    Abstract [sv]

    I den offentliga omsorgen om äldre och personer med funktionshinder arbetar i dag personal som tillsammans utgör det största yrkesområdet i Sverige. I rapporten studeras hur intentionerna med att höja utbildningsnivån för äldreomsorgens personal genom att formalisera all kompetens kring en utbildning lyckats. Till grund för analysen ligger enkätdata samt statistik från SCB.

    Undersökningen visar att gymnasieskolans omvårdnadsprogram inte som enda aktör kan utbilda personal i en omfattning som motsvarar de framtida rekryteringsbehoven. Detta trots att omvårdnadsprogrammet har en god genomströmning och trots att en stor andel av eleverna arbetar inom avsedda målgruppsområden efter avslutade studier. Av de som avgick från omvårdnadsprogrammet 2000 och som finns i arbete 2008, arbetade 78 procent i ett målgruppsområde, de flesta som vård- och omsorgspersonal (SSYK 513) följt av sjukstöterskor (SSYK 223 och 323).

    Rapporten visar att utbildningen på omvårdnadsprogrammet har förberett eleverna för kommande arbetsliv på ett bra sätt, ett påstående som både de som arbetade och de som inte arbetade inom vård- och omsorg instämde i. Däremot råder det en stor variation bland de som arbetade inom vård- och omsorgsfältet vad gäller om kunskapen tas tillvara på den nuvarande arbetsplatsen.

    Abstract [en]

    An important aim for Swedish elderly care is to make possible for frail elderly people to remain living in their own homes since home, it is argued, is a vital resource for an independent ageing. Our interviews with elderly with home-based community care show however that the importance that is attributed to home exceeds the notion of home as a resource. Home has a more profound meaning; it is seen as a source of independence and an expression of identity. In the article we argue, that the shift in perspective, from home as a resource to home as source of independence and an expression of identity, carries welfare political implications. The perspective on home as a source of independence surpass the prevailing notion of independence as something that a person achieve outside of home. The emphasis put on home by the elderly, as a source of independence and an expression of identity, points to the need and importance of services that address elderly peoples difficulties uphold their sense of home.

    Abstract [en]

    Aim.  This paper is a report of a study exploring possible external and inner sources of the sense of security felt by older people aged 65 and 75.

    Background.  The concept of ‘security’ has been parsimoniously defined and explored, especially as pertains to older people, and previous research has primarily focused on risks in relation to individuals’ declining health and functional ability.

    Methods.  In 2005, population-based cross-sectional surveys were carried out in 15 municipalities in Västerbotten, Sweden and 18 municipalities in Ostrobothnia, Finland. A 15-page questionnaire (84 questions) was distributed, with the final total response rate reaching 68·4% (n = 3370): 1825 Swedish-speaking Swedes, 926 Swedish-speaking Finns and 621 Finnish-speaking Finns. The statistical analysis was three-stage, including nominal stepwise regressions.

    Results.  Older people who indicate that they are ‘very secure’ are more likely to consider life meaningful and are able to master various life crises. Moreover, they possess confidence in economic institutions and are more likely to feel economically secure. Weak trust in family and friends or neighbours and decreased functional ability seem to indicate a subjective feeling of ‘insecurity’.

    Conclusion. Caregivers in health care should focus more on supporting older persons’ meaningfulness in life and how their feelings of security can be strengthened by trusting relationships and activities. In nursing, there should be more focus on developing knowledge of inner health resources, including positive life orientation and meaningfulness in life.

    Abstract [en]

    In recent years, researchers have paid increasing attention to the issue of care in the new types of family and partner relationships. The aim of this study is to examine expectations and attitudes concerning care among elderly people in LAT (Living Apart Together)- relationships in Sweden, that is, people who have a long-term intimate relationship, but who do not share a common home. Questionnaires were completed by 116 elderly people in LAT-relationships. Partners are ranked as the main providers of care, especially by men, but there is considerable variation in the answers. This seems to be principally related to the degree of flexibility in this type of relationship. Some individuals see a LAT-relationship as a marriage-like relationship, while others see it primarily as a type of relationship that guarantees them the possibility of maintaining their own independent way of life.

    Abstract [en]

    The chapter deals with the question how old pensions and care work for old people have been discussed earlier and at presnt in Sweden. The chapeter starts with a brief overview if the Swedish welfare model, focussed on two distinct periods: the 1950s - 1960s with a rapid econmic growth and the present period of economic decline. After a long expansion period of a tax - financed public sector under the name of "productive welfare strategy", calculations indicated a state economy in imbalance. Resources did not increase at the same pace as the numbers of elderly. An organizationnal reconstruction of the public sector started with market solutions, encouragement of an increase in voluntary work and family based care. A new pensions system was introduced in the 1990s, and higher expectations of quality at a lower cost turned questions of staff teaining and qualifications to highly strategical themes. A new anti-discrimination Act came into force in 2009. The chapter is finalized by a presentation of some of the research performed within the research profile Care Work and Social Gerontology at the Department of Social Work at Umea University. Issues like social capital and care work, old womens life stories, family obligations in transition, attitudes to old people including ageism as well as training for care workers are issues researched within the team.     Abstract [en]

    Expectations for older people’s active participation, both in society and in their private lives, have increased in comparison with previous attitudes, which considered them as more dependent. The elderly today are expected to take responsibility for their own health and well-being. In this chapter, contradictory perspectives related to older people and their engagement in the community will be addressed.

    Material from a NETWORK-study has been re-interpreted in order to clarify how attitudes and norms have changed since the original material was collected in the early 1990s. Further material from a survey of people aged 65 and 75 years was collected 15 years later in the Ostrobothnia Region, and the county of Västerbotten, the GERDA-study. A third set of material was collected in an Australian-Swedish project called the SOCIAL CAPITAL-study with the aim of comparing the social capital generated through volunteering in the two countries. All three materials focus on rural areas, allowing for a comparison to the predominant urban research.

    In this chapter, these projects which all were included in a campaign called ‘All Sweden shall live’ will be analyzed. A demographic perspective will bring a special dimension to the understanding. From other studies from small communities where public care is either absent or insufficient, we know that a reciprocal exchange between friends or extended families is developed. Social movements and special interest organisations have acted as a driving force in the fight for equal treatment and political equality, sometimes contradictory to the Scandinavian welfare model of extended public services. The analysis opens for interesting new insights, including a finding related to unexpected gender roles. The interpretation is that under such circumstances gender is re-negotiated.

    In the chapter formal and informal activities are analysed in relation to health as well as in relation to motives for engagement. The chapter ends with a discussion about the “ageing friendly community”.

    Abstract [en]

    Ageing populations comprise of many frail individuals who need care and support. In some cases, this need exceeds the help that families can provide. When this happens, social care services come into play. However, the help and support that older people need is not uniform. The challenges that older people face differ, and so do the support strategies that older people prefer. Consequently, social care providers need a deeper understanding of life-courses if they want to design care services that suit the older individuals’ needs and that can easily be understood and accepted by the older individuals. This chapter explains how social care providers can learn from the life-courses of the care-recipients, using older migrants in Sweden as an example.
