    Abstract [en]

    Objective: The objective of this study was to from a gender perspective, explore elementary school teacher' experiences of their work situation, and identify conditions that could be health risks. Participants: Eighteen female teachers who work in an elementary school in Northern Sweden. Method: Thematic interviews were conducted using an interview guide. The interviews were tape-recorded and transcribed verbatim. Qualitative content analysis was used to analyse the transcribed text and interpretations were made within gender theory. Results: Four categories emerged: "Squeezed between dream and reality", "Effort to keep up with demands", "We can make it together" and "The school needs men's qualities". The categories were linked together with the theme "A struggle for time to teach". The theme describes the conflict between the teachers' ambitions to teach and create a stimulating learning environment versus the increased need for behaviour control that took time from classroom work. Beside work at the school, the teachers carried a large burden of domestic work. Conclusions: Teachers' work includes both endless demands and great joy. Their work is structured within the schools gender system in which caring duties are subordinated despite a growing demand for behaviour control. Traditional gender roles affect their domestic work load.

    Abstract [en]

    Using a multilevel approach, this study explores how job demands and control are dependent on the organizational context in which work is performed. The data set consisted of 3,485 employees at 51 establishments divided into 141 sub-units and belonging to 10 parent organizations. Data were collected by means of a survey answered by the employees and structured interviews conducted with operative managers at participating establishments. The results showed that a significant proportion of the variance in job demands and control was attributed to the organization, and that the three organizational levels varied in terms of their impact on the two psychosocial dimensions. More specifically, job demands were mostly affected by the establishment level, while the sub-unit level seemed to be the most important for job control. It is concluded that in studies of working conditions, the organizational context should be considered.

    There is conflicting evidence on the extent that medical conditions, particularly musculoskeletal conditions related to work, cause disability and premature retirement in dentists. Reports based on data from disability insurance in the United States suggest dentists are not susceptible to work related musculoskeletal disability. Surveys of symptom rated debility suggest higher rates of dysfunction, however, as do compulsory employment injury reports from European countries. These data, including information on Swedish dentists, analyzed for this study, tend to put dentists at the higher end of health care professionals in terms of musculoskeletal injury and lost work time. Because compensation patterns and proprietorship vary between national systems, the relationship between exposure and injury and retirement from the active work force may include differing national characteristics.

    BACKGROUND: It is possible that people with psychiatric disabilities who visit day centres have previous work experiences that may be seen as resources for their current engagement in day centre activities. Research in this respect seems to lack, however.

    OBJECTIVE: To investigate work experiences among attendees at day centres for people with psychiatric disabilities and relationships with current type of day centre (work-oriented, meeting place-oriented or mixed), engagement in day centre activities, motivation and socio-demographic and health-related factors.

    METHODS: Seventy-seven attendees responded to questionnaires. Global Assessment of Functioning, GAF, was also used. Work was categorised into Group I (professionals, semi-professionals), Group II (clerical support, services workers) and Group III (e.g. craft workers, elementary occupations).

    RESULTS: Almost everyone had previously had open-market employment; more than half for ≥10 years. Group I was more common in mixed centres, Group II in meeting place-oriented ones and Group III in work-oriented ones. Group I more frequently had college degree and was rated high on GAF functioning. Women were over-represented in Group II, and men in Group III and in meeting place-oriented centres. Attending mixed centres was more likely when having a college degree, scoring high on GAF functioning and being highly engaged in activities. Attendees at work-oriented day centres were characterised by being motivated for spending time alone and reporting a diagnosis of psychosis.

    CONCLUSIONS: The participants had unused working capacity. No clear-cut relationships were found between work experiences and the investigated correlates.

    In order to test the feasibility and sensitivity of the ergonomic exposure assessment tool Quick Exposure Check (QEC), a pilot-study was conducted. The aim was to test QEC in different occupational groups to compare the exposure in the most common work task with the exposure in the work task perceived as the most strenuous for the neck/shoulder region, and to test intra-observer reliability. One experienced ergonomist observed 23 workers. The mean observation time was 45 minutes, waiting time and time for complementary questions included. The exposure scores varied between the different occupational groups as well as between workers within the occupational groups. Eighteen workers rated their most common work task as also being the most strenuous for the neck/shoulder region. For the remaining five workers, the mean exposure score were higher both for the neck and shoulder/arm in the most common work task. Intra-observer reliability shows agreement in 86% of the exposure interactions in the neck and in 71% in the shoulder/arm. QEC seems to fulfill the expectations of being a quick, sensible and practical exposure assessment tool that covers physical risk factors in the neck, upper extremities and low back.

    Background: Despite their disadvantaged labour market position, women consistently report higher levels of job satisfaction than men. Researchers have attributed women's higher job satisfaction to their lower expectations, arguing that gender differences will fade away as women's labour market prospects improve. Others, however, argue that women are more contented than men because their jobs satisfy a need for family adaptions.

