    Hälso- och sjukvården anses vara en viktig del av svensk välfärd som alltid ska värna sin humana karaktär. Förändringarna som har skett inom svensk hälso- och sjukvård under de senaste decennierna, har lett till en mer byråkratiserad sjukvård där läkare och sjuksköterskor ofrivilligt har blivit byråkrater. Syftet med denna studie är dels att undersöka hur läkare och sjuksköterskor vid en privat vårdcentral i Västra Götalandsregionen gör prioriteringar av patientbesök, dels att jämföra hur dessa två professionerna gör sina prioriteringar. Den här studien är en fallstudie där tre läkare och tre sjuksköterskor har intervjuats. Det empiriska materialet har senare analyserats med hjälp av kvalitativ innehållsanalys. Studien utgår och baseras på Lipskys teori om gräsrotsbyråkrater. Resultaten som har fåtts visar att läkare och sjuksköterskor upplever att de har ett stort handlingsutrymme när de gör medicinska bedömningar under prioriteringsprocessen, samt att deras handlingsutrymme inte begränsas av statligt regelverk och organisationens mål i någon större utsträckning. En tydlig svårighet som samtliga läkare och sjuksköterskor lyfter är tidsbrist som de försöker hantera genom användning av olika rutiner och hanteringssystem.

    Den nationella vårdgarantin syftar till att ge vård till alla medborgare inom rimliga tidsgränser men utvärderingar visar att detta inte realiseras fullt ut. Den medicinska professionens handlingsutrymme att agera kan vara en förklaring till detta. Huruvida läkare som implementerande gräsrotsbyråkrater förstår, vill och kan implementera en reform påverkar således hur väl den implementeras. Denna studie syftar till att fördjupa förståelsen för läkarens roll i implementeringen av vårdgarantin genom att undersöka hur ett antal läkare i Region Västerbotten upplever sina kunskaper om vårdgarantin, deras roll i implementeringen, deras inställning till reformen och hur de upplever de organisatoriska förutsättningarna för att implementera den. För att fånga läkarnas uppfattningar användes samtalsintervjuer. Respondenterna utgjordes av tre läkare med olika kompetenser och ur olika kontexter då maximal variation och teoretisk mättnad eftersträvades. Undersökningens resultat och slutsatser visar att läkarna upplever att de har bristande kunskaper om vårdgarantin och bedöms inte fullt ut förstå innebörden av vårdgarantin. Detta innefattar även förståelsen för den informationsroll som läkaren bör ha i vårdgarantiarbetet. Denna roll har varken förtydligats för läkarna eller anammats fullt ut av dem. Läkarnas inställning till reformen är inte heller renodlat positiv och den negativa påverkan som två av läkarna anser att vårdgarantin kan få på prioriteringen av patienter och deras eget handlingsutrymmet bedöms påverka viljan att implementera vårdgarantin. Läkarna upplever även organisatoriska hinder i form av personalbrist och brist på information, vilket även bedöms försvåra möjligheterna för läkarna att kunna implementera vårdgarantin fullt ut.

    Abstract [en]

    As more and more local governments begin to understand that the great promises of e-service delivery are harder than expected to realize, efficient use of ICT-resources have become increasingly important. Since simply providing more e-services is not the solution, the need to understand what constitutes a suitable e-service has arisen. Public services reach beyond the market domain; therefore, the complexities of public value must be dealt with when services are appraised. Furthermore, due to the heterogeneous nature of local government services it is impossible to evaluate all the options in depth; thus, there is a clear need for early-stage appraisal. However, existing methods of appraisal are burdened by intricacy, and associated with high costs. In response, this paper presents a model capable of reducing this intricacy. The model was developed through a participatory design process involving members on both operational and strategic level in the municipality of Skellefteå. The model implements state of the art into the workspace context while taking measures to reduce intricacy such as: incremental filtering, moving high intricacy elements to the end of the process, and exploitation of available data. As a result the organization is enabled to capture not only the low hanging fruit, but also the long tail of services. Furthermore, the improved understanding of e-service delivery has the potential to open up opportunities for new ways of business development and private-public partnerships. Finally, whereas the model presented is highly context-dependent, the implications outlined in this paper are not limited to this narrow scope.

