    Abstract [en]

    Introduction: Proprioception, including joint position sense (JPS), is important when coordinating body movements which require an efficient and intact sensorimotor system. However, there is no gold standard test method for evaluating JPS. The supine, active and non-weight-bearing test position in this study may make it possible to detect poor JPS in newly-injured persons. Aim: To evaluate the test-retest reliability of a novel knee JPS test in a supine position.. Method: 15 healthy and active students from Umeå University (Mean±SD: age 25±3.1 years, height 178±9.1 cm and weight 74±11.2 kg) participated on two test occasions with one week in between. The participants were instructed to flex their knee from an extended position untill a stop sign was displayed and then to reproduce the angle. The stop sign was activated when participants reached either 35 or 60 degrees knee flexion, taking into considiration reaction time and system delay within the program, 40 and 65 degrees were analysed. Thus, the participants were told that the knee angles were randomly selected. The test occasions involved five repetitions for each knee angle on each leg. An eight-camera motion capture system registered the knee joint movement of reflective markers on the participant. The intraclass correlation coefficient (ICC3,1) and standard error of the measurement (SEM) were used to report the result of the test-retest reliability. Results:. Our test indicated moderate- to good test-retest reliability where the knee flexion of 40 degrees showed a higher reliability than 65 degrees. Conclusions: This test has the potential to be developed for use in clinics to evaluate JPS. Further research should analyse the velocity component, whether there was a learning effect, and the impact of different angles.

    Syftet med studien var att jämföra motivation att delta i ett högintensivt funktionellt träningsprogram (HIFE) med motivation att delta i en stillasittande social kontrollaktivitet hos äldre med demens boende i särskilda boenden. 186 personer randomiserades till träning (n=93) eller kontroll (n=93). De hade en medelålder på 85 år och 76 % var kvinnor. De hade fysiska och kognitiva nedsättningar och aktivitetsbegränsningar. Interventionen pågick under 4 månader med totalt 40 tillfällen om 45 minuter vardera. Frågor om deltagarnas motivation och utmaningar för genomförandet av aktiviteterna besvarades av aktivitetsledarna och personal.

    I hela gruppen fanns signifikant korrelation mellan motivation innan och under aktivitet. Motivation innan aktivitet skiljde sig signifikant till fördel för kontrollgruppen  vid ett av fyra mättillfällen. De största hindren för att utföra aktiviteterna var för träningsgruppen “låg motivation” och “smärta” och för kontrollaktiviteten “trötthet” och “minnesnedsättning”. 

    Äldre personer med demens som bor på serviceboende och är beroende i dagliga aktiviteter är i vår studie lika motiverade att delta i funktionellt högintensivt träningsprogram jämfört med social kontrollaktivitet. Därför kan funktionell högintensiv träning rekommenderas när sociala  aktiviteter blir svåra att genomföra pga. kognitiva nedsättningar.

    Abstract [en]

    Intra-abdominal pressure (IAP) increases during many tasks and has been argued to increase stability and stiffness of the spine. Although several studies have shown a relationship between the IAP increase and spinal stability, it has been impossible to determine whether this augmentation of mechanical support for the spine is due to the increase in IAP or the abdominal muscle activity which contributes to it. The present study determined whether spinal stiffness increased when IAP increased without concurrent activity of the abdominal and back extensor muscles. A sustained increase in IAP was evoked by tetanic stimulation of the phrenic nerves either. unilaterally or bilaterally at 20 Hz (for 5 s) via percutaneous electrodes in three subjects. Spinal stiffness was measured as the force required to displace an indentor over the L4 or L2 spinous process with the subjects lying prone. Stiffness was measured as the slope of the regression line fitted to the linear region of the force-displacement curve. Tetanic stimulation of the diaphragm increased IAP by 27-61% of a maximal voluntary pressure increase and increased the stiffness of the spine by 8-31% of resting levels. The increase in spinal stiffness was positively correlated with the size of the IAP increase. IAP increased stiffness at L2 and L4 level. The results of this:study provide evidence that the stiffness of the lumbar spine is increased when IAP is elevated.

    Introduktion: Majoriteten av sveriges barn uppnår mindre än de rekommenderade 60 minuterna måttlig till kraftfull fysisk aktivitet (FA) per dag. Tidigare forskning indikerar att träning och FA i skolan kan förbättra resultaten i teoretiska ämenen, och att utökad FA inte försämrar resultaten.

    Syfte: Syftet med studien är att utifrån senaste forskningen sammanställa rekommenderad organiserad FA för skolbarn under skoltid för att maximera resultat i teoretiska ämnen och kognitiv prestation. 

    Metod: Systematisk litteraturgranskning enligt rekommendationer från en handbok av Statens beredning för medicinsk utvärdering (SBU).

    Resultat: Efter sållning kvalitetsgranskades 27 artiklar varav 18 bedömdes ha medel- eller hög kvalitet, de inkluderades därmed i den narrativa sammanvägningen. Resultaten från studierna kategoriserades mellan idrottslektioner, fysiskt aktiva teoretiska lektioner och fysiskt aktiva pauser. Studierna involverar barn och ungdomar i åldrarna 6–16 år och interventionernas duration varier från ett testtillfälle under en dag till en intervention över hela grundskolan, 9 år.

    Konklusion: Spridningen av inkluderade studiers interventioner och resultat är stor och det finns ett behov av vidare, mer omfattande, forskning. Med nuvarande evidensläge rekommenderar vi Sveriges grundskolor att överväga daglig FA av högre aerob intensitet med inslag av motorisk träning under idrottslektioner på 30-55 minuter. De äldre skolungdomarna rekommenderas även ha minst 30 minuter FA paus per dag med lägst moderat intensitet.

    Syfte: Att undersöka skillnader i motivationsprofil och mentalt välbefinnande mellan individuella idrottare och lagidrottare på elitnivå. Ytterligare syften var att studera skillnader i mentalt välbefinnande mellan elitidrottare och svensk normalpopulation, samt eventuella samband mellan motivationsprofil och mentalt välbefinnande.

     

    Metod: Självskattningsformulären BREQ-3 och HADS besvarades online av 80 elitidrottare, 45 individuella idrottare och 35 lagidrottare. Svarsenkäterna analyserades med avseende på skillnader i träningsmotivation och mentalt välbefinnande mellan de två grupperna. Även skillnader i mentalt välbefinnande mellan elitidrottarna och normativ data för Sverige analyserades.

     

    Resultat: Ingen signifikant skillnad mellan grupperna gällande motivationsprofil eller mentalt välbefinnande påvisades. Analysen visade ett signifikant samband mellan motivationsprofil och mentalt välbefinnande. Elitidrottarna skattade högre ångestnivåer men lägre depressionsnivåer jämfört med normalbefolkningen.

     

    Konklusion: Varken motivationsprofil eller mentalt välbefinnande skiljer sig för individuella idrottare och lagidrottare på elitnivå. En mer självbestämmande motivationsprofil vilket bygger på en hög grad av inre motivation, är associerat med mentalt välbefinnande. Detta innebär att en inre motivation bör eftersträvas inom elitidrotten för att öka välbefinnandet. Detta resultat indikerar även att psykisk ohälsa hos elitidrottare bör studeras ytterligare.