    Objective: In this article, we put the hypotheses of transitions and trade-offs to a strong test, by comparing men and women with comparable human capital investments living in a country where women's employment is strongly supported by policies, practices and social norms.

    Methods: The relationship between gender and job satisfaction is analysed with stepwise OLS regressions. The analysis is based on a survey to newly graduated highly educated men and women in five occupations in Sweden (n≈2 450).

    Results: First, we show that, after controlling for a range of job characteristics, women report a higher level of job satisfaction than men. Second, although the paradox appears to be surprisingly persistent, it cannot be attributed to work-family trade-offs.

    Conclusions: Future research should consider job satisfaction more broadly in the light of gender role socialization and persistent gender inequalities.

    There are increasing numbers of people on long-term sick leave, especially with stress-related ill health and musculoskeletal disorders. The main purpose of this study is to, early on in the sick leave, find predictors of individual resources for a return to work. The study group consists of women and men on sick leave compared with reference groups of healthy persons. A questionnaire was used regarding diagnosis given on the doctor's certificate, self-reported symptoms, residential area, education, and kind of work. It also contained open questions about consequences in daily life and beliefs about future. Checklists concerning life satisfaction, sense of coherence, locus of control, coping resources and sick leave information over a four-year period have been used. Significant differences between the study group and the reference groups were found, relating to sense of coherence, locus of control, life satisfaction and coping resources. More than half of the study group were still on sick leave at a two-year follow-up. Important predictive factors turned out to be: previous sick leave, own belief about future and self-reported symptoms.

    This paper contributes to the understanding of the factors that are significant for returning to work, and identifies factors that might be used early on in a period of sick leave to discern whether people are likely to work again. In the design, the multivariate Partial Least Squares (PLS) of Latent Structures method was used to analyse information from a questionnaire containing socio-demographic items, and information on symptoms, consequences on daily life, expectations and psychosocial factors. Data about the incidence of sickness over a four-year period was included. 121 adults aged 18-64 years on sick leave participated, irrespective of their diagnoses. A reliable prediction of a return to work required the combination of many factors: individual psychosocial instruments are not useful when considered in isolation. The strongest predictive factors for a return to work concern the individuals' expectations, the number of days of sick leave taken in the past, somatic disorders, and a high level of life satisfaction and sense of coherence. Many factors influence the outcome for people on sick leave: PLS analysis demonstrated that a multivariate approach using this method could predict the long-term outcome early on in a period of sick leave.

    BACKGROUND: Poor co-operation between the various stakeholders underscores the need for reviewing important factors that facilitate return to work (RTW) after stroke.

    OBJECTIVE: To explore and describe important aspects expressed by Swedish stakeholders in the RTW process for persons post stroke and to contrast the stakeholders’ aspects exploring different perspectives that may influence optimal RTW.

    METHODS: Data from seven focus group interviews with the stakeholders were analyzed using qualitative content analysis.

    RESULTS: Three main categories made up the findings; the prolonged RTW process, the need for extended knowledge, and the assessment of work ability in the RTW process. Despite the stakeholders’ agreement on these important aspects, they presented divergent views of the categories, representing their diverse agendas.

    CONCLUSIONS: A stroke coordinator may remediate the stakeholders’ divergent perspectives. The coordinator should be versed in stroke specific knowledge and in a person centered approach to inform the decision process regarding RTW, while finding optimal solutions within regulatory boundary conditions. A coordinator could increase collaboration of the various stakeholders, provide support and coordinate services for the client with stroke and the employer throughout the whole process facilitating RTW after stroke. Future studies are needed to confirm this proposal.

    The welfare sector in Sweden has undergone extensive changes during the last 15 years, and private and cooperative actors have entered the public market. In the light of high sick-leave rates, especially in female-dominated professions, it is important to identify factors that can help to improve the working conditions and promote health among employees. The purpose of this study was to compare how two of these factors, participation and control, are perceived by employees in three different forms of ownership: public, cooperative and private. In all, 186 employees working at seven geriatric care institutions with three ownership forms were invited to participate in the study. 82% responded to a questionnaire containing issues related to working conditions, e.g. control and participation. The one-way ANOVA and Kruskal-Wallis were used to analyse the findings among the three groups of employees working in public, cooperative or private setting. Results showed that employees in cooperatives experienced more participation than employees working in the public and private sectors in two out of four variables - employee's voice concerning work environment issues and sympathetic response from the manager and decision-making concerning work activities at large. As expected, there were no difference in perceived control between ownership forms, which might be explained by the fact that the work nature in geriatric care is rather regulated, restricted and formalized, regardless of ownership form, resulting in limited freedom over the work situation for the individual employee.

