    Muskuloskeletala besvär i överkroppen och belastningsexponering hos svetsare och förskolepersonal: - En jämförelse2015Independent thesis Advanced level (degree of Master (One Year)), 10 credits / 15 HE creditsStudent thesis
    Abstract [sv]

    Introduktion Arbetsrelaterade muskuloskeletala besvär (WMSD) är vanligt förekommande inom manuellt arbete, varav besvär i nacke och överkropp är vanligt. Tunga statiska och tunga dynamiska exponeringar för nack- och skuldermuskulatur har visat samband med besvär i nacke/axlar. Att arbeta som svetsare innebär tungt statiskt arbete och vid arbete i förskola utsätts personalen för ett tungt varierande arbete.

    Syftet är att jämföra svetsare och förskolepersonal med avseende på muskuloskeletala besvär och fysisk belastningsexponering i överkroppen.

    Metod Med MEBA undersöktes prevalensen av muskuloskeletala besvär i överkroppen och med QEC undersöktes belastningsexponeringen. Totalt deltog 25 personer, 15 svetsare (Md 44 år) och 10 personer (Md 50 år) som arbetade inom förskola.

    Resultat Inga signifikanta skillnader fanns mellan grupperna med avseende på besvärsförekomst eller belastningsexponering. Hög prevalens av nackbesvär rapporterades inom båda yrkesgrupperna. Svetsarna hade också stor andel besvär i axlar, armbågar/underarmar som i handleder/händer, medan förskolepersonalen hade stor andel besvär i axlar och handleder/händer. Belastningsexponeringen vid svetsning bedömdes som mycket hög för nacken och mellanhög för resten av överkroppen. För arbete på förskola bedömdes exponeringen som mellan för alla delar av överkroppen.  

    Konklusion Det var en hög besvärsförekomst inom båda yrken, nackbesvär stod ut som det mest förekommande besväret.

    Abstract [en]

    Background: We aimed to generate a bioengineered multi-lamellar human corneal stroma tissue in vitro by differentiating periodontal ligament stem cells (PDLSCs) towards keratocytes on an aligned silk membrane.

    Methods: Human PDLSCs were isolated and identified. The neuropeptide substance P (SP) was added in keratocyte differentiation medium (KDM) to evaluate its effect on keratocyte differentiation of PDLSCs. PDLSCs were then seeded on patterned silk membrane and cultured with KDM and SP. Cell alignment was evaluated and the expression of extracellular matrix (ECM) components of corneal stroma was detected. Finally, multi-lamellar tissue was constructed in vitro by PDLSCs seeded on patterned silk membranes, which were stacked orthogonally and stimulated by KDM supplemented with SP for 18 days. Sections were prepared and subsequently stained with hematoxylin and eosin or antibodies for immunofluorescence observation of human corneal stroma-related proteins.

    Results: SP promoted the expression of corneal stroma-related collagens (collagen types I, III, V, and VI) during the differentiation induced by KDM. Patterned silk membrane guided cell alignment of PDLSCs, and important ECM components of the corneal stroma were shown to be deposited by the cells. The constructed multi-lamellar tissue was found to support cells growing between every two layers and expressing the main type of collagens (collagen types I and V) and proteoglycans (lumican and keratocan) of normal human corneal stroma.

    Conclusions: Multi-lamellar human corneal stroma-like tissue can be constructed successfully in vitro by PDLSCs seeded on orthogonally aligned, multi-layered silk membranes with SP supplementation, which shows potential for future corneal tissue engineering.

    Abstract [en]

    Alternative approaches to power in youth politics are needed to overcome the conceptual dichotomy between youth political action that is either linked to – or delinked from – state institutions. This paper offers an alternative drawn from a study that sought to empirically explore, and build theory upon, how teenagers construct their political action. Our qualitative study among 10 activists aged between 17 and 19 in a medium-size city in Northern Sweden found that youth constructed their political action as four different processes: moving from consciousness to action, moving from personal experience to shared goals, moving from social activities to political activities, and moving from single to multiple arenas. We integrated these processes in the concept Youth Politics as Multiple Processes. Youth efforts to bring about these processes were not always fruitful because, as their political action gained complexity, youth faced greater constraints for recognizing, addressing and challenging power from age-based exclusion, state-centered definitions of politics, and adult disinterest in youth demands. According to our findings, youth constructed political action based in an approach to power that was not state-centered. We linked our findings to youth politics research and social movement theory that similarly proposed alternative approaches to power.

    Introduktion: Kropps- och träningsideal sprids globalt med hjälp av bland annat sociala medier. Många ungdomar strävar efter en viss idealkropp, vilket i vissa fall resulterar i ätstörningar och negativa träningsbeteenden. Tidigare forskning är främst gjord på tjejer men det har visat sig att även killar drabbas. Elitsatsande ungdomar inom olika idrotter kan utgöra en riskgrupp.

     

    Syfte: Att undersöka unga elitsatsande hockeykillars syn på träning, kropp och hälsa i relation till sociala medier och rådande kropps- och träningsideal.

     

    Metod: En kvalitativ studiedesign valdes där semistrukturerade intervjuer genomfördes med nio spelare från en elitsatsande klubb i åldrarna tretton till sexton år. Intervjuerna analyserades enligt Grounded Theory.

     

    Resultat: Intervjuerna resulterade i kärnkategorin ”Sunt hälsotänk – lagom är bäst” som innefattar fyra kategorier med underkategorier. Kategorierna är Målmedvetenhet med en sund inställning, Fysisk aktivitet är bra för alla, men mängden är upp till var och en, Medvetna men uttrycker sig vara opåverkade av sociala medier och Stödjande omgivning. Slutligen skapades en modell, det vill säga en mindre teori.

     

    Konklusion: Spelarna beskrev ett sunt hälsotänk där de uttryckte att de inte påverkades av yttre faktorer. De var medvetna om att mycket på sociala medier handlar om träning och kost, men påverkades inte negativt av det och tyckte att tränings- och kostvanor inte ska gå till en överdrift. Orsaken till detta kan vara att de har ett bra stödnät och att vara en spelare i ett elitsatsande hockeylag kan ha en skyddande effekt. En ökad kunskap inom fysioterapin om ungdomars syn på träning, kropp och hälsa skulle kunna bidra till att negativa beteenden upptäcks i god tid. Mer forskning krävs som stödjer detta.

    Introduktion: Fallolyckor bland äldre innebär stora samhällskostnader och lidande för individen. Träning av balansförmågan kan minska fallrisken men tillförlitliga och lättanvända metoder för att utvärdering av sådan träning behövs. Accelerometer är ett enkelt instrument validerat för att mäta balansförmåga och sådana finns idag inbyggda i de flesta mobiltelefoner. En applikation för självtest av balans i hemmiljö skulle vara ett hjälpmedel vid fallpreventiv träning.

    Syftet med denna studie är att utvärdera applikationen “Balansera” som är under utveckling för att äldre personer ska kunna testa sin balans med hjälp av en mobiltelefon eller surfplatta.

    Metod: Under ett användartest observerades 14 deltagare när de använde applikationen. Validiteten testades genom att applikationens tidtagningsförmåga jämfördes statistiskt med manuell tidtagning och utifrån accelerometerdata.

    Resultat: Under användartesterna uppvisade de äldre deltagarna en rad svårigheter med att genomföra självtestet med hjälp av applikationen. Den statistiskt beräknade korrelationen mellan applikationens tidtagning och den manuella tidtagningen gav ett r2-värde på 0,904 samt ett p-värde på 0,000.

    Slutsats: Denna studie visar att applikationen ”Balansera” på ett tillförlitligt sätt kan mäta balansförmåga med hjälp av tidtagning. För att applikationen ska bli tillförlitlig som självtest för hemmabruk måste den vidareutvecklas så att användaren utför testet korrekt.

    Bakgrund

    Ridterapi är en hästbaserad behandlingsform som har visat sig ge många olika fysiska förbättringar för personer med cerebral pares. Ridterapi använts i vissa fall av fysioterapeuter som ett alternativ till behandling för dessa personer. Fortfarande finns få kvalitativa studier av hur ridterapi upplevs kroppsligen av ryttarna samt vilka intentioner som finns.

    Syfte

    Syftet med denna studie var att utifrån ryttares och instruktörs perspektiv belysa intentioner och upplevelser i samband med ridterapi som behandling vid cerebral pares.

    Metod

    Kvalitativ intervjustudie med fyra kropps- och upplevelseorienterade intervjuer samt en avslutande member check som utfördes med deltagarna, en ryttare och en instruktör, i nära anslutning till ridterapisessionerna. Studien genomfördes enligt Grounded theory.