    Introduktion: Många inom tandvårdsyrken har spänningsrelaterade besvär men det råder brist på tidigare studier om hur tandvårdspersonal upplever och hanterar muskulär spänning. Syfte: Syftet med den här uppsatsen var att beskriva hur tandvårdsstudenter och tandvårdspersonal uppfattar, upplever och hanterar muskulär spänning. Metod: Studien utgick från en kvalitativ design med semistrukturerade individuellla intervjuer. Vid analysen av intervjuerna användes en kvalitativ innehållsanalys. Åtta personer som antingen arbetade som tandvårdspersonal eller studerade till det deltog i intervjuerna. Resultat: Resultatet från intervjuerna presenterade en bred bild av hur tandvårdsstudenter och personal upplevde och hanterade muskulär spänning. Innehåller i intervjuerna utmynnade i 5 kategorier: Spänning uppfattas som en rörelseinskränkning, Fysiska besvär och spänning kopplas till arbetsställningar och långa behandlingar, Höga prestationskrav leder till stress och spänningar, Medveten hållning och träning används som strategier samt Oro för vad spänningar skulle kunna leda till i framtiden. Konklusion: Muskulär spänning uppfattades som en ofrivillig muskelaktivering som kunde leda till en rörelseinskräkning och undvikande av rörelser. Spänning upplevdes bland annat som en stramhet och smärta. Den upplevda spänningen hanterades genom fysisk aktivitet, ergonomiska justeringar och strävan mot att släppa prestationskrav. Undervisning om ergonomi och tips om träning av fysioterapeut upplevdes underlätta för tandvårdsstudenter och tandvårdspersonals uppelvda spänning. Framtida forskning inom ergonomiska hjäpmedel och behandlingsmetoder för muskuloskeletala besvär är nödvändigt hos denna yrkesgrupp.

    Sammanfattning:

     Introduktion:

    Fysisk aktivitet och träning är viktiga faktorer för att hålla sig hälsosam och frisk. Trots att fysisk inaktivitet kan påverka både den fysiska och psykiska hälsan negativt så är 1,5 miljarder vuxna i hela världen inaktiva. Att förstå drivkrafterna till varför man väljer att vara fysiskt aktiv eller inte är viktigt. Tidigare studier har visat att olika typer av motivation kan vara av olika betydelser för motionärer jämfört med det som driver elitidrottare till att träna.

     Syfte:

    Syftet med denna studie var att undersöka skillnader i motivationsfaktorer bland elitidrottare inom längdskidåkning och orientering jämfört med motionärer. Hypotesen var att elitidrottarna bör ha mer inre motivation och att motionärer styrs av yttre faktorer.

     Metod:

    Självskattningsformuläret BREQ-3 delades ut till elitidrottande längdskidåkare och orienterare samt motionärer på Umeå Universitet och genom sociala medier. Tjugoen stycken elitidrottare och 25 stycken motionärer svarade på enkäten. Svaren sammanställdes och analyserades med avseende på skillnader i graden av inre och yttre motivation samt icke-motivation mellan de båda grupperna.

     Resultat:

    Resultatet visade på en signifikant skillnad i motivationsfaktorer mellan elitidrottare och motionärer där motionärerna hade en högre grad av icke-motivation och elitidrottarna hade en högre grad av inre motivationsfaktorer och av de yttre motivationsfaktorerna integrerad, identifierad och introjected reglering.

     Konklusion:

    Studien visade att elitidrottare (inom längdskidåkning och orientering) tränar framförallt för att nå ett mål och värderar fördelarna med sin träning medan motionärerna inte är lika motiverade till fysisk träning.

    ABSTRAKT

    Introduktion:

    Nackbesvär är ett samhällsproblem som kan bero på både fysiska och psykiska orsaker. Ett tänkbart fenomen som skulle kunna ligga bakom en del utav besvären är creep. Begreppet creep innebär att vid långvarig flexion så töjs den viskoelastiska vävnaden ut. Det i sin tur skulle kunna leda till instabilitet runt den drabbade leden.

    Syfte:

    Syftet med studien är att undersöka om creep vid statisk flexion av nacken i ett avlastat läge kommer att öka det maximala rörelseomfånget i extension/flexion hos friska kvinnor.

    Metod:

    Den valda metoden är en tvärsnittsstudie med jämförelser mellan två kontrollgrupper. Försökspersonerna var kvinnor mellan 20-30 år (medelålder 24.9) utan pågående nackbesvär. De två exponeringstyperna var neutral nackposition samt 30° avlastad flexion i nacken med en duration på 30 min. Rörelseomfånget mättes med ett elektromagnetiskt tracker system (FASTRAK ™, Polhemus Inc, USA).

    Resultat:

    Det första resultatet visade en minskning av det totala rörelseomfånget från flexion till extension (P = 0.0006) efter testpersonerna utsatts för exponering 1 (flexion 30°). Samma skillnad kunde inte ses vid exponering 2 (neutral nackposition). Vidare explorativa mätningar visade att denna minskning främst kommer från den övre nackleden i extension (P = 0.03).

    Diskussion:

    Mätningarna utfördes på hela halskotpelaren trots att den anatomiskt sett skiljer sig åt när det kommer till de övre/nedre nacksegmentet. En förändring av flexion-relaxations fenomenet (FRP) skulle kunna leda till denna minskning av rörelseomfånget. Tiden mellan testen, gradantal, neutral huvudposition samt litet urval kan vara påverkande faktorer för vårt resultat.

    Slutsats: Det sker en minskning utav rörelseomfånget vid flexion i 30° i 30 min. Minskningen sker främst i övre nackleden i extension. Resultatet var inte vad vi hade förväntat oss men det var tydligt att exponeringen faktiskt påverkade rörelseomfånget. Vi skulle därför säga att mer forskning inom området är nödvändig för att förklara vilken mekanism/er som ligger bakom.

    BACKGROUND: Despite interventions, anterior cruciate ligament ruptures can cause long-term deficits. To assist in identifying and treating deficiencies, 3D-motion analysis is used for objectivizing data. Conventional statistics are commonly employed to analyze kinematics, reducing continuous data series to discrete variables. Conversely, functional data analysis considers the entire data series.

    METHODS: Here, we employ functional data analysis to examine and compare the entire time-domain of knee-kinematic curves from one-leg hops between and within three groups. All subjects (n=95) were part of a long-term follow-up study involving anterior cruciate ligament ruptures treated ~20years ago conservatively with physiotherapy only or with reconstructive surgery and physiotherapy, and matched knee-healthy controls.

    FINDINGS: Between-group differences (injured leg, treated groups; non-dominant leg, controls) were identified during the take-off and landing phases, and in the sagittal (flexion/extension) rather than coronal (abduction/adduction) and transverse (internal/external) planes. Overall, surgical and control groups demonstrated comparable knee-kinematic curves. However, compared to controls, the physiotherapy-only group exhibited less flexion during the take-off (0-55% of the normalized phase) and landing (44-73%) phase. Between-leg differences were absent in controls and the surgically treated group, but observed during the flight (4-22%, injured leg>flexion) and the landing (57-85%, injured leg<internal rotation) phases in the physiotherapy-only group.

    INTERPRETATION: Functional data analysis identified specific functional knee-joint deviations from controls persisting 20years post anterior cruciate ligament rupture, especially when treated conservatively. This approach is suggested as a means for comprehensively analyzing complex movements, adding to previous analyses.

    Background: Anterior cruciate ligament (ACL) ruptures may lead to knee dysfunctions later in life. Single-leg tasks are often evaluated, but bilateral movements may also be compromised. Our aim was to use curve analyses to examine double-leg drop–jump kinematics in ACL-reconstructed, ACL-deficient, and healthy-knee cohorts.

    Methods: Subjects with unilateral ACL ruptures treated more than two decades ago (17–28 years) conservatively with physiotherapy (ACLPT, n = 26) or in combination with reconstructive surgery (ACLR, n = 28) and healthy-knee controls (n = 25) performed 40-cm drop–jumps. Three-dimensional knee, hip, and trunk kinematics were analyzed during Rebound, Flight, and Landing phases. Curves were time-normalized and compared between groups (injured and non-injured legs of ACLPT and ACLR vs. non-dominant and dominant legs of controls) and within groups (between legs) using functional analysis of variance methods.

