obligation to work' discourse, and the 'disability rights' discourse. This group of individuals are high functioning on the one hand, while at the same time holding legally-mandated special disability rights, an exploration of how this group reason about work-life and satisfactory jobs is particularly interesting. Through ethnographic fieldwork in Sweden, seven young adults with HFA were followed, and 17 interviews were conducted. The main findings and conclusions are that jobs that are individually assessed to be ‘ideal’, are put aside in favor of jobs that are more compatible with general labor market demands, as long as enjoyment and meaningfulness can still be experienced.

    Abstract [en]

    User-focused monitoring (UFM) is an emerging method for user involvement and quality development in the Swedish mental health sector. It involves people with lived experience conducting a systematized evaluation of a mental health service or support programme. The aim of this study is to analyse motives and central concerns when initiating a UFM project. Five UFM projects in Sweden were sampled, 16 semi-structured interviews were performed with user monitors and commissioners and analysed using conventional content analysis. Our results showcase a range of motives for conducting UFM, illustrate the centrality of trust in the relationships of the UFM practice and the varying degrees of user autonomy in UFM projects. We further discuss our results in relation to epistemic trust and organizational learning theories. Accountability processes are discussed as important for securing continuous trust between the stakeholders. Furthermore, trust is required for UFM being applied to engage in continuous learning and review of organizational norms and goals together with service users.

    The aim of this article is to map the field of Swedish mental health service user organisations (MHSUOs) with a focus on organisational characteristics, positions and relationships. This mapping enables us to discuss these organisations’ repertoires of contention and their connections to governmental actors. Annual reports, organisational by-laws, and financial reports were collected for each of the 12 MHSUOs and the two network organisations that were included in the study. The empirical material was analysed according to the organisations’ size, activities, target groups, relationships and main knowledge base. Developments towards professionalization and hybridisation are evident within the field, and repertoires of contention are focused on advocacy and educational activities, with organisations providing experiential knowledge as a service to external actors. We further discuss how close ties to governmental actors and a consensus-oriented approach is related to risks of failure in recognising the conflicts that social mobilisation presupposes. It is crucial to strengthen MHSUOs independence, financial and otherwise, in order for these organisations to remain responsive to demands from the collective of service users.

    This article presents findings from a survey conducted on all school counsellors in Sweden. The focus of the article is on Swedish school counsellors’ demographics, professional practice, educational background, working life history and professional autonomy. The survey was web-based and contained 91 questions of both quantitative and qualitative nature. The results show that Swedish school counsellors are predominantly experienced female social workers with academic education in social work and several years’ experience of social work in areas other than school. Employment as a school counsellor is not an entry-level job. The professional practice of Swedish school counsellors predominately focus on remedial work with individual students. Their professional practice focus to a lesser extent on preventive and health promoting work despite the prominent position that preventive and health promoting work are given in the Swedish school act and in the national guidelines. The professional focus of Swedish school counsellors raises questions whether the emphasis on individual remedial work is to be interpreted as a lack of organizational awareness and resources, or if it is an expression of a professional ‘heritage’ from school counsellors prior experience of professional social work within other welfare areas were remedial work are custom. We argue that the focus on remedial work makes it valid to describe the character of Swedish school counsellors’ professional practice as a reflection of Swedish social services in general.

    Each year a large number of children are exposed to violence in their homes, for example, by witnessing one parent using violence against the other, or experiencing violence directed at them personally. As a result, together with a parent, often the mother, the children might need to flee from the violence to a domestic violence shelter. The present study is set in a Swedish context and aims to analyse mothers’ narratives of the initial time at a domestic violence shelter for mothers and their young children (aged 0–6), with a focus on children’s right to protection and participation. The study consists of interviews with thirteen mothers, which are analysed using thematic analysis. The findings show that the children rarely know why they are at the shelter, and that it is an unfamiliar place for them, as they have little knowledge of what a shelter is. The children also seem to be afraid of losing their mothers, which is expressed in how they monitor their mothers. Based on the findings, the children need to build trustful relationships, a process that takes time but can already begin on the first day at the shelter. The first day signifies the start of something different, which means that the mothers and children need time to settle into their new lives.