    Resultat

    Resultatet presenteras i sju kategorier “Hästen och naturen som träningsredskap”, “Stärkt motivation genom omgivning och utveckling”, “Trygghet - viktig faktor inom ridterapin”, “Utmaningar på hästryggen”, “Från mental frånvaro till mental närvaro”, “Stärkande upplevelser” och “Gemensamma mål”. De sju kategorierna mynnade ut i en huvudkategori “Ridterapi som lustfylld drivkraft att möta kroppsliga och mentala utmaningar” samt en teoretisk modell.

    Konklusion

    Ridterapi påverkar kroppen likväl som det mentala. Det kan bidra till ökad närvaro samt höjd motivation till fortsatt träning. Ridterapin kan bidra till upplevelser av kompetens, samhörighet och autonomi.

    Bakgrund: Unga personer med funktionsnedsättning, rörelsehinder i synnerhet, skattar lägre hälsa än andra. Ohälsan kommer dels av funktionsnedsättningen i sig men också av påverkbara orsaker som låg grad av fysisk aktivitet och diskriminering.

    Syfte: Att utifrån ett rättvise- och genusperspektiv kartlägga hur de fysioterapeutiska interventionerna samt fysisk aktivitet fördelas för barn upp till 18 år med Cerebral Pares (CP) i de fem norra landstingen i Sverige 2013 utifrån grovmotorisk funktionsnivå och ålder. Syftet var också att undersöka samband mellan behandlingar som Botulinumtoxin (botox), spasticitetsreducerande medicinering, operationer och ortoser och mängden fysioterapi och fysisk aktivitet.

    Metod: Studien är en registerstudie utifrån ett nationellt kvalitetsregister, CPUP. Data innefattande 313 barn 0-18 år med CP från de fem nordliga länen i Sverige år 2013. Studien behandlar den första mätningen för året i de fall flera gjorts.

    Resultat:  Inom tre landsting fick pojkarna fler eller mer frekventa fysioterapeutiska interventioner. Skillnader mellan landsting sågs gällande om, hur ofta och i vilket syfte fysioterpeutiska interventioner givits (p>0,001). Fysioterapeuten (FT) medverkade oftare för barn med lägre grovmotorisk funktionsnivå och oftare för barn som inte hade fysisk fritidsaktivitet. Barn med lägre funktionsnivå deltog i lägre grad i fritidsaktivitet (p>0,001). Pojkar deltog i högre grad i skolidrott jmf med flickor (p=0,028). Det fanns samband mellan övriga behandlingar och fysioterapeutiska interventioner samt närvaro av FT.

    Slutsats: Förliggande studie tyder på skillnader i fysioterapeutiska insatser avseende på kön och landstingstillhörighet. Det fanns samband mellan grovmotorisk nivå och deltagande i fysisk fritidsaktivitet samt mellan kön deltagandet i skolidrott.

    Background: Young people with disabilities, especially physical disabilities, report worse health than others. This may be because of the disability, lower levels of physical activity, and discrimination. For children with cerebral palsy, access to physiotherapy and physical activity is a crucial prerequisite for good health and function. To date, there is limited knowledge regarding potential gender bias and inequity in habilitation services.

    Objectives: To map how physiotherapeutic interventions (PTI), physical leisure activity, and physical education are allocated for children with cerebral palsy regarding sex, age, level of gross motor function, and county council affiliation. This was done from a gender and equity perspective.

    Methods: A register study using data from the Cerebral Palsy follow-Up Program (CPUP). Data included 313 children ≤18 years with cerebral palsy from the five northern counties in Sweden during 2013. Motor impairment of the children was classified according to the expanded and revised Gross Motor Function Classification System (GMFCS).

    Results: In three county councils, boys received more physiotherapy interventions and received them more frequently than girls did. Differences between county councils were seen for frequency and reasons for physiotherapy interventions (p < 0.001). The physiotherapist was involved more often with children who had lower motor function and with children who had low physical leisure activity. Children with lower motor function level participated in physical leisure activity less often than children with less motor impairment (p < 0.001). Boys participated more frequently in physical education than did girls (p = 0.028).

    Conclusion: Gender and county council affiliation affect the distribution of physiotherapy interventions for children with cerebral palsy, and there are associations between gender and physical activity. Thus, the intervention is not always determined by the needs of the child or the degree of impairment. A gender-bias is indicated. Further studies are needed to ensure fair interventions.

    Introduktion: Stroke medför såväl fysiska som kognitiva konsekvenser för den drabbade vilket kan påverka förmågan till säker förflyttning. Personer med funktionsnedsättning kan erbjudas olika typer av hjälpmedel och rollatorn är det mest använda förflyttningshjälpmedlet. Forskare vid Umeå Universitet utvecklar en Intelligent rollator utrustad med sensorer som kan detektera hinder och automatiska bromsar som kan korrigera körriktningen.

     

    Syfte: Att undersöka hur personer som haft stroke upplever det att gå med Intelligent rollator inomhus samt ta reda på deras åsikter om rollatorns funktion och användbarhet.

     

    Metod: Fem personer (två kvinnor och tre män) som haft stroke fick testa att gå med Intelligent rollator inomhus. För att undersöka deltagarnas upplevelser och åsikter genomfördes semistrukturerade intervjuer som analyserades med hjälp av kvalitativ innehållsananlys.

     

    Resultat: Analysen av intervjuerna resulterade i de tre kategorierna ”Intelligent rollator väcker många tankar och frågor”, ”En hjälp i många situationer och för många personer” och ”Intelligent rollator ger feedback”. Efterhand växte det övergripande temat ”Intelligent rollator hejdar framfarten men styr i rätt riktning” fram, som beskriver deltagarnas positiva och negativa upplevelser av rollatorn.

     

    Slutsats: Deltagarna i den här studien var positiva till Intelligent rollator men ansåg att sensorerna var för känsliga och reagerade på för långt avstånd till de hinder som fanns.

    Arbetet har en stor betydelse gällande uppkomst av belastningsbesvär. Var femte yrkesverksam uppskattas ha av besvär till följd av arbetet. Det finns begränsad yrkesspecifik statistik och få studier på yrkena bagare/konditorer gällande den ergonomiska arbetsmiljön eller arbetsrelaterade muskuloskeletala besvär.

     

    Syftet med studien var att kartlägga förekomsten av muskuloskeletala besvär samt genomföra en belastningsergonomisk riskbedömning av vanligt förekommande arbetsmoment bland bagare och konditorer. Studiepopulationen är bagare och konditorer på mindre och medelstora företag.  Totalt svarade 69 stycken (30 män och 39 kvinnor) på enkäten Nordic Musculoskeletal Questionnaire (NMQ) och 15 stycken deltog i den belastningsergonomisk riskbedömning, Quick Exposure Checklist (QEC).

     

    Kartläggningen visar att ungefär tre fjärdedelar av deltagarna har haft smärta i någon kroppsregion de senaste tolv månaderna. Högst andel besvär rapporteras i kroppsregionerna nacke, skuldror och ryggens nedre del. Den belastningsergonomiska riskbedömningen visar att det är högst risk att utveckla besvär i rygg, hand/handled och nacke. Nacke är den kroppsregion där deltagare har bedömts som mycket hög risk. Stress är ett observandum då tolv av femton deltagare har uppgett en mellanexponering. Resultatet indikerar att det finns arbetsuppgifter som bör undersökas vidare. Ytterligare kartläggningar där besvärsförekomst tydligare relateras till arbetet rekommenderas. Även belastningsergonomiska analyser är av vikt för att vidare utreda vilka riskexponeringar som föreligger yrkena i dagsläget.

    This study investigated developmental changes and differences in upper-limb movement organization from 4 to 8 years of age in children born preterm (PT) and fullterm (FT). Kinematic recordings of precision-demanding unimanual movements and lateral assessments were carried out in 37 children (18 PT). All children, particularly children born PT, displayed considerable gain in movement kinematics. Contrary to controls, children born PT displayed persistently less-evident side preference. Gestational age (GA) contributed significantly to kinematic differences shown, with larger upper-limb deviances in the lowest GAs, in agreement with cross-sectional findings of altered hemispheric connections and delayed side-specialization among children born very PT.

    Bakgrund: Långvarig inaktivitet och brist på låggradig fysisk aktivitet är riskfaktorer för ohälsa även för personer som tränar regelbundet. Samtidigt har en stor och ökande del av befolkningen stillasittande jobb, inte minst kontorsarbetare som kan tillbringa mer än 80 % av arbetsdagen stillasittande. Syftet med denna systematiska litteraturgranskning var att utvärdera forskningsläget avseendet användandet av gåbands-skrivbord för att minska inaktivitet och öka den lågintensiva fysiska aktiviteten hos personer med administrativa arbetsuppgifter.