    Results: Compared to controls, ACL groups exhibited less knee and hip flexion on both legs during Rebound and greater knee external rotation on their injured leg at the start of Rebound and Landing. ACLR also showed less trunk flexion during Rebound. Between-leg differences were observed in ACLR only, with the injured leg more internally rotated at the hip. Overall, kinematic curves were similar between ACLR and ACLPT. However, compared to controls, deviations spanned a greater proportion of the drop–jump movement at the hip in ACLR and at the knee in ACLPT.

    Conclusions: Trunk and bilateral leg kinematics during double-leg drop–jumps are still compromised long after ACL-rupture care, independent of treatment. Curve analyses indicate the presence of distinct compensatory mechanisms in ACLPT and ACLR compared to controls.

    The aim of this prospective study was to analyse joint and muscle function and aerobic capacity over 2 years in a group of patients with early rheumatoid arthritis (RA; i.e. symptomatic for <12 months) in relation to age, pain and disease activity. Sixty-six patients with early RA were followed for 24 months. Joint and muscle function was measured using Signals of Functional Impairment (SOFI) index and aerobic capacity with sub-maximal test on ergometer bicycle. Pain was estimated on Visual Analogue Scale (VAS) and disease activity using 28-joint count Disease Activity Score (DAS 28). Joint and muscle function was impaired in almost all patients at inclusion and after 24 months. Male patients were more affected compared with females and the significant correlation with pain and DAS 28 decreased over time. Aerobic capacity was maintained and not influenced by sex, age or pain but related significantly to disease activity. The majority of the patients with early RA had impairments of the joint and muscle function over 24 months, even though disease activity decreased significantly. There is a need for regular evaluations of physical functions besides disease activity to prevent continuous development of functional losses.

    Introduktion: En främre korsbandsskada (ACL-skada) kan för en idrottare innebära lång frånvaro från idrott. Att hitta rätt strategi för åtgärder utifrån skaderelaterade riskfaktorer är därför viktigt för att ta fram de bästa skadeförebyggande interventionerna.

    Syfte: Syftet med denna litteraturgranskning var att undersöka om träningsåtgärder för höften kan påverka knäets dynamiska stabilitet i potentiell skadeförebyggande riktning för ACL-skada.

    Metod: Litteratursökningen gjordes i databaserna PubMed, Cinahl, Medline och Web of Science. Studier som undersökte huruvida träningsåtgärder inriktade mot höften kunde förbättra den dynamiska stabiliteten i knäleden inkluderades i litteraturgranskningen. Studiepopulationen skulle vara unga och vuxna, män och kvinnor. Studietypen skulle vara antingen randomiserade kontrollerade studier eller kliniskt kontrollerade studier, som sedan kvalitetsgranskades enligt SBU:s mall för randomiserade studier.

    Resultat: Två studier inkluderades i litteraturgranskningen, dessa bedömdes ha ”låg” och ”medelhög” risk för bias. Studierna undersökte huruvida valgisering vid dynamiska test kunde förbättras efter en styrkeintervention fokuserad mot höften. Studiernas resultat visade inga signifikanta förändringar i knäkinematik trots signifikanta förbättringar i styrkan i höftmuskulatur.

    Slutsats: Det bristande underlaget av interventionsstudier på träningsåtgärder för höften, med fokus på knäets dynamiska stabilitet, gör det svårt att dra en säker slutsats kring området. Mer studier på området är önskvärt.

    Syfte Studien adresserar hur ett hindertest kan utvecklas och användas för att studera den proprioceptiva förmågan i knäleden under ett funktionellt test.

    Metod 25 försökspersoner rekryterades med varierande aktivitetsgrad. Elitidrottare, kontroller samt individer med främre korsbandsskada (ACL) delades in i tre grupper. Grupperna genomförde ett hindertest utan visuell feedback av hinderhöjden. Försökspersonerna fick ställa sig på en standardiserad startposition, vila det ledande benet på hindret för att skapa en efferenskopia. De försökte kliva över hindret med så liten marginal som möjligt utan att slå i hindret. Hindret flyttades utan individens vetskap. Försöken filmades i ett 3D-laboratorium. Två hinderhöjder användes, totalt gjordes 16 försök (åtta försök/ben och hinderhöjd). Avstånd i hinder till fot, knä- och höftflexionsvinkel samt hits studerades. Absoluta felet samt variationskoefficienten analyserades med oberoende t-test samt Mann Whitney u-test. Hits analyserades med Pearsons Chi2-test, signifikansnivån var p<0.05 i alla test.

    Resultat Testet var genomförbart för både knäfriska samt individer med främre korsbandsskador. Kontrollgruppen uppvisade lägre avstånd i hinder till fot för både det låga och höga hindret, för det sistnämnda var skillnaden signifikant (p=0.00). Atletgruppen uppvisade procentuellt mindre spridning i avstånd hinder till fot för både det låga och höga hindret, skillnaderna var icke signifikanta. En signifikant skillnad fanns mellan grupperna gällande antal hits, ACL-gruppen hade slagit i hindret flest gånger om detta inte flyttats under utförandet (p=0.021).

    Slutsats Att utveckla ett validerat och reliabelt mätinstrument av stor vikt för att under och efter rehabiliteringsfasen kunna utvärdera den proprioceptiva förmågan hos individer med en främre korsbandsskada.

    Kontorsarbetare spenderar lång tid stillasittande. Negativa hälsoeffekter förknippas med en stillasittande livsstil, däribland muskuloskeletal smärta. Lösningen verkar inte vara stillastående då även detta kan leda till muskuloskeletal smärta. Det finns HSB (höj- och sänkbara bord) för att minska stillasittande/stillastående hos kontorsarbetare. Mer forskning krävs gällande hur HSB används i praktiken.

    Syfte: Undersöka om det finns skillnad i användning av HSB mellan de som upplever jämfört med de som inte upplever rygg-och nacksmärta.

    Undersöka om det finns samband mellan självskattad tid stående respektive sittande och tid uppmätt med aktivitetsmätare.

    Metod: Data samlades in via självskattningenkäter som delades ut till 26 kontorsarbetare. Enkätena bestod av frågor gällande användning av HSB samt upplevd muskuloskeletal smärta. Kontorsarbetarna delades in i två grupper, smärtgruppen (SG) och icke- smärtgruppen (ISG), som jämfördes med varandra. Elva kontorsarbetare tilldelades aktivitetsmätare samt en självskattningsenkät gällande hur mycket de suttit och stått dagarna de bar aktivitetsmätaren. Sambandsanalyser mellan självskattad tid och tid uppmätt med aktivitetsmätare utfördes. 

    Resultat: Smärtgruppen varierade mer frekvent, satt mindre och stod mer men skillnaderna mot ISG var inte signifikanta (p=0,276; p=0,054; p=0,108). Svagt positiva samband (rho=0,46; rho=0,58) fanns mellan självskattad tid sittande respektive stående och tid uppmätt med aktivitetsmätare, dock var resultaten inte signifikanta (p=0,08; p=0,178).

    Konklusion: Denna studie visade inga signifikanta skillnader gällande användning av HSB mellan SG och ISG. Att SG varierade sig mer frekvent samt stod mer och satt mindre skulle dock kunna tyda på att smärta påverkar användandet av HSB. Mer forskning, fördelaktligen med aktivitetsmätare, krävs angående hur HBS används på kontorsarbetsplatser.