    Abstract [en]

    This essay investigates how employees on the Swedish international development cooperation agency (Sida) perceives that the new public management (NPM) reform, result based management (RBM), have affected the international aid. And also, how the employees think that managing based on results works together with a holistic approach to aid. The material for the study was collected through qualitative interviews with five senior employees on Sida, mostly people that today sits on leading positions in the agency. The interviews had a reflective character where the respondents looked back on the twenty-year period that the agency have managed via results. What became evident in the study was that RBM have looked very different over the years, with different effect on the international aid as a result. There have been

    waves of result based management, and an oscillation between quantitative and qualitative results. The Swedish international aid through Sida has changed over time, some changes can be attributed to RBM, while others are due to political priorities, a changing world, and trends in the international aid. RBM effects the aid, primarily as it is supposed to. For example, the best performing organisations and actors in a sector gets the aid. But it also affects the aid in a manner perceived negative by the respondents. This when the focus lies on quantitative results. The focus on results have oscillated over the years between a quantitative and qualitative focus, with consequences for which aid-projects that get support from Sida. When focus lies on quantitative results, easily quantifiable, concrete projects gains ground to the disadvantage of non-quantifiable projects, and for example big reform processes. The respondents do not think that there are any discrepancies between a holistic approach to aid and result based management, if the focus on results are balanced and leaning towards easily quantifiable results. When the focus tips over towards quantifiability, a holistic view on aid is hard to maintain.

    Abstract [en]

    Using a simple stages model of the policy process, we explore the politics of ecological restoration using an array of examples drawn across sector, different size and scale, and from different countries. A policy analysis perspective reveals how, at both the program and project levels, ecological restoration operates within a complex and dynamic interplay between technical decision making, ideologies, and interest politics. Viewed through the stages model, restoration policy involves negotiating nature across stages in the policy making process, including agenda setting, policy formulation, implementation, and evaluation. The stages model is a useful heuristic devise; however, this linear model assumes that policy makers approach the issue rationally. In practice, ecological restoration policy takes place in the context of different distributions of power between the various public and private actors involved at the different stages of restoration policy making. This allows us to reiterate the point that ecological restoration is best seen not only as a technical task but as a social and political project.

    Abstract [en]

    This article discusses how ecological restoration success can be understood and evaluated using a policy analysis lens. First, this article details a conceptual tool that helps to develop a more encompassing set of criteria to assess restoration activities that provide socioeconomic benefits. Second, by broadening the understanding of restoration success and how it can be evaluated, it allows a more critical view of evaluation itself and its uses as a policy tool. A table is presented that can help practitioners reveal preferences and clarify the aims and objectives of particular initiatives. The table also sensitizes practitioners to the complexity of the links between restoration rationales and evaluation criteria, which in turn may open up much needed discussion and dialogue between restoration participants about the underlying values an actor may wish to promote. It heightens awareness of the fact that evaluation methods need to recognize that restoration is driven by multiple rationales often in the same project, both process driven and output oriented, which in turn can change over time. Adding process and output criteria together may also raise issues of priority. Evaluation criteria thus need to be assigned in ways that reflect these multiplicities, while at the same time recognizing that some restoration values might be conflictual and that there may be winners and losers. Furthermore, judgement about "failure" of a project can change as new goals emerge in delivery and implementation. Ecological restoration evaluation should therefore be ongoing, contextual, and not a one-off event.

    Abstract [en]

    This chapter investigates the role of the state in the governance of sustainable development at the sub-national, regional and local levels in Europe, including 1) the scope of state involvement, 2) the function of the state in sustainable development policy making. The state is shown to be a key player in initiating and co-ordinating the sustainable development planning processes in the four key themes across European states: (1) multi-level governance; (2) networks and public / private partnerships; (3) participation and (4) the use of marked based instruments.