    Metod: PubMed samt PsycINFO genomsöktes efter relavanta artiklar och SBU:s mall för kvalitetsgranskning av observationsstudier användes för granskning.

    Resultat: Sex studier kunde hittas som uppfyllde inklusions- och exklusionskriterierna. En studie bedömdes ha låg risk för systematisk bias, fyra studier hade medelhög risk och en studie hade hög risk för systematisk bias. Endast en RCT kunde hittas. Det vetenskapliga underlaget är i dag inte tillräckligt för gradering av evidensnivå enligt GRADE. Samtliga studier uppvisade en signifikant ökning av den fysiska aktivitetsnivån och fem av sex studier visade en signifikant minskning av den inaktiva tiden.

    Slutsats: Gåbands-skrivbord verkar kunna minska inaktiviteten och öka den lågintensiva fysiska aktivitetsnivån hos personer med administrativa arbetsuppgifter, men det behövs fler studier av hög kvalitet för att klargöra detta.

    Sammanfattning:

    Introduktion: God neuromuskulär funktion och förmågan att kunna landa ett hopp med symmetri i nedre extremitet anses viktigt för att undvika allvarliga knäskador såsom exempelvis främre korsbandsskada.

    Syfte: Syftet med studien var att jämföra fotbollsspelande pojkar och flickor i åldrarna 13-16 år avseende neuromuskulär funktion samt även undersöka effekten av att träna ett neuromuskulärt träningsprogram (NMT).

    Metod: Studien inkluderade 114 försökspersoner vid baslinjemätning. Försökspersonerna filmades under utförandet av Drop vertical jump (DVJ) och Tuck jumps (TJ). I DVJ analyserades hoppen visuellt med en skala 0-2 och i TJ 0-10 utifrån en checklista (tio kategorier). Analysen genomfördes före och efter en 8-9 veckors interventionsperiod med ett NMT. 80 försökspersoner deltog vid uppföljningsmätning.

    Resultat: Vid baslinjemätning DVJ och TJ hade flickor sämre resultat än pojkar vilket indikerar en sämre neuromuskulär kontroll, skillnaden i TJ var dock inte statistiskt signifikant (p=0,02 respektive 0,078). Vid uppföljningsmätning förekom ingen könsskillnad i något av testerna (p=0,662 respektive 0,971). I DVJ var det ingen skillnad avseende utveckling baslinje-uppföljning för något av könen (p=0,414 respektive 0,509) men förändringen i TJ var statistiskt signifikant för båda könen (p=0,03 respektive 0,018). Det fanns ingen statistiskt signifikant skillnad mellan könens utveckling i något av testerna (DVJ p=0,327 respektive TJ 0,065).

    Slutsats: Resultaten indikerar att tidigare identifierade riskfaktorer för allvarlig knäskada är mer uttalade för unga flickor än pojkar. 8-9 veckors neuromuskulär träning kan möjligtvis påverka riskfaktorerna, vid uppföljning var det inte någon större skillnad mellan könen, men mer studier behövs för att utreda på vilket sätt träningen påverkar respektive kön.

    Syfte

    Syftet med studien var att undersöka hur patienter med utmattningssyndrom upplever att deras minnes- och koncentrationsförmåga påverkas under genomförandet av ett fysiskt- och kognitivt inriktat multimodalt rehabiliteringsprogram.

     

    Metod

    Fjorton patienter med utmattningssyndrom intervjuades med hjälp av en semistrukturerad intervjuguide efter att ha deltagit i ett multimodalt rehabiliteringsprogram under 24 veckor. Programmet innehöll kognitivt beteendeinriktad rehabilitering i grupp, individuellt motiverande samtal enligt fysisk aktivitet på recept (FaR©), samt arbetslivsinriktade åtgärder. Programmet kompletterades med; 1/endast multimodal rehabilitering, 2/tillägg av datorbaserad kognitiv träning, 3/tillägg av fysisk träning. Studien hade en kvalitativ ansats och analyserades med metoden Grounded theory.

     

    Resultat

    Analysen resulterade i kärnkategorin, ”Tilltro och struktur i vardagen ger lugn och förbättrar minne och koncentration”. Minnes- och koncentrationsförmågan beskrevs påverkas av en fortlöpande process av fyra faktorer under rehabiliteringen. Dessa faktorer refererar till kategorierna ”stöd”, ”kontroll”, ”acceptans” och ”ambivalens” och var alla betydelsefulla för att patienterna skulle uppleva en förbättrad och hanterlig minnes- och koncentrationsförmåga.

     

    Slutsats

    Patienterna upplevde att de multimodala rehabiliteringsinsatserna gav dem stöd till att återfå kontroll och acceptans att få vara den man är, vilket ökade patienternas tilltro och struktur i vardagen. Detta gav patienterna ett lugn, vilket upplevdes gynnsamt för både minnes- och koncentrationsförmågan. Vid utformning av ett multimodalt rehabiliteringsprogram för patienter med utmattningsssyndrom är det viktigt att insatserna tillmötesgår patienternas behov av stöd, kontroll och acceptans för att skapa tilltro och struktur i vardagen. Då har minnes- och koncentrationsförmågan bäst förutsättningar för att utvecklas i positiv riktning.

    Introduktion: Hypermobilitetssyndrom är den främst förekommande bindvävssjukdomen i samhället. Patientgruppen uppvisar ett brett spektra av symtom där inga tydliga riktlinjer för behandling finns.

    Syfte: Syftet med denna studie är att undersöka kvinnliga hypermobila smärtpatienters upplevelser av att delta i en åtta veckors intervention innehållande teori och träning fokuserad på kroppens bindväv.

    Metod: Sex kvinnliga hypermobila smärtpatienter rekryterades från en hälsocentral i Norra Sverige och deltog  i en åtta veckors lång intervention. Efter interventionen genomfördes semistrukturerade intervjuer för att undersöka informanternas upplevelser. Kvalitativ innehållsanalys användes vid analysen.

    Resultat: Vid analysen framställdes sex kategorier som sedan mynnade ut i temat ett bredare perspektiv och ökad förståelse leder till acceptans. Kategorierna som skapades var värdefullt med professionell personal, givande gruppgemenskap, positivt med ökad kunskap om sjukdomen, föredelar med individuellt doserad träning, vikten av struktur i vardagen och förändring av hälsa. Resultatet visade positiva upplevelser av såväl teori som träning. Flera informanter upplevde förbättringar i sina besvär av träningen medan några inte upplevde någon positiv förändring, däremot fick alla informanter ökad förståelse kring fördelarna med fysisk aktivitet.

    Konklusion: En intervention bestående av teori och träning kan vara betydelsefull för hypermobila smärtpatienters hälsa och välmående. Interventionen gav ett bredare perspektiv och ökad förståelse om sjukdomen och om varje individs förutsättningar, vilket bidrog till ökad acceptans. För en optimal behandling krävs erfaren vårdpersonal och teambaserat arbete där fysioterapeuten har en betydande roll. Gruppträning samt individuellt doserad träning är att föredra. Det behövs mer forskning om denna form av intervention för att veta om den är applicerbar på patientgruppen i helhet.

    Background: Tendon disorders are common and lead to significant disability and pain. Our knowledge of the ‘tennis elbow’, the ‘jumpers knee’, and Achilles tendinosis has increased over the years, but changes in denervated tendons is yet to be described in detail. The aim of the present study was to investigate the morphological and biochemical changes in tendon tissue following two weeks of denervation using a unilateral sciatic nerve transection model in rat Achilles tendons.

    Methods: Tendons were compared with respect to cell number, nuclear roundness, and fiber structure. The non-denervated contralateral tendon served as a control. Also, the expression of neuromodulators such as substance P and its preferred receptor neurokinin-1 receptor, NK-1R, was evaluated using real-time qRT-PCR.

    Results: Our results showed that denervated tendons expressed morphological changes such as hypercellularity; disfigured cells; disorganization of the collagen network; increased production of type III collagen; and increased expression of NK-1R.

    Conclusion: Taken together these data provide new insights into the histopathology of denervated tendons showing that denervation causes somewhat similar changes in the Achilles tendon as does tendinosis in rats.

    Syfte: Att med Raschanalys undersöka de psykometriska egenskaperna i EQ-5D-3L och EQ-5D-5L för patienter med rygg- och nackbesvär.

    Metod: Studien genomfördes som enkätstudie och bestod av två grupper som hade fyllt i antingen EQ-5D-3L eller EQ-5D-5L, i samband med besök hos fysioterapeut. Totalt inkluderades 164 testpersoner. I båda grupperna var majoriteten kvinnor och det var vanligast med ryggbesvär. Svarsskalans psykometriska egenskaper undersöktes med hjälp av Raschanalys, mätinstrumentens endimensionalitet och reliabiliteten för både frågorna och testpersonerna analyserades också. 