    Introduktion: Att arbeta inom yrken med nära mänskliga relationer kan innebära höga psykiska krav som kan leda till stressrelaterade symtom. Poliser har ett yrke med höga fysiska och psykosociala påfrestningar. Syftet med denna studie var att kartlägga arbetsrelaterade psykosociala faktorer och stressrelaterade symtom, och sambandet mellan dessa variabler bland patrullerande poliser i Sverige. 

    Metod: Totalt deltog 856 patrullerande poliser (437 kvinnor, 419 män) med en medelålder på 36.8 ± 9.5 år. Poliserna besvarade ett självskattningsformulär innehållande bakgrundsfrågor, arbetsrelaterade psykosociala faktorer och stressrelaterade symtom. Materialet analyserades via tvärsnittsdesign. Statistiska analyser genomfördes med Pearson Chi-Square och oberoende T-test. En bivariat analys genomfördes via Pearsons korrelation för att studera samband. Signifikansnivå p≤ 0.05. 

    Resultat: Inga signifikanta skillnader förkom mellan kvinnliga- och manliga patrullerande poliser vad gäller arbetsrelaterade psykosociala faktorer och stressrelaterade symtom. Mag- och sömnbesvär var sällan förekommande bland poliserna. Signifikanta samband påvisades mellan skattade höga krav, låg kontroll, lågt socialt stöd och upplevda mag- och sömnbesvär bland poliserna. Bland kvinnliga poliser förekom inget signifikant samband mellan kontroll och sömnbesvär, samt krav och magbesvär. 

    Slutsats: Svaga signifikanta samband tyder på att poliser som upplever låg kontroll, bristande stöd och höga krav även upplever sömn- och magbesvär.

    Abstract [en]

    ObjectivesWe developed clinical guidelines for the management of bone health in Rett syndrome through evidence review and the consensus of an expert panel of clinicians.

    MethodsAn initial guidelines draft was created which included statements based upon literature review and 11 open-ended questions where literature was lacking. The international expert panel reviewed the draft online using a 2-stage Delphi process to reach consensus agreement. Items describe the clinical assessment of bone health, bone mineral density assessment and technique, and pharmacological and non-pharmacological interventions.

    ResultsAgreement was reached on 39 statements which were formulated from 41 statements and 11 questions. When assessing bone health in Rett syndrome a comprehensive assessment of fracture history, mutation type, prescribed medication, pubertal development, mobility level, dietary intake and biochemical bone markers is recommended. A baseline densitometry assessment should be performed with accommodations made for size, with the frequency of surveillance determined according to individual risk. Lateral spine x-rays are also suggested. Increasing physical activity and initiating calcium and vitamin D supplementation when low are the first approaches to optimizing bone health in Rett syndrome. If individuals with Rett syndrome meet the ISCD criterion for osteoporosis in children, the use of bisphosphonates is recommended.

    ConclusionA clinically significant history of fracture in combination with low bone densitometry findings is necessary for a diagnosis of osteoporosis. These evidence and consensus-based guidelines have the potential to improve bone health in those with Rett syndrome, reduce the frequency of fractures, and stimulate further research that aims to ameliorate the impacts of this serious comorbidity.

    Introduktion: Stressrelaterad ohälsa är vanligt bland ensamkommande flyktingbarn. Fysioterapeutiska metoder mot stress har visat sig vara effektiva men kunskapen är otillräcklig om vilka metoder som passar denna grupp.

    Syfte: Att undersöka om fysioterapeutiska metoder, så som basal kroppskännedom, kan minska stressrelaterad ohälsa bland ensamkommande flyktingbarn samt hur dessa metoder upplevs och huruvida en interventionsstudie är genomförbar i denna typ av grupp.

    Metod: En interventionsstudie (n=2) genomfördes där Single-case research design kombinerades med mixed-method. Det kvantitativa datat utgjordes av enkäter och VAS vilket analyseras i tabeller och figurer medan den kvalitativa dataanalysen bestod av loggböcker samt intervjuer som analyserades med innehållsanalys.

    Resultat: Enkäter och VAS visade en tydlig minskning av kroppsliga besvär för den ene deltagaren medan det för den andre varierade. I loggböckerna framkom att de liggande avspänningsövningarna uppskattades mest samt att gruppträffarna var lärorika. Sju kategorier bildades i analysen av intervjuerna som slutligen formade temat: Görande och lärande tillsammans genom kropp och tanke.

    Konklusion: Vår interventionsmodell samt studiedesign är väl värda att vidareutveckla och utvärdera i exempelvis randomiserade och kontrollerade studier.

    Abstract [en]

    BACKGROUND AND AIM: Impaired finger dexterity is common after stroke, often affecting activities of daily living. Knowledge of kinematic characteristics and of underlying neurological mechanisms of such impairments is important to understand functional recovery. This study aims to investigate finger movement control and related brain activity patterns post-stroke (PS).

    METHODS: Data from a subsample including 9 participants PS with residual hemiparesis affecting manual dexterity (M age- 66; 3 female) and 12 able-bodied control (C) participants (M age- 65; 3 female) were analyzed. Two series of self-paced cyclic finger extension-flexion movements in random order were performed for each hand (4 series with vision, V, and 4 without vision, NV). Optoelectronic cameras monitored the 3D movement of markers affixed to the fingertips. Motion data was used to calculate each finger's individuation index (II), reflecting movement independence, each finger's Stationarity index (SI), reflecting the ability to keep the finger still while another moves [1] and Movement frequency (MF). Functional magnetic resonance imaging, with simultaneous movement recording, was used to investigate brain activity patterns in relation to the kinematic parameters. II, SI, MF and the effect of vision were analyzed for the 4th digit.

    RESULTS: A factorial ANOVA 2 [group] x 2 [condition] x 2 [side] x [index type] showed an effect for group (p < .0001; PS < C); condition (p < .01; NV < V); side (p < .0001; affected/non-preferred < non-affected/preferred); and index type (p < .0001; SI < II). An interaction between group and side (p < .01) showed that indices of the affected side were lower compared to the non-affected side within the PS group and compared to both sides in the C group. No significant effects were apparent for MF but significant correlations were found between the indices and MF that were restricted to the PS group alone (over all conditions- r = -0.22; p < .01; within the NV condition- r = -0.19; p < .01; within the affected side r = -0.15; p < .05; and within the SI categorization r = -0.14; p < .05). Furthermore, within NV for the non-affected hand on the SI alone (r = -0.54; p < .05). All indicate that slower movements had higher indices.

    DISCUSSION: The associations between slower MF and higher index values within the PS group were located to conditions with increased difficulty (NV, affected side, and SI). Thus, reducing speed may be a selected strategy to increase control of finger movements PS when the demand on motor control is high. Further, with the applied calculation of finger movement independence we were able detect group differences, side differences within the PS group, and a positive effect of vision of the hands during performance. This indicates that this calculation is a sensitive measure that could be used to study the effects of stroke and to monitor progression in motor recovery.     Research question: Objectives of the present study are: (1) to quantify finger movements in a 3D context and (2) by this method investigate the ability to perform individualized finger movements, with and without vision of the hands, in persons with a chronic stroke diagnosis compared to able-bodied controls.

    Introduction: Increased knowledge of how fine movement control is affected by stroke is important for the understanding of recovery of function. This is crucial for the development of reliable and valid assessment methods for evaluation of rehabilitation of the upper limbs. This study is part of the MOST project (MOST-MOvement control in STroke) where both clinical tests and 3D movement assessments are performed.