    The main function of the state in the analyzed policy areas is, on the one hand to act as coordinator between different interests, and on the other to act as stimulator for initiating new policy initiatives. The state both initiates and co-ordinates policy networks and retains a great deal of power over the nature and functioning of network forms of governance. Evidence also points out how the use of new environmental policy tools can strengthen the hand of the state by supporting hierarchical governance.     Abstract [en]

    Ecological restoration has taken on a new significance in the face of climate change and biodiversity loss. Despite its growing policy salience, however, the social and political sciences have paid limited attention to the study of ecological restoration policy and practice. By drawing upon the political science study of multilevel governance, institutions, power elations, and place-based politics, a flavour is given of what a political science engagement might contribute to the rich tapestry of analysis that has already been produced by other disciplines on ecological restoration. As the use of restoration grows, it is increasingly likely that it will give rise to social dispute and be brought into conflict with a variety of environmental, cultural, economic, and community interests. Restoration policy and projects encounter professional and institutional norms as well as place-specific interests and values. There is urgent need to investigate how and in what ways some interests become winners and others losers in these activities, and how this in turn can influence ecological restoration outcomes. A political science lens could help build new criteria for evaluating the success of ecological restoration, ones that combine both process- and product-driven considerations.

    Making democracy work is a continuous struggle (Putnam 1993). This also goes for local democracy (Wollmann 1999), including the maintenance of the autonomy of local government, which is the basis for local democracy. The autonomy of local government varies across countries but there is little understanding of why systems of local government develop in such different ways. The purpose of this article is to contribute to such an understanding. The variation in local autonomy as well as the lack of explanations for it is remarkable since local autonomy is a cherished ideal for local government in liberal-democratic societies. Indeed, local autonomy is one of the constituent features of local government, perhaps the most fundamental feature. Local government is of course established by the state and serves the state both literally and in a wider sense. Yet, without some measure of autonomy from the state local democracy is meaningless — there will be no policy space for decision-makers and no choice for citizens. Arguably, local autonomy is also a driver of efficiency in local government affairs through the adaptive opportunities to local circumstances made possible by autonomous decision-makers accountable to local citizens as tax payers and consumers.

    When processes collide: leadership, legitimacy and liberation in Palestine2019Doktoravhandling, monografi (Annet vitenskapelig)

    Palestinian national movement leadership has long been intertwined with thecontext of the national movement processes – liberation, peace and statebuilding. Over time, as these processes have not come to fruition, the numerousleadership groups have had to negotiate their relationships with these processesas both the groups and processes increasingly overlap, creating significantobservable points of tension within Palestinian politics. There are currentlymultiple levels of leadership across the national movement: two representativegoverning institutions – the Palestine Liberation Organisation and thePalestinian Authority; two dominant political movements – Fatah and Hamas;and numerous popular resistance initiatives such as the Boycott, Divestment andSanctions movement coalition that has different levels of endorsement (or lackof) by the other leaderships. This thesis seeks to map the Palestinian nationalliberation movement leadership, examining the inter-relations between themultiple leadership groups and internal (i.e. intra-Palestinian) legitimacies.Examining the internal legitimacies of the Palestinian leaderships results in anexpansion of how internal legitimacy can be conceptualised. For the historicalperiod (1958-2008) analysed, I found revolutionary, representative,oppositional, institutional, democratic and moral legitimacy types within thePalestinian case. Furthermore, these were all attributed to respective nationalmovement processes. Analysing the recent period (2016-2017) requires the useof a relational approach to further develop understandings of legitimacy. Thisapproach transforms legitimacy into a process of (de)legitimisation, whichinteracts with the national movement processes and helps us capture and analysethe complexities of the Palestinian case – that of concurrent, multiple andcontending perspectives. I found the continuation of the liberation and statebuilding processes as simultaneous bases of legitimisation to be a critical point oftension within the tandem legitimisation-delegitisimation process. Engaging arelational approach demonstrated the need for ongoing leadershipreconstruction. I conclude that, in order to negotiate the interactions andcontestations between the multiple and dynamic processes that underlielegitimacy, leaderships face an ultimatum of ‘reconstruct or delegitimise’. WherePalestinian leadership groups have stagnated and not engaged with a process ofreconstruction, we see processes of delegitimisation arising that can explain thecurrent leadership complexities within the Palestinian national movement.

    The purpose of this thesis is to see how the political elite of the Russian Federation uses the historical events of 1612 to advance their preferred national identity. By using qualitative content analysis, speeches held by the president on national unity day will be analysed and compared. The study finds that there have been changes in the themes between 2005 and 2017, and that the Russian people are expected to increasingly prioritize their motherland.