    Resultat: Genomgående visade EQ-5D-5L något bättre resultat än EQ-5D-3L. Svarsskalan för EQ-5D-5L och till viss del även för EQ-5D-3L visade på goda psykometriska egenskaper. Frågorna i de båda versionerna visade acceptabel goodness-of-fit, vilket tyder på att frågorna i båda mätinstrumenten fångar konceptet hälsorelaterad livskvalitet. Alla tespersoner visade på valida svarsmönster. Reliabilitetskoefficienten (0.74 logit) för testpersonerna var acceptabel för EQ-5D-5L. Separationsindex (1.67 logits) innebar att EQ-5D-5L kunde separera mellan två nivåer av besvär medan EQ-­5D-3L endast visade en nivå.

    Slutsats: För EQ-5D är det enkelheten i kombination med informationen den genererar som är dess styrka. Trots brister i att fånga in alla nivåer av besvär fångar EQ-5D-5L ändå in fler nivåer än EQ-5D-3L. Därför bör EQ-5D-5L numera vara förstahandsvalet när hälsorelaterad livskvalitet ska utvärderas med EQ-5D.

    Ryggsmärta är ett omfattande hälsoproblem som förekommer i hela världen, inte minst inom idrottsvärlden. Ryggsmärta är vanligare bland elitidrottare jämfört med icke-idrottare, framförallt inom idrotter som utsätter ryggen för hög belastning.

    Syfte: Syftet med vår studie var att belysa upplevelser av ländryggsmärta och dess rehabilitering bland unga kvinnliga elitidrottare.

    Metod: I denna studie användes en kvalitativ design med semistrukturerade intervjuer. Vid analys av intervjuerna utgick vi ifrån kvalitativ innehållsanalys, enligt Granenheim och Lundman. Fyra kvinnliga elitidrottare med ryggsmärta, som gick på ett skidgymnasium i Sverige rekryterades till studien.

    Resultat: Intervjuerna utmynnade i fyra kategorier ordnade under det övergripande temat: Vägen till ett utvecklat förhållningsätt till träning. Kategorierna som växte fram var; Svårigheter  att bryta invant träningsmönster, Lärt sig lyssna på kroppens signaler, Genomtänkt träningsupplägg efter skada samt Betydelsefullt att bli sedd och bekräftad. Resultatet visade att informanterna upplevt att ryggsmärtan medfört en omfattande omställning både i träningen och i vardagen, vilket inneburit en process som varit svår att acceptera. Informanterna uttryckte en stor svårighet och en känsla av förvirring när de i samband med skadan tvingades bryta sitt tidigare invanda träningsmönster.

    Slutsats: Att drabbas av en omfattande ryggskada kan medföra både positiva och negativa konsekvenser. Efter att ha genomgått en långvarig rehabiliteringsperiod hade de unga kvinnorna utvecklat sin syn på vad som är viktigt för att uppnå en bra effekt av träningen. De hade uppmärksammat betydelsen av återhämtning och att lyssna på kroppens signaler om behov av vila. Att få gehör av personer från sin omgivning upplevdes som betydelsefullt.

    Introduction: Youth mental health is an important public health concern and is given international attention. In physiotherapy, as well as in general, there is still limited knowledge of young men’s embodied experiences of mental illness, young masculinity in a social context. 

    Aim: From a gender perspective, to explore embodied experiences of mental illness and psychosomatic problems, and the use and meaning of internet, social media and digital gaming among young men with mental health problems.

    Methods: Individual in-depth interviews were collected and analysed with grounded theory. Nine young men in the age of 17-19 who had sought help for mental health problems participated in the study.

     

    Results: The analysis of the interviews resulted in the core category wants to be seen but unnoticed and three categories with eight associated subcategories A model was created which showed the young men’s lives with mental illness, digitalised lives, and gender- and masculinity constructs – and how these psychosocial paralleled processes formed each other.

    Conclusion: This study illuminates the complex intertwining of embodiment of mental illness and psychosomatic disorders, digital life and social construction of gender/masculinity among the young men. The core category represents their longing to be ‘ordinary’ youths in an associated community, at the same time they are not seeking any extra attention – and made themselves ‘invisible’ when feeling mentally ill. The results can help physiotherapists and other healthcare professionals working with young men to enhance knowledge about young men’s experiences of mental illness and how their digitalised life and masculinity constructs form each other.

    The aims of this review were to determine the level of evidence for exercise training in the management of patients with chronic obstructive pulmonary disease (COPD) and provide evidence-based recommendations on exercise training. This review was performed in PubMed and Cochrane Library. Included studies investigated patients with COPD who had been randomised to exercise training or no training. Six systematic reviews were included. The methodological quality was scored using a grading system (GRADE). The analysis showed that aerobic and resistance training in patients in a stable state of COPD results in improved health-related quality of life and decreased dyspnoea, anxiety and depression (moderately strong scientific evidence, grade +++), and increased physical capacity and decreased dyspnoea in daily activities (limited scientific evidence, grade ++). In patients with an acute exacerbation, aerobic and resistance training, performed directly after the exacerbation, results in improved health-related quality of life (moderately strong scientific evidence, grade +++), improved exercise capacity and decreased mortality and hospitalisation (limited scientific evidence, grade ++). Thus, patients with COPD should be recommended to take part in exercise training.

    Work experiences among nurses and physicians in the beginning of their professional careers analyses using the effortreward imbalance model The aim of the study was to scrutinise how nurses and physicians, employed by the county councils in Sweden, assess their work environment in terms of effort and reward at the start of their career. The aim was also to estimate associations between work satisfaction and the potential outcomes from the effortreward imbalance (ERI) questionnaire. The study group, 198 nurses and 242 physicians who graduated in 1999, is a subsample drawn from a national cross-sectional survey. Data were collected in the third year after graduation among the nurses and in the fourth year after graduation among registered physicians. The effortreward imbalance questionnaire, together with a question on work satisfaction, was used to evaluate psychosocial factors at work. The results reveal that nurses scored higher on effort, lower on reward and experienced higher effortreward imbalance, compared with physicians. Women scored higher on work-related overcommitment (WOC) compared with men. Among the physicians, logistic regression analysis revealed a statistically significant association between WOC and ERI, sex, effort and reward. Logistic regression analysis also revealed a statistically significant association between WOC and ERI and between WOC and effort among the nurses. Dissatisfaction with work was significantly higher among those who scored worst on all three ERI subscales (effort, reward and WOC) and also among those with the highest ERI ratios compared with the other respondents. In conclusion, to prevent future work-related health problems and work dissatisfaction among nurses and physicians in the beginning of their professional careers, signs of poor psychosocial working conditions have to been taken seriously. In future work-related stress research among healthcare personnel, gender-specific aspects of working conditions must be further highlighted to develop more gender-sensitive analyses.

    Introduktion:Det är viktigt att poliser testas kontinuerligt för att säkerställa att de är redo för arbetssituationer inom polisyrket.I Sverige genomförs tester endast vid antagning till polisutbildningen och då har polisstudenterna visat sig ha en högre fysisk förmåga jämfört med normalpopulationen.

     Syftet med denna studie var att undersöka om det sker någon förändring i fysisk förmåga och motionsvanor hos polisstudenter under studietiden. Ytterligare ett syfte var att undersöka om polisstudenter efter ett års studier har en högre fysisk förmåga än normalpopulationen för samma ålder.

     Metod:De 74 polisstudenter som genomfört tester i första terminen erbjöds att upprepa testerna i tredje terminen. Deltagarnas kondition, greppstyrka, bålstabilitet och explosivitet mättes och en enkät kring deras motionsvanor samt syn på träning i polisyrket besvarades.

     Resultat: På gruppnivå fanns en signifikant försämrad greppstyrka men i övrigt sågs inga skillnader mellan första och tredje terminens fysiska förmåga. Polisstudenterna hade en bättre fysisk förmåga jämfört med normalpopulationen. Under tredje terminen ansåg gruppen att explosivitetsstyrka blivit en viktigare fysisk egenskap medan koordination blivit oviktigare.

     Slutsats: Det är positivt att polisstudenterna bibehåller en hög fysisk förmåga längre in i utbildningen så att de är redo för en lång karriär inom polisyrket. I tredje terminen anser polisstudenterna att explosivitetsstyrka blivit en viktigare fysisk egenskap medan koordination blivit oviktigare, vilket på sikt kan leda till förändrade testresultat.