    Materials and methods: At present, 18 persons post-stroke (M age = 67 years; 6 women) and 26 able-bodied controls (M age = 62 years, 11 women) have participated. The ability to perform uni-manual individualized finger movements and the effect of vison of the hands were evaluated. Participants were instructed to move a specific finger in cyclic extension–flexion movements at the metacarpophalangeal joint, keeping the rest of the finger straight and the other fingers still, at a self-paced speed during 10 s (2 test series for each hand; 8 test series in total). The task was performed seated. The wrists were extended about 10° and fixated to a wooden frame with forearm support. Reflective markers were affixed to each fingertip and movements were recorded by optoelectronic cameras. Based on the positional change of the fingers during task performance, two indices ranging from 0-1 were calculated: (1) Individuation index (II) where the independence of each finger movement is shown and where 1 indicate complete independence, (2) stationary index (SI) where 1 indicate that the finger remains still when the other fingers move [1].

    Results: Our results show that it is possible to quantify individual finger movements by use of 3D movement analysis addressing the quality of movement performance in stroke survivors: all but 3 persons post-stroke were able to perform the task. Preliminary analyses (based on a subsample constituted of 8 post-stroke and 8 controls) verify that the test discriminated between groups where participants post-stroke had lower values on II and SI as compared to the control persons, the lowest values were observed for the middle and ring fingers. Ongoing analyses will show if vision influences the outcomes.

    Discussion: A set-up has been tested where individual finger movements can be quantified in 3D, and that discriminates between persons post stroke compared to controls. This advancement carries a promise for development of better assessment methods for recovery of function post-stroke.

    Sammanfattning

     Syfte: Syftet med denna studie var att undersöka dos respons förhållandet mellan cykling på högintensiv (HI) respektive lågintensiv (LI) nivå och dess påverkan på trycksmärttröskelen primärt samt kognitiv arbetsförmåga sekundärt.

    Metod: 10 studiedeltagare, samtliga studenter deltog i denna randomiserande 2x2 cross-over studie och rekryterades via annonsering på Umeå Universitet. Samtliga studiedeltagare deltog i två testtillfällen under en tvåveckorsperiod; HI och LI. Utfallsvariablerna bestod av trycksmärttrösklar i form av Pain Pressure Threshold test (PPT-test) och en kognitiv uppgift bestående av Stroop Colour Word test (SCW-test).

    Resultat: Trycksmärttröskelnivån ökade efter genomförd cykling kontra innan cyklingen, oberoende testbetingelse. Efter HI ökade medelvärdet för PPT-värdena signifikant för trapezius med 26,7% (p=0,047) och efter LI med 32,3% (p=0,037). PPT-värdet för quadriceps ökade med 22,8% (p=0,051) vid HI och 17,0% (p=0,203) vid LI. Ingen skillnad sågs mellan testbetingelserna (p=0,445). Deltagarna presterade bättre på SCW-test under cykling kontra innan oberoende testbetingelse, både i antalet avklarade omgångar (HI, p=0,041, LI, p=0,016) och felprocent (HI, p=0,008, LI, p=0,021). Ingen skillnad sågs mellan de båda testbetingelserna under aktivitet.

    Konklusion: Resultaten indikerar att studiedeltagarna fick en minskad smärta och förbättrade kognitiva förmågor i samband med utförd cykling Resultaten indikerar mot att trycksmärttröskeln och kognitionsförmågan ökar i samband med utförd cykling, inga signifikanta skillnader visades i resultatet mellan utfört HI kontra LI cykling. Studiens resultat bör tolkas med försiktighet då tillförlitligheten generaliserbarheten på resultaten studiedeltagarna är försämrad då studiedeltagarna utgjordes av en homogen grupp samt studiens låga, dålig styrka i studien, p.g.a. få studiedeltagare. Fortsatta studier inom området behövs för att kunna ge välgrundade rekommendationer på vilken intensitet och duration som cykling är mest lämplig för att uppnå bäst effekter på trycksmärttröskeln samt kognitiv arbetsförmåga.

    Background:

    Impaired balance is one of the main features in Parkinson´s Disease (PD), which can result in falls with consequences that can be crucial for people with PD. It is important to detect an impaired balance early in the disease in order to prevent falls.

    Aim:

    The aim of this study was to examine balance, subjective experienced unsteadiness and self-reported falls among patients with idiopathic Parkinson´s Disease, at the very first visit at the neurological clinic and over time during the first three years.

    Materials and methods:

    The subjects were collected from a prospective longitudinal study, the so called NYPUM-study. 150 patients with PD and 31 healthy controls were included. The measurements that have been used are the Berg Balance Scale (BBS), the Timed Up and Go (TUG) and a postural stability-test together with a questionnaire about falls and experienced unsteadiness.

    Results:

    There was statistically significant balance impairment already at the first visit at neurological clinic. The patients had to greater extent experienced unsteadiness and had a higher incidence of falls during the last year, compared to controls. After one year there was an improvement in the TUG. Between one and three year there was a worsening in postural responses seen in the postural stability-test. The balance in general did however not show a significant difference after three years compared to the baseline result.

    Discussion:

    Even though the measured balance impairment might be considered as relatively small, the experienced unsteadiness was apparent. It is therefore of great importance to pay attention to balance early in the disease and to ask the patients about their own experience, to prevent future falls.

    BACKGROUND: The presence of early balance impairment in patients with Parkinson's disease has not been fully investigated.

    PURPOSE: The purpose of this study was to examine balance and mobility, self-perceived unsteadiness, self-reported falls, and effects of medication on balance among patients at their first visit to a neurological clinic and during the ensuing five years.

    MATERIALS AND METHODS: The participants were collected from a prospective longitudinal study. One hundred and forty-five patients with idiopathic Parkinson's disease and 31 healthy controls were included. The outcome measures were the Berg Balance Scale, the Timed Up and Go, the Postural Stability test and a questionnaire.

    RESULTS: At their first visit to the neurological clinic, the patients performed less well on the Berg Balance Scale (p < 0.001, r = 0.36), the Timed Up and Go (p < 0.001, r = 0.32), and the Postural Stability test (p < 0.001, r = 0.35) compared with the controls. In addition, a higher percentage of the patients reported self-perceived unsteadiness (p < 0.001, phi = 0.47). During the ensuing five years, balance and mobility worsened both with and without medication (p < 0.01, r = 0.24-0.37), although with small median differences.

    CONCLUSIONS: Further studies are needed to confirm that minor balance impairments exist even at the time of diagnosis and worsen during the ensuing five years. IMPLICATIONS FOR REHABILITATION Impairments in balance and mobility may occur early in Parkinson's disease, especially in the elderly patients, and seem to worsen during the first five years. There is a need to use sensitive outcome measures and to ask the patients about unsteadiness and falls to detect balance impairment in this cohort. Parkinsonian medication has a limited effect on balance and may preferably be complemented with balance exercises to target balance impairment early in Parkinson's disease.

    Som hälso- och sjukvårdspersonal måste man kunna förmedla sin kunskap och nå ut till patienten för att uppnå en lyckad behandling. Den icke-verbala kommunikationen är en viktig del i detta utbyte och forskning har visat att denna kan påverka viktiga utfallsmått i behandlingen. Hälso- och sjukvårdspersonal behöver få kunskap om hur deras beteenden påverkar patientmötet för att sedan kunna ge en bättre vård.

       Syftet i denna studie var att söka svar på hur den icke-verbala kommunikationen i patientmötet påverkar patientens nöjdhet, följsamhet och upplevelse av yrkeskompetens. Detta syfte besvarades genom en systematisk litteraturöversikt. Ett inklusionskriterie för artiklarna i litteraturöversikten var att de skulle var publicerade år 2007-2017. Metodiken följde en handbok från Statens beredning för medicinsk utvärdering (SBU). Efter litteratursökning, relevansbedömning och kvalitetsgranskning kom 10 artiklar att användas till att bygga upp resultatet.