    In ergonomics, assessing the working postures of an individual by observation is a very common practice. The present study investigated whether monetary resources devoted to an observational study should preferably be invested in collecting many video recordings of the work, or in having several observers estimate postures from available videos multiple times. On the basis of a data set of observed working postures among hairdressers, necessary information in terms of posture variability, observer variability, and costs for recording and observing videos was entered into equations providing the total cost of data collection and the precision (informative value) of the resulting estimates of two variables: percentages time with the arm elevated <15 degrees and >90 degrees. In all 160 data collection strategies, differing with respect to the number of video recordings and the number of repeated observations of each recording, were simulated and compared for cost and precision. For both posture variables, the most cost-efficient strategy for a given budget was to engage 4 observers to look at available video recordings, rather than to have one observer look at more recordings. Since the latter strategy is the more common in ergonomics practice, we recommend reconsidering standard practice in observational posture assessment.

    OBJECTIVE: People with disabilities find it harder to enter the labour market than people without disabilities and those with a mental illness are, in relation to people with other disabilities, employed at an essentially lower extent. Many are effectively helped by the vocational rehabilitation model Individual Placement and Support (IPS), but there are still many individuals left in undesired unemployment. This study investigates potential predictors of the vocational outcomes of a one-year follow up of IPS in the north of Sweden. PARTICIPANTS: The participants were 65 men and women, mostly younger than 30 years of age diagnosed with a mental illness (predominantly with a diagnosis of anxiety and/or depression). METHODS: Baseline data related to sociodemographic and clinical characteristics of the client, the client's own perceptions of every day living and participation, self-esteem and quality of life, as well as the quantity of employment support, were investigated using analyses of logistic regression. RESULTS: Of three identified potential predictors, only psychiatric symptoms remained significant in the multivariate logistic regression analyses. A lower level of symptoms increased the odds with 5.5 for gaining employment during one year. CONCLUSIONS: Careful investigation of how psychiatric symptoms influence clients' occupational performance is of importance. By understanding essential aspects of the relationships between the clients' individual characteristics, the rehabilitation context and the vocational outcomes, more appropriate and effective interventions may be offered to the individual client.

    BACKGROUND: In aging societies, zest for work may be pivotal when deciding to stay occupationally active longer. Psychosocial work stress is a prevalent public health problem and may have an impact on zest for work. Work over-commitment (WOC) is a personal coping strategy for work stress with excessive striving and a health risk. However, the long-term effect of WOC on zest for work is poorly understood.

    OBJECTIVE: To investigate the age-related associations of work over-commitment with zest for work.

    METHODS: During 1996-1998 and 2000-2003, predominantly industrial workers (n = 2940) participated in the WOLF-Norrland study and responded to a questionnaire referring to socio-demographics, WOC, zest for work, effort-reward imbalance proxies, and mental health. Age-adjusted multiple logistic regressions were performed with original and imputed datasets.

    RESULTS: Cross-sectionally, work overcommitted middle-aged employees had an increased prevalence of poor zest for work compared to their contemporaries without WOC (OR: 3.74 [95%-CI 2.19; 6.40]). However, in a longitudinal analysis associations between onset of 'poor zest for work' and the WOC subscales 'need for approval' (OR: 3.29 [95%-CI 1.04; 10.37]) and 'inability to withdraw from work' (OR: 5.14 [95%-CI 1.32; 20.03]) were observed.

    CONCLUSION: The longitudinal findings among older employees could be relevant regarding the expected need to remain occupationally active longer.

    OBJECTIVES: To examine the levels of life satisfaction for patients with chronic obstructive disease (COPD) and to explore the relationships between life satisfaction, lung function, walking distance test/exercise capacity and quality of life. STUDY DESIGN: The population comprised 91 patients with COPD, 28 patients with COPD using long term oxygen therapy (LTOT) and a reference group (R) of 150 healthy individuals. Before the study, a number of the COPD patients had been tested with regard to spirometry, walking distance and quality of life using the Chronic Respiratory Disease Questionnaire (CRQ). All subjects filled in an eight-item checklist on levels of life satisfaction. RESULTS: Significantly lower levels of satisfaction were reported by both patient groups than the R group for satisfaction with life as a whole and satisfaction derived from vocational/occupational situation, sexual life and ADL, and by the LTOT group, furthermore, also with family life. Only 10% of the patients were satisfied with their health. The LTOT group reported significantly lower levels of satisfaction than the COPD group for life as a whole, satisfaction with their vocational/occupational situation, leisure, ADL situation and their satisfaction with family life and partnership relation. Factor analyses demonstrated different patterns between the R group and the patient group. No correlation was found between satisfaction with life as a whole and lung function parameters whereas three of the CRQ dimensions; emotional function, fatigue and mastery correlateded significicantly with satisfaction with life as a whole and several of the domains. No relationships was found between satisfaction with life as a whole or any of the domains and the CRQ dimension dyspnea. CONCLUSIONS: The low levels of satisfaction with life as a whole, in addition to low levels of satisfaction with several domains, indicate that the majority of the patients had not managed to cope successfully with the consequences of their impairment. More attention must, thus, be given to these patients in the rehabilitation work.