    Exploring Evaluation Policy: as a process, concept and activity2013Konferansepaper (Fagfellevurdert)

    The concept of evaluation policy usually refers to "the way to do evaluations" in line with SIDA's (Swedish International Development Cooperation Agency) strategy for conducting evaluations (SIDA 1999; 2012). This way of framing evaluation policy is in my experience, the most common way to think of the concept. A strategic and written plan for how evaluations should be conducted. That is, policy is the product of authoritative decision making and the policy work contains the implementation of this decision. However, this is not the only way to view evaluation policy, and a few years back New Directions for Evaluation devoted a special issue to highlight evaluation policy. With scholars such as Mark, Cooksy, Trochim and Chelimsky (2009) arguing for the need of a "fresh look on evaluation policy" and especially the intimate relation with the evaluation practice (Trochim 2009). Although this sounds like an attempting and promising opening for a debate, these scholars have not adequately addressed the issue of evaluation policy by focusing solely on the hierarchy of policy-making, thus ignoring the vertical dimension of evaluation policy, which is highly relevant to develop a deeper understanding of the making of evaluation policy. My paper addresses the issue of making evaluation policy and with special attention to the vertical and horizontal dimension, emanating from the understanding of policy as a contested, interactive process and activity of many hands. Specifically, in my PhD-project, I will be looking at the making of evaluation policy in the education field, a frequently evaluated domain. This paper will be devoted to the discussion of the concept of evaluation policy and the different dimensions of policy, that is the vertical and horizontal aspect (Colebatch 1998) with examples of evaluation policies from the educational field. This also involves evaluation steering which affect and constitute the conditions for how the evaluation practice can evolve (Hanberger 2013; Foss Hansen 2013). Hence, this sheds light on the constant question of power in evaluation policy. I argue that evaluation policy is an important concept that needs a different or more inclusive approach emerging from the literature on policy work. It also has to be recognized that the practice of evaluation policy is not so much about implementing an already formulated evaluation policy, but rather about creating evaluation policies which is a process of social construction, of collective sense making. In conclusion, this project, by closely examining evaluation policy, sheds new light on the processes of making evaluation policy, and how this can be researched.

    From local conditionality to complexity reductionism? Exploring national evaluation policy in educational reforms in Sweden2016Konferansepaper (Annet vitenskapelig)

    The demand for evaluation and monitoring in governance is well known. A development that has taken different forms in different countries, this paper focusing on Sweden and the educational sector. Even though evaluation is an old practice within the government of Sweden, different times have called for different kinds of evaluation (see e.g. Vedung 2010). In the late 1980s the governing of the education system was reformed to a system of management by objectives and results, hence demanding new functions for evaluation in the governing of education. This could be seen as a shift into governing by evaluation (see Foss Hansen 2013). But how this takes shape can be expected to be a constant struggle since the question of what evaluation is, and should be, in a specific governing setting is a political question in a world of symbols, ideologies and power relations (Hanberger 2012). Therefore, how evaluation is perceived, what is being evaluated and how and whom having a voice are matters concerning governing of evaluation. The argument here being that by focusing on, and scrutinizing, evaluation policy these kinds of questions can be explored, questioned and possibly criticized. With evaluation policy being defined as the formal and informal rules, routines and norms that is used to shape, conduct or change decisions and actions regarding evaluation. This means that both explicit and implicit evaluation policy can be studied.

    The aim of the paper is to explore and describe how evaluation policy is formed and steered within the educational sector in Sweden during two periods of intense educational reforms (1988-1994 and 2008-2014). Drawing on a policy design framework different design elements is identified from empirical material such as governmental and educational agencies documents, and by mapping of evaluation systems and evaluations conducted by responsible agencies. The analysis show that the amount of evaluations and evaluation systems has increased over the years. Today, the Swedish education system rely extensively on evaluations of different kinds as a way to control and enhance quality and performance in education and schooling but also to enable competition and support school choice on the Swedish education market. But while more measuring and evaluation could possibly widen the debate the preliminary results indicate that ‘complexity reductionism’ rather being the case. More performance measurements and use of indicators and to a larger extent relying on international student assessments to value the national educational system while stand-alone evaluations adhering to the complex nature of education taking the backseat. The preliminary findings thus indicate that evaluation policy could be one possible way to enhance our understanding of the relationship between evaluation and governance. Making it possible to question the role and function of evaluation in governance. And thus, to explore governance of and by evaluation.