       Litteraturöversikten kunde presentera resultat på de tre undersökta utfallsmåtten. Att sitta framåtlutad mot patienten, ha mycket ögonkontakt samt ha ett uppmuntrande och varmt tonläge påverkar nöjdheten positivt. En professionell klädstil samt ett omtänksamt och empatiskt beteende gör att man uppfattas som mer kompetent. Den förmedlade yrkeskompetensen ihop med ett omtänksamt beteende kan sedan öka följsamheten.

       Det finns således indikeringar på att hälso-och sjukvårdpersonal bör vara medvetna om hur den icke-verbala kommunikationen påverkar patientmötet. Som hälso- och sjukvårdspersonal är det viktigt att veta vilka signaler man sänder ut och hur den icke-verbala kommunikationen kan användas för att optimera patientmötet.

    Abstract [en]

    Stroke is a common and multifaceted disease that often involves motor deficits in the upper limb. This thesis investigated reliability and validity of existing clinical assessments of upper limb function in persons with post-stroke symptoms and in non-disabled controls. Study I was conducted in a clinical setting where the Motor Evaluation Scale of Upper Extremity in Stroke patients (MESUPES) was assessed in persons post-stroke by two physiotherapists selected from a group of four.  Study II-IV took place in a motion analysis laboratory with an optoelectronic system. Kinematic measures and clinical measures were used to investigate the validity of the Arm Posture Score (APS), the Finger-to-Nose test (FNT), and the Standardised Nine Hole Peg test (S-NHPT) in persons post-stroke and non-disabled controls.

     

    The results showed that the MESUPES had a high inter-rater reliability while the concurrent validity was not fully confirmed. MESUPES has a maximum score of 58 and the minimal detectable change ranged from 5 to 8 for a confidence level of 80% and 95%. The Arm Posture Scores, which include either four or six arm movement variables, were able to distinguish between the affected and non-affected arms, as well as between the affected arm and the non-dominant arm of the controls. The total movement time of the FNT, which is a coordination test, was able to distinguish persons post-stroke from controls, at least at a group level. Movement smoothness, accuracy and compensation, obtained from kinematic analysis, were the most discriminative variables for the FNT. Smoothness was most strongly correlated with the timed FNT and had the greatest association with the variance of the timed FNT. For the S-NHPT, which is a dexterity test involving grasping and reaching, the movement times, smoothness and compensation discriminated between the stroke group and the control group. Persons post-stroke spent considerably more time in the grasp-related parts of the task compared to controls. Smoothness and upper limb impairments had the strongest correlation with the S-NHPT.

     

    In conclusion, the clinical measures used within stroke rehabilitation seem valid and reliable, although some limitations are highlighted by the kinematic assessment. MESUPES was shown to be a reliable assessment of upper limb movement quality after stroke. The kinematic analysis revealed that the timed FNT does not have sufficient discriminative validity at an individual level. The timed FNT reflected speed-related aspects of pointing movements such as smoothness and length of the deceleration phase, but should not be used as an overall measure of upper limb coordination after stroke. The timed S-NHPT demonstrated sufficient discriminative validity and reflected smoothness and upper limb impairments. For both the FNT and S-NHPT, kinematic analysis showed that the clinical outcomes of those tests (time of performance) did not adequately detect qualitative aspects of the upper limb movements after stroke such as possible compensatory movements. Therefore, clinical assessments that capture qualitative aspects of upper limb movements would improve the assessment of upper limb coordination and dexterity after stroke. In addition, 3D movement analysis provided unique information about upper limb function after stroke, not least in persons with mild post-stroke impairments. The APS, for instance, which quantifies how much the arm swing during gait deviates from normal, discriminated between persons with stroke and non-disabled persons. Such subtle deviations are not possible to quantify with the human eye.

    The purpose of the study was to apply the Arm Posture Score (APS) to a stroke population, since comprehensive measures to quantify arm swing in the affected and non-affected arms during gait are lacking. A further aim was to investigate how gait speed and upper limb function estimated by clinical measures are related to the APS in the stroke group. The APS is the summarized root mean square deviation (RMSD) from normal, based on kinematics. Four arm movements (sagittal and frontal planes) as well as six arm movements (incorporating transversal plane) were included in the calculation of APS, referred to as APS4 and APS6, respectively. The study population consisted of 25 persons with stroke and 25 age- and gender-matched controls. The APS measures were significantly different between the affected and non-affected arms, as well as between the affected arm and the non-dominant arm of the controls (p≤0.001). Spasticity significantly influenced both APS measures, while speed only had a significant effect on the APS4. The APS measures correlated significantly to clinical measures of upper limb function. Both APS measures seem to be useful indices to quantify and discriminate between impaired and normal arm swing during gait after stroke. The variability of rotational arm movements needs to be studied further before considering the additional value of the APS6 over the APS4. When interpreting the APS, complementary kinematics should be taken into account, as the single value of the APS gives no information about the direction of the deviation.

    BACKGROUND: Upper limb coordination in persons post-stroke may be estimated by the commonly used Finger-to-Nose Test (FNT), which is also part of the Fugl-Meyer Assessment. The total movement time (TMT) is used as a clinical outcome measure, while kinematic evaluation also enables an objective quantification of movement quality and motor performance. Our aims were to kinematically characterize FNT performance in persons post-stroke and controls and to investigate the construct validity of the test in persons with varying levels of impairment post-stroke.

    METHODS: A three-dimensional motion capture system recorded body movements during performance of the FNT in 33 persons post-stroke who had mild or moderate upper limb motor impairments (Fugl-Meyer scores of 50-62 or 32-49, respectively), and 41 non-disabled controls. TMT and kinematic variables of the hand (pointing time, peak speed, time to peak speed, number of movement units, path ratio, and pointing accuracy), elbow/shoulder joints (range of motion, interjoint coordination), and scapular/trunk movement were calculated. Our analysis focused on the pointing phase (knee to nose movement of the FNT). Independent t or Mann-Whitney U tests and effect sizes were used to analyze group differences. Sub-group analyses based on movement time and stroke severity were performed. Within the stroke group, simple and multiple linear regression were used to identify relationships between TMT to kinematic variables.

    RESULTS: The stroke group had significant slower TMT (mean difference 2.6 s, d = 1.33) than the control group, and six other kinematic variables showed significant group differences. At matched speeds, the stroke group had lower accuracy and excessive scapular and trunk movements compared to controls. Pointing time and elbow flexion during the pointing phase were most related to stroke severity. For the stroke group, the number of movement units during the pointing phase showed the strongest association with the TMT, and explained 60% of the TMT variance.

    CONCLUSIONS: The timed FNT discriminates between persons with mild and moderate upper limb impairments. However, kinematic analysis to address construct validity highlights differences in pointing movement post-stroke that are not captured in the timed FNT.

    BACKGROUND: Impairments in dexterity after stroke are commonly assessed by the Nine Hole Peg Test (NHPT), where the only outcome variable is the time taken to complete the test. We aimed to kinematically quantify and to compare the motor performance of the NHPT in persons post-stroke and controls (discriminant validity), to compare kinematics to clinical assessments of upper extremity function (convergent validity), and to establish the within-session reliability.

    METHODS: The NHPT was modified and standardized (S-NHPT) by 1) replacing the original peg container with an additional identical nine hole pegboard, 2) adding a specific order of which peg to pick, and 3) specifying to insert the peg taken from the original pegboard into the corresponding hole of the target pegboard. Eight optical cameras registered upper body kinematics of 30 persons post-stroke and 41 controls during the S-NHPT. Four sequential phases of the task were identified and analyzed for kinematic group differences. Clinical assessments were performed.