    Abstract [en]

    BACKGROUND: General practitioners (GPs) have expressed difficulties in issuing sickness certificates and problems may arise if this work is not performed in an adequate manner. There is scant knowledge about how collaboration with other professions could be organized to enhance this work.

    OBJECTIVE: Evaluate the feasibility of occupational therapists (OTs) performing supplementary assessments for persons on sick leave.

    METHODS: Four healthcare centres (HCs) tested a working approach intervention where sick-listed patients were offered a complementary occupational therapy assessment. The OT assessments were intended to provide useful information for GP issued extended sickness certificates. Data on sick leave, sickness certificates and patient questionnaires were collected at different HCs. Interviews were conducted with GPs and OTs and the Consolidated Framework for Implementation Research was used to analyse the intervention’s implementation.

    RESULTS: No major differences in the sickness certificate quality was found. Available data on sick leave increased for all HCs during the project. Not all GPs used the OT assessments, which indicates that the implementation of the intervention was insufficient.

    CONCLUSION: Testing a new working approach in primary healthcare requires an implementation strategy. To improve sickness certification quality, this work needs to be prioritized as an important healthcare task.

    The concept of work ability is not clearly defined although it has a central place in vocational rehabilitation. Several health professions are involved in assessing work ability, physicians and occupational therapists are two of these. OBJECTIVE: The purpose of this study was to explore occupational therapist and physician views about work ability and experiences in assessing work ability. PARTICIPANTS: Fourteen physicians and 23 occupational therapists participated in seven focus group discussions that were audio taped and transcribed verbatim. METHODS: Qualitative content analysis was used. Each author performed an individual preliminary analysis. These analyses were later discussed and refined in the research team and a workshop. The final categorization resulted in one theme, four categories and 13 sub-categories. RESULTS: The overall theme expressed work ability as an obscure, complex and unique concept. The four categories illustrate the affecting factors and confirm the complex structure of work ability: the person, the context of life, the work, and the society. Physicians expressed greater difficulty in assessing work ability than occupational therapists did, because they have fewer instruments to access this concept. CONCLUSIONS: Assessment of work ability requires team cooperation with several different professionals. Cooperation could increase accuracy in issuing sickness certification and strengthens the ability of identifying individual requirements for rehabilitation.

    Documentation of posture measurement cost is rare and cost models that do exist are generally naive. This paper provides a comprehensive cost model for ergonomic research, documents the monetary costs of three exposure assessment methods (inclinometry, video observation, and self-report), and discusses cost components. Trunk and shoulder posture were assessed for 27 aircraft baggage handlers for 3 full shifts each using three methods typical to ergonomics: self-report via questionnaire, observation via video film, and full-shift inclinometer registration. The model accounted for costs related to meetings to plan the study, administration, recruitment, equipment, training of data collectors, travel, and onsite data collection. Findings show that inclinometer was the most expensive method, followed by observation and then self report; the majority of costs (90%) were borne by researchers. Study design parameters such as sample size, measurement scheduling and spacing, concurrent measurements, location and travel, and equipment acquisition were shown to have wide-ranging impacts on costs. This study provided empirical cost data for use in cost models that can facilitate decision making and planning of future studies, and can be used to investigate cost efficiency in future studies.

    BACKGROUND: The number of older workers will expand during the next decades. Older workers have more long-term health problems and related limitations.

    OBJECTIVE: This study examined supportive actions provided in occupational healthcare services to older workers after vocational rehabilitation. An additional purpose was to explore occupational healthcare professionals' views on how to realize and improve adequate support activities.

    METHODS: Qualitative and quantitative methods were used, including a postal questionnaire and focus group discussions. Sixty-seven occupational healthcare service units participated in the postal questionnaire. Eight occupational healthcare professionals participated in two focus group discussions. The qualitative data was analyzed using qualitative content analysis.

    RESULTS: The qualitative analysis resulted in one theme (Act with respect), and four categories (Need for cooperation, Collaborative resources of involved stakeholders, Individual needs for support, and Gender as homogenous and separate groups). Quantitative results revealed that the workers' initiative strongly influenced the support carried out. Recommendations from the rehabilitation clinic were almost always considered when deciding on supportive actions. Focus group discussions brought up gender differences especially highlighted in the category Gender as homogenous and separate groups.

    CONCLUSIONS: Appropriate support of older workers requires cooperation between involved stakeholders, including occupational healthcare services. Provided support should be based on individual needs, but a mutual practice of determining needed support is requested.