    Objective: To determine whether self-reported pain measures are associated with selection for multimodal or multidisciplinary rehabilitation (MMR) and whether this selection is influenced by sex. Design: Cross-sectional cohort study. Subjects: A total of 1,226 women and 464 men with chronic pain conditions from 2 university hospitals. Methods: Drawing from the Swedish Quality Registry for Pain Rehabilitation (SQRP), data on pain, psychological symptoms, function, health, and activity/participation were collected. Multiple logistic regression was used to investigate association of pain measures with selection for MMR (no/yes) after multidisciplinary assessment. Covariates were: age, educational level, anxiety, depression, working status, and several pain measures. Results: High pain intensity in the previous week (odds ratio (OR) 0.92; 95% confidence interval (CI) 0.86-0.99) and high pain severity (Multidimensional Pain Inventory) (OR 0.83; 95% CI 0.74-0.95) were negatively associated with selection for MMR, whereas higher number of pain quadrants was positively associated with selection for MMR. Similar results were obtained for women, but none of the measures was predictive for men. Conclusion: This practice-based study showed that higher scores on self-reported pain were not associated with selection for MMR, and in women there was a negative association for higher pain intensity and pain severity. Thus, other factors than pain determine whether patients are selected for MMR.

    Syftet med denna studie är att undersöka i vilken utsträckning barn med Juvenil Idiopatisk Artrit (JIA) deltar i skolidrotten utifrån tre frågeställningar: Finns det några skillnader mellan dem som helt eller delvis deltar i skolidrotten gällande graden av ledskador, självskattad smärta, funktionsnedsättning hälsorelaterad livskvalitet (HRQOL) samt delkomponenterna psykosocial förmåga och fysisk funktion. Finns det några könsskillnader och finns det några åldersskillnader mellan barnen som helt eller delvis deltar i skolidrotten?

     

    I studien ingick 49 barn med JIA. Graden av ledskador mättes med Juvenile Arthritis Damage Index for Articular Damage (JADI-A), smärta med Visuell Aanalog Skala (VAS), funktionsnedsättning med Childhood Health Assessement Questionnaire (CHAQ) och HRQOL med Pediatric Quality of Life Inventory 4.0 Generic Core Scales (PedsQL). Jämförelseanalys gjordes med Independent Samples T-test, primära utfallsvariabeln var deltagande i skolidrott, sekundära kön och ålder. Statistisk signifikansnivå var p<0,05

    Resultatet visade att mer än hälften, 28 stycken, delvis deltog i skolidrotten och att de barnen hade högre grad av ledskador (p=0,013), smärta (p=0,000) och funktionsnedsättning (0,000) samt lägre självskattad HRQOL (p=0,000), fysisk funktion (p=0,000) och psykosocial förmåga (p=0,000). Pojkarna som delvis deltog hade högre grad av smärta och skattade lägre HRQOL än flickorna.

    Huvudvärk påverkar världens befolkning med en förekomst varje år som motsvarar 47%. Muskuloskeletala problem i de tre övre segmenten i nacken så som rörelseinskränkning, ömhet och smärta i huvud och nacke kan bidra till Cervikogen huvudvärk. Manuella tester behöver hög reliabilitet och validitet för att kunna användas som ett verktyg i den kliniska verksamheten.  Syftet med den här studien var att försöka fastställa interbedömmarreliabilitet på tre manuella tester genom att undersöka position och rörlighet vid C2 i nacken hos personer med huvudvärk. För att undersöka användbarheten i dessa test jämfördes även resultaten mellan två erfarna fysioterapeuter och mellan en fysioterapeut och en sista års fysioterapeutstudent. 16deltagare med huvudvärk inkluderades i studien och tre deltagare utan sådana problem för blindat syfte. Överensstämmelse i procent och Cohen’s kappa (κ) beräknades för att fastställa interbedömmarreliabiliteten. Det enda testet som nådde acceptabelt κ värde (κ ≥ 0.5) var testet där patienterna rapporterade familjär smärta. Mellan experter visade κ =0.67 och mellan expert/student κ =0.82. De övriga testerna visade på lägre värden från κ =-0.27 till 0.38. Samstämmigheten i bedömningarna mellan student och expert visade inte på betydande avvikelser jämfört med samstämmigheten mellan två experter. Eftersom resultaten från de manuella testerna visade på låga interbedömmar reliabilitet värden bör sådana tester som undersöker rörlighet och positionering av C2 hos huvudvärkspatienter användas med försiktighet. Tester som rapporterar patientens upplevda smärta kan vara pålitliga.

    Introduktion: Testbatteriet The Functional Movement Screen (FMS) anses bedöma kroppens naturliga rörelsemönster. Koppsammansättningsundersökning har visat att bålens sammansättning har betydelse för topphastigheten i klassisk stil hos längdskidåkare.

     

    Syfte: Undersöka om resultaten från FMS-testet har något samband med prestation i avseende uthållighet hos elitaktiva längdskidåkare och skidorienterare. Ytterligare ett syfte är att undersöka om det finns något samband mellan FMS-testet, delövningen TSPU, bålens fettfria massa, den relativa fettfria massan i bålen och uthållighetstestet.

    Metod: Tjugo elitaktiva längdskidåkare och skidorienterare ingick i studien. Deltagarna genomförde FMS, kroppsammansättningsundersökning och uthållighetstest på rullskidor i klassisk stil.

     

    Resultat: Den totala FMS-poängen samvarierade inte med prestation i uthållighetstestet. Inga signifikanta samband hittades i prestationstestet när deltagarna grupperades efter poängen i delövningarna i FMS-testet. Delövningen TSPU samvarierade med uthållighetstestet hos samtliga deltagare (r=0.461, p=0.041), delövningen Hurdle Step samvarierade med uthållighetstestet hos kvinnor, (r=0.680, p=0.021) och delövningen Deep Squat samvarierade med uthållighetstestetet hos män (r=-0.687, p=0.041). Hos män fanns ett signifikant samband mellan relativ fettfri kroppsmassa och uthållighetstestet (r= 0.689, p=0.045). Delövningen TSPU samvarierade med totala kroppsvikten (r=0.521, p=0.018), relativ fettfri kroppsmassa (r=0.450, p=0.025), total bålmassa (r=0.478, p=0.033) och totala fettfria bålmassan (r=0.562, p=0.010).

    Slutsats: Generellt verkar FMS inte vara av betydelse för uthållighet på rullskidor i klassisk stil hos längdskidåkarna och skidorienternarna vid Umeå Unviersitet.  TSPU skulle kunna vara av betydelse både för prestation och bålens sammansättning.

    BACKGROUND: Patients with stress-related exhaustion suffer from cognitive impairments, which often remain after psychological treatment or work place interventions. It is important to find effective treatments that can address this problem. Therefore, the aim of this study was to investigate the effects on cognitive performance and psychological variables of a 12-week aerobic training program performed at a moderate-vigorous intensity for patients with exhaustion disorder who participated in a multimodal rehabilitation program.

    METHODS: In this open-label, parallel, randomized and controlled trial, 88 patients diagnosed with exhaustion disorder participated in a 24-week multimodal rehabilitation program. After 12 weeks in the program the patients were randomized to either a 12-week aerobic training intervention or to a control group with no additional training. Primary outcome measure was cognitive function, and secondary outcome measures were psychological health variables and aerobic capacity.

    RESULTS: In total, 51% patients in the aerobic training group and 78% patients in the control group completed the intervention period. The aerobic training group significantly improved in maximal oxygen uptake and episodic memory performance. No additional improvement in burnout, depression or anxiety was observed in the aerobic group compared with controls.

    CONCLUSION: Aerobic training at a moderate-vigorous intensity within a multimodal rehabilitation program for patients with exhaustion disorder facilitated episodic memory. A future challenge would be the clinical implementation of aerobic training and methods to increase feasibility in this patient group.

    Introduktion: Pensionen är en livshändelse som bidrar till en stor förändring i människors liv och som kan påverka den fysiska aktiviteten. Fysiskt aktivitet genererar både fysiska och psykiska fördelar, vilket främjar benmassa, muskulatur, balans, koordination, rörlighet och välbefinnande.

     

    Syfte:Syftet med denna litteraturöversikt var att undersöka den befintliga litteraturen angående hur pensionering påverkar förändringar i fysisk aktivitet. De undersökta studierna är samtliga gjorda i västerländska miljöer och deras fokus ligger på övergång från tiden innan pensionering till tiden efter pensionering.

     

    Metod: En sammanställning av artiklarna genomfördes på inkluderade artiklar som funnits genom systematiska litteratursökningar på databaserna Pubmed, Medline och Cinahl.

     

    Resultat: Totalt inkluderades tio stycken artiklar. Sju studiers resultat visade på att deltagande i fysisk aktivitet ökade efter pensioneringen jämfört med innan pensioneringen. Av dessa studierna visade två studier att denna ökning inte kompenserades av den förlorade aktiviteten som relateras till arbete. Två studier visade, trots resultat på ett ökat deltagande i fysisk aktivitet, samtidigt en ökning i stillasittande efter pensionering.   