    RESULTS: The stroke group performed the S-NHPT slower (total movement time; mean diff 9.8 s, SE diff 1.4), less smoothly (number of movement units; mean diff 0.4, SE diff 0.1) and less efficiently (path ratio; mean diff 0.05, SE diff 0.02), and used increased scapular/trunk movements (acromion displacement; mean diff 15.7 mm, SE diff 3.5) than controls (P < 0.000, r ≥ 0.32), indicating discriminant validity. The stroke group also spent a significantly longer time grasping and releasing pegs relative to the transfer phases of the task compared to controls. Within the stroke group, kinematics correlated with time to complete the S-NHPT and the Fugl-Meyer Assessment (rs 0.38-0.70), suggesting convergent validity. Within-session reliability for the S-NHPT was generally high to very high for both groups (ICCs 0.71-0.94).

    CONCLUSIONS: The S-NHPT shows adequate discriminant validity, convergent validity and within-session reliability. Standardization of the test facilitates kinematic analysis of movement performance, which in turn enables identification of differences in movement control between persons post-stroke and controls that may otherwise not be captured through the traditional time-based NHPT. Future research should ascertain further psychometric properties, e.g. sensitivity, of the S-NHPT.

    Sammanfattning:

    Syfte. Syftet med studien var att undersöka samband mellan gånghastighet (framlänges och baklänges), balans och kognition vid demenssjukdom.

     

    Metod. Designen var en tvärsnittsstudie där 161 personer med demenssjukdom inkluderades. Deltagarna undersöktes för gånghastighet framåt och bakåt på en sträcka på 2.4 meter samt med Bergs balansskala, Mini-Mental State Examination, Verbalt Flöde och Alzheimers Disease Assessment Scale. Bivariata korrelationsanalyser utfördes mellan variablerna. Multivariata analyser utfördes sedan mellan samtliga variabler utom Alzheimers Disease Assessment Scale.

     

    Resultat. Balans visade en signifikant och måttlig korrelation till gånghastighet både framåt (r=.698) och bakåt (r=.527). Verbalt Flöde visade en signifikant, men låg korrelation (r=.249) till gånghastighet framåt. Verbalt Flöde och Mini-Mental State Examination visade signifikant, men låg korrelation (r=.294 och r=.248) till gånghastighet bakåt. Alzheimers Disease Assessment Scale var inte korrelerad till gånghastighet varken framåt eller bakåt i denna studie.

     

    Slutsats. Det finns samband mellan gånghastighet, kognitiv funktion och balans hos personer med demenssjukdom. Balans tenderar att vara mer korrelerat till framlängesgång än baklängesgång, medan kognitiv funktion förefaller ha ett starkare samband med baklängesgång än framlängesgång.

    How forward and backward walking, both central to everyday life, relate to cognition are relatively unexplored in people with dementia. This study aimed to investigate if forward and backward walking velocity respectively, associated with global cognition and executive function in people with dementia, and whether the association differed according to walking aid use or dementia type. Using a cross-sectional design, 161 participants (77% women), a mean Mini-Mental State Examination (MMSE) score of 15, and mean age of 85.5 years and living in nursing homes were included. Self-paced forward walking (FW) and backward walking (BW) velocity over 2.4 m was measured. Global cognitive outcome measurements included MMSE and Alzheimer Disease Assessment Scale - Cognitive subscale (ADAS-Cog). Executive function was measured using Verbal Fluency (VF). In comprehensively adjusted multivariate linear regression analyses, FW was independently associated with VF (p = 0.001), but not MMSE (p = 0.126) or ADAS-Cog (p = 0.818). BW was independently associated with VF (p = 0.043) and MMSE (p = 0.022), but not ADAS-Cog (p = 0.519). Interaction analyses showed that the association between BW velocity and executive function were stronger in participants who walked without a walking aid. No associations differed according to dementia type. In conclusion, executive function appears important to walking velocity, both forward and backward, in people with dementia with mild to moderately severe cognitive impairment. Global cognitive function was associated with backward walking only, perhaps due to it being more challenging. The association between BW velocity and executive function differed according to use of walking aids, which appeared to attenuate the association.

    Sammanfattning:

    Övervikt och fetma är idag ett stort samhällsproblem. Utöver ekonomisk börda för samhället kan detta problem även innebära stora påfrestningar för den enskilde individen. På Arbets- och Beteendemedicinskt centrum på Norrlands Universitetssjukhus behandlas patienter för att minska risken för sjukdomar och ohälsa till följd av ett för högt Body Mass Index (BMI). Patienterna får möjlighet att genomföra en Low-Calorie Diet (LCD), maximalt 900 kcal per dag. För att utvärdera behandlingen används ett konditionstest på cykel, Åstrands, före och efter påbörjad behandling.

    Syfte. Syftet med denna studie är att undersöka om det finns skillnader i testdata insamlad i samband med konditionstest på cykel som utförts före och 2-4 veckor in på en LCD, hos individer med fetma. Metod. I den här studien har två konditionstest på ergometercykel gjorts, ett före dietstart och det andra två till fyra veckor in på dieten. Testgruppen bestod av 11 individer, fyra män, sju kvinnor. 39-61 år. BMI >32. Alla deltagare har genomgått en LCD med måltidsersättning. Testdata bearbetades i SPSS och bestod av BMI, puls minut 1, puls minut 6, testvärde och VO2 max korrigerat efter Åstrands tabell.

    Resultat. En signifikant skillnad i BMI och vikt på gruppnivå sågs (p=0.003). Ingen signifikant skillnad i testresultat mellan de två testerna kunde påvisas vad gäller Puls minut 1, Puls minut 6, VO2 max, Testvärde för Åstrands cykeltest (p<0.05). Konklusion. Det går i dagsläget inte att säga att det är någon skilland på testdata insamlad i samband med konditionstest på cykel som utförts före och 2-4 veckor in på en LCD, hos individer med fetma.

    Syftet med studien var att undersöka om samband fanns mellan graden av fysisk aktivitet, stillasittande och upplevda stressrelaterade symtom, kopplade till arbetsmiljön hos anställda på en socialförvaltning.

    Data samlades i en tvärsnittsstudie genom en webbenkät. Två frågor om fysisk aktivitet och en fråga om stillasittande lades till i slutet av enkäten.

    Studiepopulationen bestod av tillsvidareanställda inom socialförvaltningen Östhammars kommun. Resultaten analyserades genom Spermans rh0 och chi två test. Av 741 respondenter svarade 457 stycken (61,7 %). Av dessa var 43 män och 414 kvinnor.

     

    Mellan fysisk aktivitet och stressrelaterade symptom fanns ett mycket svagt signifikant samband (rh0= -0,111), p=0,023) som visade att ju mer fysisk aktivitet desto mindre symptom upplevdes. Något samband sågs ej mellan stillasittande och upplevda symptom. Det var inte heller någon skillnad i upplevda stressrelaterade besvär mellan de som uppnår rekommenderade nivåer för hälsofrämjande fysisk aktivitet jämfört med de som inte uppnår rekommenderade nivåer.

     

    Tidigare forskning visar att fysisk aktivitet fortsatt kan vara en viktig del för vidare arbete med hälsopromotion och prevention avseende stressrelaterade symptom vilket vi dock inte kunde se något tydligt belägg för i denna undersökning. Här behövs vidare forskning avseende vilka nivåer och vilka typer av aktiviteter som kan ge bäst effekt. Det behövs också belysas tydligare vilka andra faktorer och eventuella samband, som förutom fysisk aktivitet behöver beaktas vid planering av förebyggande hälsoinsatser för stressrelaterade symptom.