     

    Slutsats: Mängden deltagande i fysisk aktivitet ökar och tillsammans med ett förhöjt stillasittande beteende kan detta innebära en totalt lägre aktivitetsnivå efter pensioneringen. Vidare forskning på hur den totala mängden fysiska aktivitet förändras vid pensionering är av stor vikt.

    Background: To evaluate intervention, implement evidence-based practice and enhance the welfare of dogs with naturally occurring osteoarthritis (OA), access to valid, reliable and clinically relevant outcome measures is crucial for researchers, veterinarians and rehabilitation practitioners. The objectives of the present study were to translate and evaluate psychometric properties, in terms of internal consistency and construct validity, of the owner-reported measure canine brief pain inventory (CBPI) in a Swedish sample of dogs with pain related to OA.

    Results: Twenty-one owners of clinically sound dogs and 58 owners of dogs with pain related to OA were included in this observational and cross-sectional study. After being translated according to the guidelines for patient-reported outcome measures, the CBPI was completed by the canine owners. Construct validity was assessed by confirmatory factor analysis, by repeating the principal component analysis and by assessing for differences between clinically sound dogs and dogs with pain related to OA. Internal consistency was estimated by Cronbach's a. Confirmatory factor analysis was not able to confirm the factor-structure models tested in our sample. Principal component analysis showed a two-component structure, pain severity and pain interference of function. Two components accounted for 76.8% of the total variance, suggesting an acceptable fit of a two-component structure. The ratings from the clinically sound dogs differed from OA dogs and showed significantly lower CBPI total sum. Cronbach's a was 0.94 for the total CBPI, 0.91 for the pain severity and 0.91 for the pain interference of function.

    Conclusions: The results indicate that the translated version of the CBPI is valid for use in the Swedish language. The findings suggest satisfying psychometric properties in terms of high internal consistencies and ability to discriminate clinically sound dogs from OA dogs. However, based on the confirmatory factor analysis, the original factor structure in the CBPI is not ideally suited to measure pain related to OA in our sample and the hypothesis of the presented two-factor structure was rejected. Further research needs to be conducted to determine whether the original psychometric results from CBPI can be replicated across different target groups and particularly with larger sample size.

    Introduction The lower back is one of the most susceptible body parts for injuries during the barbell back squat and the deadlift. Assessment and correction of lifting technique could have an important role in injury prevention and rehabilitation. Aim To investigate the accuracy of the visual assessments of physiotherapists and how many degrees of movement that is required for the movement to be associated with an increased risk of injury. Methods 25 powerlifters and Olympic weightlifters were simultaneously recorded by video and a movement analysis system while performing squats and deadlifts. The videos were visually assessed by six physiotherapists that rated specific movements as positive (there is a movement) or negative (there is not a movement) during the lifts, also if the amplitude of the movements were thought to increase the risk of injuries. The nominal results from the physiotherapists were compared to the numerical results from the movement analysis system with t-tests or Mann-Whitney U test and ROC-curve analysis. Results While visually assessing the squat, physiotherapists can, with a sensitivity of 0.70 and a specificity of 0.69, detect a posterior pelvic tilt of 34° or more. No discrimination between the positive and negative group could be made for the other movements. Conclusion The accuracy of physiotherapists´ visual assessments of lumbo-pelvic movements during the squat and deadlift seems poor. Since the positive and negative group only could be discriminated for one of the investigated movements. The results could be a consequence of the variability of lifting strategies amongst the lifters.

    Knäböj och marklyft är två frekvent använda övningar för styrketräning och rehabilitering. På träningsfaciliteter kan man se stora variationer i utförandet av övningarna. Det primära syftet med denna studie var att undersöka huruvida det finns konsensus bland instruktörer gällande vad som är bra respektive mindre bra utföranden för dessa två övningar i avseendena träningseffekt, prestation och skaderisk. Det sekundära syftet var att utvärdera användbarheten av studiens design med användandet av en web-enkät och dess utformning. Studien inkluderade 18 deltagare ur grupperna Fysioterapeuter, Personliga Tränare, Gyminstruktörer, Crossfitinstruktörer, Styrkelyftare och Tyngdlyftare. I en webenkät fick deltagarna rangordna bilder av åtta olika lyfttekniker för knäböj respektive marklyft, de uppmanades också att kommentera varje utförande. Deltagarna ansåg att konventionellt styrkelyftsutförande i både knäböj och marklyft representerade de bästa utförandena, med avseende för maximal kraftutveckling i kombination med låg skaderisk. Djup knäböj föredrogs framför grund knäböj. Om ryggen frångår sin neutrala kurvatur ansågs detta negativt och potentiellt skadlig vid både knäböj och marklyft. Om knäna går inåt i valgusvackling ansågs det skadligt. Mer forskning gällande bedömning av rörelsemönster i knäböj och marklyft samt framtagande av konkreta riktlinjer avseende instruktioner för lyftteknik i knäböj och marklyft är önskvärt.

    OBJECTIVE: The purpose of this study was to adapt and evaluate the psychometric properties of the ProFitMap-neck to Brazilian Portuguese.

    METHODS: The cross-cultural adaptation consisted of 5 stages, and 180 female patients with chronic neck pain participated in the study. A subsample (n = 30) answered the pretest, and another subsample (n = 100) answered the questionnaire a second time. Internal consistency, test-retest reliability, and construct validity (hypothesis testing and structural validity) were estimated. For construct validity, the scores of the questionnaire were correlated with the Neck Disability Index (NDI), and the Hospital Anxiety and Depression Scale (HADS), the Tampa Scale of Kinesiophobia (TSK), and the 36-item Short-Form Health Survey (SF-36).

    RESULTS: Internal consistency was determined by adequate Cronbach's α values (α > 0.70). Strong reliability was identified by high intraclass correlation coefficients (ICC > 0.75). Construct validity was identified by moderate and strong correlations of the Br-ProFitMap-neck with total NDI score (-0.56 <R < -0.71) and with the SF-36, HADS-anxiety, HADS-depression, and TSK (-0.32 <R < -0.82). Exploratory factor analysis revealed 2 factors for the Symptom scale: intensity index and the Function scale. Symptom scale-frequency index identified 1 factor. Structural validity was determined by percentage of cumulative variance >50%, Kaiser-Meyer-Olkin index > 0.50, eigenvalue > 1, and factor loadings > 0.2.

    CONCLUSION: Br-ProFitMap-neck had adequate psychometric properties and can be used in clinical settings, as well as research, in patients with chronic neck pain.

    Musicianship and teaching: aspects of musculoskeletal disorders, physical and psychosocial work factors in musicians with focus on music teachers2003Doctoral thesis, comprehensive summary (Other academic)

    Musculoskeletal disorders are common among musicians at all levels of performance. Since music teachers train our future musicians it is important to understand their work environment. By creating good examples of a healthy work environment, they can teach their students how to stay healthy and to prevent pain. The aim of this thesis was to study the work environment of music teachers at municipal music schools, with regard to physical and psychosocial factors and musculoskeletal disorders with the focus on neck and shoulder disorders. An additional aim was to investigate the variability of the playing technique in string players and to investigate if they could play with greater variation in the trapezius muscle activity pattern after a training intervention program.

    In a cross-sectional study at 23 municipal music schools, 171 out of the 208 (82%) music teachers reported that they had experienced work related musculoskeletal disorders (WMSDs) during the previous year. Women reported significantly more symptoms in the neck, the shoulders and the upper back compared to men. Both physical and psychosocial work factors were associated with neck and shoulder disorders. For women “high mental work demands” and “teaching at many schools” could be seen as risk factors and for men “lifting”, “playing the guitar” and “low social support at work” were risk factors.

    The occurrence of WMSDs was also investigated, over an eight-year period, in music teachers at one music school. The result showed that neck, shoulder and lower back disorders were common and tended to be of long duration and to increase over the years.

    In an interview study, nine music teachers focused on what they perceived to be important for their health and well-being. Replenishing and using up energy was found to be the core category. Creativity in the music and working with other musicians were perceived as sources of energy, while the goals of the organisation were experienced as stressful and used up energy. Whether the work was regarded as pedagogical or musical could affect the perception of health and the strategies for dealing with the strains of work.

    In two studies using electromyography, the variation in the trapezius muscle activity pattern in string musicians was investigated. The results suggested that each musician could repeat their muscular activity pattern in a similar way between two playing sessions. No difference was found in the trapezius muscle activity between five violinists who trained basic Body Awareness Therapy (BAT), a technique having its roots in Tai Chi Chuan tradition, compared to a reference group of nine violinists who did not take part in any training. However, the training group perceived positive changes in breathing, muscular tension, postural control and concentration during practice sessions.

    Neck and shoulder disorders were associated with physical and psychosocial factors at work. A process of replenishing and using up energy was important for music teachers’ health. The playing technique in string musicians seemed to be repeatable but difficult to affect over a short-term period. For future musicians it is crucial to learn good working technique at an early age. In the learning process the music teacher is a vital role model.

    Dementia is a disease characterized by cognitive impairment and physical decline that worsens over time. Exercise is one lifestyle factor that has been identified as a potential means of reducing or delaying progression of the symptoms of dementia, maximizing function and independence. The purpose of this study was to explore physical therapists' (PTs) experiences and reflections on facilitating high-intensity functional exercise with older people living with dementia, in residential care home settings. The study used a qualitative design based on interviews, individually or in small groups, with seven PTs engaged as leaders in the training of older people with dementia. The interviews were analyzed with a modified Grounded Theory method with focus on constant comparisons. To increase trustworthiness the study used triangulation within investigators and member checking. The core category "Discover and act in the moment-learn over time" reflects how the PTs continuously developed their own learning in an iterative process. They built on previous knowledge to communicate with residents and staff and to tailor the high intensity training in relation to each individual at that time point. The category "Be on your toes" highlights how the PTs searched for sufficient information about each individual, before and during training, by eliciting the person's current status from staff and by interpreting the person's body language. The category "Build a bond with a palette of strategies" describes the importance of confirmation to build up trust and the use of group members and the room to create an interplay between exercise and social interaction. These findings highlight the continuous iterative process of building on existing knowledge, sharing and reflecting, being alert to any alterations needed for individuals that day, communication skills (both with residents and staff) and building a relationship and trust with residents in the effective delivery of high intensity functional exercise to older people living with dementia in care settings.

    Abstract [en]

    Musicians at all levels of performance, especially string players, are known to have a high prevalence of work-related musculoskeletal disorders. The disorders seem to be most common in the neck, shoulders and low back. In 1988, a survey of the work-related musculoskeletal disorders of 36 music teachers was carried out at a music school in northern Sweden. In 1996, the teachers were reinvestigated. The study also included an investigation of the psychosocial work environment according to the Karasek demand-control theory, as well as measurements of upper-arm elevation during a working day in five violin teachers. The results showed that music teachers, like other professional musicians, often experience discomfort in the neck, shoulders, and low back. The discomfort tended to be of long duration, increasing over the years. The psychosocial work environment was characterized by high psychological demands and low authority over decisions. This was compensated for through good social support. The work required skill and creativity but was monotonous. The measurements of upper-arm elevation indicated considerable variations in shoulder positions between teachers. There were also differences in the work done with the right and left arms, with repetitive motions more commonly involving the right arm. Approximately a fourth of the working day was spent with the arm elevated 30-90 degrees. The relationships between upper-arm movements and ratings of discomfort were moderate.

    Introduktion:Motivation är en viktig faktor vid träningssammanhang dels under själva genomförandet men även för att upprätthålla fysisk aktivitet på lång sikt. Elitidrottare och motionärer är två typer av idrottare med olika typer av träningsmängd och syn på träning. Det är värdefullt att som fysioterapeut att ha kunskap om detta för att kunna ge en bra behandling. Därför är det intressant att fördjupa sig i detta ämne och undersöka vad som motiverar elitidrottare och motionärer till att träna genom denna studie.

    Syfte:Syftet med vår studie var att genom en kvalitativ metod undersöka hur elitidrottare respektive motionärer motiverades till träning samt under träning.

    Metod:Studien genomfördes via sex semistrukturerade intervjuer, tre motionärer samt tre elitidrottare. Informanterna fick svara på frågor om känslor, tankar och strategier före, under och efter träning. Materialet sammanställdes enligt innehållsanalys.

    Resultat: Utifrån innehållsanalysen framkom sex kategorier; Träningen ger fysiska och mentala vinster, Formar en positiv identitet, Utvecklar strategier, Sätter mål, Social gemenskap, Hantera barriärer.

    Konklusion: Något olika faktorer var betydelsefulla vad gällde motivation för motionärer respektive elitidrottare. Elitidrottarna i studien motiverades främst av individuella mål och att uppnå resultat. Vad gällde motionärer var social gemenskap och att må bra viktiga faktorer för att uppnå motivation.

    Introduktion: Casemetodik (CM) och Problembaserat lärande (PBL) är två metoder som skiljer sig från de traditionella inlärningsmetoderna. Syftet med CM och PBL är att stimulera studenternas förmåga att analysera, diskutera och bedöma för att hitta kopplingen mellan kunskap, fakta och problem. Det är viktigt att tillräckligt med information finns i casen. Syfte: Syftet med denna studie var att undersöka hur patienter beskrivs i examinations- och undervisningscase utifrån releventa faktorer vid rehabilitering. Metod: Kvantitativ innehållsanalys användes. Alla grundbildningar inom fysioterapi och arbetsterapi i Sverige ombads vara med i studien. Slutmaterialet bestod av 91 case från fem utbildningar som kodades och analyserades. Resultat: Resultatet visade att det ofta saknades viktig information om patienterna i casen. Det fanns inte heller några samband mellan kön och de faktorer som beskrevs i casen. Slutsats: Studien visar att många av de faktorer som ansetts som relevanta vid rehabilitering sällan beskrevs i casen. Studien har inte visat på några signifikanta samband mellan de undersökta faktorerna och kön i casen. Trots detta har det varit möjligt att urskilja en viss likhet mellan hur kvinnor och män framställs i casen gällande yrken inom Administration och kundtjänst/service-, omsorg- och försäljningsarbete, farmakologisk behandling och vårat samhälle. Där kvinnor i större utsträckning jobbar inom denna yrkeskategori samt tillhandahålls mer farmakologisk behandling än män. I framtiden behövs fler studier som undersöker hur patienter beskrivs i examinations- och undervisningscase vid arbetsterpeut- och fysioterapeututbildningar i Sverige.

    Aim: to investigate the association between changes in neck-shoulder muscle strength and changes in self-rated pain and disability, for women with nonspecific neck pain after a neck-shoulder strength training intervention. 

     

    Methods: Observational longitudinal correlation study based on an experimental study. Data was retrieved from an RCT with neck-shoulder exercise intervention. 45 women, age 20-64, with nonspecific neck-shoulder pain were divided into three groups based on type of exercise they received. Craniocervical flexion training (CCF)-group n=15; cervicothoracic training (CTH)-group n=18; craniocervical flexion training and cervicothoracic training (CCF+CTH)-group n=12. Spearman’s correlation coefficient (rs) was used for statistical analyses of correlations between changes in neck-shoulder strength, self-rated pain and disability. Change values from baseline to follow up one week after intervention period for CCF-strength, CTH-strength, Numeric Rating Scale (NRS) and Neck Disability Index (NDI) were used for the correlation analyses.

     

    Results: Correlations between neck strength changes and changes in pain and disability were overall low or very low. An improvement in CTH-extension showed a moderate correlation with an increase in disability in the CCF+CTH-group. For all other correlation analyses, CCF-strength and CTH-strength showed low (rs 0,3-0,5) or very low (rs 0,0-0,3) correlations with both NRS and NDI.

     

    Conclusion:  The results from this study give no support for neck-shoulder muscle strength gain being the explaining mechanism behind reduced neck pain and disability in women with non-specific neck pain after neck-shoulder strength training intervention. The results give indications of the complexity around neck pain and exercise. Future research should further investigate mechanisms related to neck-shoulder exercise, pain and disability.

    Abstract:

    Aims: The aim of this study was to explore the perceptions of elderly women with osteoporosis and back pain by using a spinal orthosis.

    Methods:  A qualitative study with five focus group interviews using a semi-structured interview guide, with a total of 18 women (mean age 78 years, min 65-max 90 years) who had all used the Spinomed spinal orthosis during six months in a RCT. The interviews were analyzed using qualitative content analysis with an inductive approach.

    Results: The spinal orthosis showed perceived positive effects on the participants’everyday life by supporting the woman’s autonomy. It was perceived to facilitate performing daily activities and to relieve back pain, to increase mobility and positively influence the woman’s posture. Because the spinal orthosis was perceived as an integrated part of the woman, it was of utmost importance for the spinal orthosis to fit well.

    One overall theme was found: The spinal orthosis could be perceived as an integrated part of everyday life, if it met the needs of the individual woman.

    The four categories found under this theme was: The meaning of the spinal orthosis, The perceived physical changes, Possibilities in everyday life and The importance of the individual shaping of the spinal orthosis.

    Conclusion: The results based on interviews show that the spinal orthosis can have a positive impact on the daily life and facilitate the audience who have osteoporosis and back pain . The spinal orthosis has been shown to be an integral part of everyday life and it is therefore important that it can be individually adapted to receive such a positive impact.

    In 3D gait analysis, quantification of leg movements is well established, whereas ameasure of armswing has been lacking. Recently, the Arm Posture Score (APS) was introduced to characterize arm movements in children with cerebral palsy, including information from four variables (APS(4)) in the sagittal and frontal planes. A potential limitation of the APS is that it does not include rotational movements and has not yet been evaluated with regard to gait speed. The aims of this study were (i) to investigate the effect on APS of adding two components of arm rotation (APS(6)) and (ii) to determine the influence of gait speed on the APS measures, when applied to non-disabled adults. Forty-two subjects walked 10 m at a selfselected speed (1.34 m/s), and in addition a subgroup of 28 subjects walked at a slowspeed (0.66 m/s) set by a metronome. Data were collected from markers in a whole-body set up and by eight optoelectronic cameras. The results demonstrated significantly higher APS(6) than APS(4) values for both arms, irrespective of gait speed. Speed condition, whether self-selected or slow, had a significant effect on both APS measures. The two additional arm components are suggested to provide relevant information about arm swing during walking. However, APS(6) needs to be implemented in gait analysis of individuals with gait arm pathologies in order to further examine its utility.     Introduction: Although chronic obstructive pulmonary disease (COPD) is primarily a disease of the respiratory system, a common secondary consequence is reduced quadriceps endurance. This have been associated with important clinical outcomes. However, reliability and feasibility properties of different testing protocols have not been thoroughly investigated within the same setting. Aims: The aim for this study was to determine intra-rater test-retest reliability, minimal detectable change (MDC) as well as the feasibility of the three measurement protocols. Methods: Nineteen participants with COPD were included and tested on consecutive weeks in a test-retest design. The specific endurance measurements were conducted in a randomised order at visit two and three with protocols that were isokinetic, isometric and isotonic. All tests were performed in a computerised dynamometer. Results: Quadriceps muscle endurance measurements were reliable in this group irrespective of measurement technique used. In specific, isokinetic measurements demonstrated very high reliability with a intra class correlation (ICC) of 0.97. Isometric and isotonic measurements demonstrated a high reliability with a ICC of 0.89 and 0.72, respectively. The isokinetic protocol had the smallest relative MDC of 14% whereas the isometric and the isotonic MDCs were 43% and 40%. All three test were also feasable with an average time consumption of 7 minutes and 12 seconds, limited exertional dyspnoea and no major adverse advents. Conclusion: For measurements of quadriceps muscle endurance our result indicates that an isokinetic protocol of 30 repetitions with a pace of 90°/second is the most reliable assessment strategy.

    The aims were to determine reliability and feasibility of measurements to assess quadriceps endurance in people with chronic obstructive pulmonary disease. Sixty participants (forced expiratory volume in one second (mean ± standard deviation) 55 ± 18% of predicted, age 67 ± 8 years) were tested in an inter-day, test–retest design. Isokinetic, isometric, and isotonic protocols were performed using a computerized dynamometer. Test–retest relative and absolute reliability was determined via intraclass correlation coefficient (ICC), coefficient of variation (CV%), and limits of agreement (LoA%). Isokinetic total work demonstrated very high relative reliability (ICC: [95% confidence interval] = 0.98 [0.94–0.99]) and the best absolute reliability (CV% (LoA%) = 6.5% (18.0%)). Isokinetic fatigue index, isometric, and isotonic measures demonstrated low-to-high relative reliability (ICC = 0.64 [0.46–0.77], 0.88 [0.76–0.94], 0.91 [0.85–0.94]), and measures of absolute reliability (CV% (LoA%)) were 20.3% (56.4%), 14.9% (40.8%), and 15.8% (43.1%). For isokinetic total work and isometric measurements, participants performed better on retest (4.8% and 10.0%, respectively). The feasibility was similar across protocols with an average time consumption of less than 7.5 minutes. In conclusion, isokinetic, isometric, and isotonic measurements of quadriceps endurance were feasible to a similar extent and presented low-to-very high relative reliability. Absolute reliability seems to favor isokinetic total work measurements.

    Compared with healthy older adults, people with chronic obstructive pulmonary disease (COPD) have reduced capacity and increased symptoms during leg and arm activities. While the mechanisms underlying limitations and symptoms during leg activities have been investigated in detail, limitations and symptoms during arm activities are not well understood, and the potential differences between physiological responses of leg and arm activities have not been systematically synthesised. Determining physiological responses and symptoms of arm activities compared with physiological responses and symptoms of leg activities will help us understand the mechanisms behind the difficulties that people with COPD experience when performing physical activities, and determine how exercise training should be prescribed. Thus, the aim of this systematic review is to compare the physiological responses and symptoms during activities involving the arms relative to activities involving the legs in people diagnosed with COPD.

    Objective: The aim of this study was to investigate whether medical yoga, “MediYoga”, was an effective method of treatment as a complement to patient education, IBS-school, in the treatment of patients with Irritable bowel syndrom, (IBS) for symtoms and quality of life. MediYoga as a treatment for patients with IBS had not been investigated previously.Methods: This pilot design included a consecutive sample of participants that took place at a clinic in Stockholm among patients enrolled in patient education "IBS school”. The included participants in this study had a diagnosed IBS and understood Swedish in speech and writing. Those diagnosed for other causes of abdominal pain were excluded. Two diagnosis-specific questionnaires were used, the IBS-SSS, measure for symptoms change and IBS-QOL for perceived quality of life. The measurements where assessed when IBS school started and after completed the school. Thereafter the intervention by Medi Yoga initiated by those who voluntarily chose to participate. MediYoga consisted of five occasions, once a week. Those who only participated in the IBS school constituted a control group. When MediYoga sessions where completed summoned all participants to a follow-up occasion, where measurements were assessed a third and final time.Results: There was no significant difference between groups at start, or end of IBS-school. However, when comparing the groups at follow-up time, there was a significant improvement for the intervention group regarding quality of life (p = 0.036), and on the verge of significant decreased IBS symptoms (p = 0.051).Conclusion: The intervention with the addition of MediYoga following IBS school provides improvements in terms of reducing IBS-symtoms and improved quality of life. Few participants mean that the results should be interpreted with caution, consequently it is of interest to a larger study on MediYoga for patients with IBS.

    The aim of this systematic review was to synthesize the evidence for changes for proprioceptive variables consisting of movement and position sense in participants with glenohumeral musculoskeletal disorders. Five databases were searched until 13th August 2013. Methodological quality was assessed and meta-analyses were performed for active and passive joint reposition sense (AJPS and PJPS) and movement sense, determined with threshold to detection of passive motion (TTDPM). The search yielded 17 studies, four of which were classified as having high methodological quality, seven as moderate and six as low quality. For participants with post-traumatic glenohumeral instability, pooled findings indicate moderate evidence for higher TTDPM for involved shoulders compared to control groups and the contralateral uninvolved side, indicating decreased movement sense. For AJPS and PJPS there was moderate to limited evidence for significant increased errors for involved compared to uninvovled shoulders, but not when compared to the control groups. Limited evidence was found for decreased AJPS acuity for patients with chronic rotator cuff pain and for patients with unspecified shoulder pain compared to healthy controls. Movement sense is most likely to be impaired after shoulder injury involving post-traumatic instability when compared to the contralateral shoulder and to controls, while deficits for AJPS and PJPS are more likely to be evident compared to the contralateral shoulder in participants with glenohumeral musculoskeletal disorders. Inkluderingskriterierna för att vara med i studien var att man skulle gå termin två på fysioterapeutprogrammet på Umeå universitet, vara kvinna mellan 20-25 år och träna regelbundet, minst två gånger/vecka.

    Resultat:

    Analysen av innehållet resulterade i kärnkategorin “Träning – nödvändigt måste för att leva som man lär” samt fem kategorier med tillhörande underkategorier. Det framkom att fysioterapeutstudenter tränade mycket och att det var en del av deras vardag. En fysioterapeut beskrevs som en vältränad person, något som ansågs vara viktigt då fysioterapeuter ”bör leva som de lär” och vara ”trovärdiga” för patienterna. Fysioterapeutstudenterna levde efter normen att man ska träna som fysioterapeut och fysioterapeutstudent för att inte hamna utanför normen, studenterna ansåg att det skulle bli motsägelsefullt om man inte tränade och var överviktig som fysioterapeut.

    Konklusion:

    I resultatet framkom att normen för en fysioterapeut var att man skulle vara fysiskt aktiv för att vara lämpad som fysioterapeut. Det är viktigt att fysioterapeuter är medvetna om sin egen träning och hur den kan upplevas av andra.