    Introduktion: Det finns anledning att misstänka en hög fysisk arbetsbelastning hos skogsröjare i Sverige, men bristen på yrkesspecifik statistik och studier medför svårigheter att identifiera problemområden. Eftersom en för hög fysisk arbetsbelastning kan leda till arbetsrelaterade besvär behövs studier som identifierar problemområden för skogsröjare. Syfte: Att kartlägga förekomsten av muskuloskeletala besvär samt att undersöka den generella fysiska arbetsbelastningen hos en grupp skogsröjare i Sverige. Metod: I denna studie deltog 28 män och 2 kvinnor. För att kartlägga förekomsten av muskuloskeletala besvär svarade samtliga forskningspersoner på en webbaserad version av Nordic Musculoskeletal Questionnaire (NMQ). Av dessa forskningspersoner deltog fyra män och en kvinna i hjärtfrekvensmätningar över en arbetsdag. Forskningspersonernas genomsnittliga hjärtfrekvens och hjärtfrekvens i vila användes tillsammans med uppskattad maximal hjärtfrekvens för att räkna ut hur stor del av hjärtfrekvensreserven som nyttjades. Resultat: Resultatet från NMQ undersökningen visar att 28 av 30 hade någon form av muskuloskeletala besvär de senaste 12 månaderna. Besvärsförekomsten var vanligast i ländryggen (66.7%), knän (53,3%) och skuldror/axlar (40%). Hjärtfrekvensmätningarna visade att forskningspersonerna i genomsnitt arbetade på 39,6% av hjärtfrekvensreserven. Konklusion: Resultaten i denna studie tyder på att förekomsten av muskuloskeletala besvär är vanligt förekommande hos skogsröjare i Sverige och att de arbetar på en generell fysisk arbetsbelastning över de föreslagna rekommendationerna.

    Introduktion: Under fysiskt arbete påverkas kognitiv funktion på olika sätt men det finns idag inget kliniskt användbart testprotokoll. 

    Syfte: Syftet var att utforma ett kliniskt användbart testprotokoll samt att med detta undersöka hur friska försökspersoners kognitiva funktion direkt påverkades av ett fysiskt arbete.

    Metod: Fyrtiofem friska försökspersoner (medelålder 25 år, medelvikt 70 kg) män och kvinnor utförde fyra kognitiva tester i vila. Därefter utfördes samma tester under fysiskt arbete på cykel vid ca 70 % av ålderspredicerad maxpuls. Testerna gick ut på att så snabbt och korrekt som möjligt, nämna numren 1 till 20 samt årets månader baklänges respektive framlänges. Dessutom beräknades differensen av tiden det tog att nämna numren respektive månaderna baklänges jämfört med framlänges (Reverse effect).

    Resultat: För flertalet deltester påvisades ingen skillnad i kognitiv funktion mellan vila och fysiskt arbete. En liten, men signifikant, skillnad noterades för deltestet räkna 1-20 framlänges som gick 0,63 sekunder långsammare under arbete jämfört med vila (p=0,001). En liten signifikant skillnad noterades även för Reverse effect för nummertestet som var 0.46 sekunder lägre under arbete jämfört med vila (p= 0,042).

    Konklusion: Då resultatet inte visade på någon större effekt av arbete ansågs testprotokollet vara för lätt för unga och friska personer. Ändå var testprotokollet lätt att genomföra, tog relativt kort tid samt genererade data som till stor del var normalfördelad. Därför tycks resultatet och testmetodiken vara möjlig att använda som referensdata samt mätmetod för att undersöka den kognitiva funktionens effekt av arbete på en matchad population med kognitiva nedsättningar.

    Metod:!denna  studie tillämpades scmistrukturerade  intenjuer som analyserades med kvalitat iv innehällsanalys enligt Grancheim och Lundman. Totalt intenjuades tio gymnasieungdomar från en av de större kommunala gymnasieskolorna i Umeå.Deras ålder, kön och utbildningsprogram varierade.

     

    Resultat:Analysen av intenjuerna resulterade itemat Den osunda sundheten samt fem kategorier: Träning och hälsa som tävling och hets,Jakten p å bekräftelse, Ungapressade av styrande och orimliga ideal, Unga behöver skyddsnät och Träning och hälsa som

    välmående. Resultatet visade på att de intervjuvade ungdomarna uppfattade att det råder en hälsobets isamhället som unga påverkas mycket av. De upplevde att det finns osunda kroppsideal som kan leda till ortorexi. Vidare upplevdes sociala media ha stor makt och bidra till hälsohetsen. Omgivningen uppmärksammades som ett betyd elsefullt skydd mot hälsobetsen.

     

    Slutsats:Resultatet visade att de intervjuvade ungdomarna uppfatt ade att det finns en hälsohets isamhället som bidrar till orimliga och osunda kroppsideal. Det är viktigt att fysioterapeuter har god kunskap och medvetenhet om de hälso- och kroppsideal som påverkar u ngas inställning till hälsa, mat och träning.Eftersom fysioterapeuter möter många unga kan de fånga eventuella problem iett tidigt skede.

    Syfte: Huvudsyftet med denna studie var att utforska hur gången är påverkad hos personer som drabbats av en stroke gällande avvikelse, rörelsemönster och asymmetri i armar och ben genom att använda tredimensionell analys. Sekundära syften med studien var att undersöka hur kinematisk testning korrelerar med kliniska test och utforska hur spasticitet påverkar gången baserat på dessa test. Metod: Tjugofem personer som drabbats av en stroke och 25 friska köns- och åldersmatchade kontroller undersöktes med tredmiensionell analys. Strokegruppen undersöktes också med klinska test. Resultat: Strokegruppen uppvisade avvikelser från kontrollerna i GVS för det påverkade knät och fötterna (p<0,001), men inte på GPS. I det påverkade benet hade strokegruppen lägre total ROM (p<0,001). Det fanns korrelationer mellan GPS på det påverkade benet och gånghastighet (rs=-0,79), Fugl-Mayer Assessment (rs=-0,70) och 30-second chair-stand test (rs=-0,47). Subgruppen som var spastiska efter sin stroke hade högre GPS i båda benen, gick långsammare, hade en mer asymmetrisk gång och lägre total ROM på den påverkade sidan jämfört med kontrollgruppen. Slutsats: GPS kunde inte upptäcka kinematiska avvikelser mellan grupperna i denna studie och kan kanske inte är lämpligt för att upptäcka gångavvikelser på grupper med personer med lättare gångnedsättningar efter stroke. Vi föreslår att GVS och ROM också bör undersökas för att få en mer komplett förståelse för kinematiken av gången. Klinska tester och gångförmåga kan indikera att gången är förändrad. Spasticitet har en negativ påverkan på gångparametrar.

    Introduktion: Få studier har tidigare utfört Stroops test under gång på äldre personer. Resultaten har visat att det går att särskilja personer som fallit tidigare och att det tidigt går att upptäcka en kognitiv försämring. Dessa studier saknar dock tydliga instruktioner om hur testet ska genomföras och hur tiderna ska tolkas i förhållande till antal felsteg.

    Syfte: Syftet var att undersöka genomförbarheten av Stroops test under gång för äldre personer. Dessutom undersöka om det gick att se några samband mellan vanlig gånghastighet, vanligt Strooptest och gång på Stroopmatta.

    Metod: Till studien rekryterades 5 personer (2 män och 3 kvinnor) över 70 år genom ett bekvämlighetsurval. Varje deltagare fick utföra ett vanligt Strooptest sittande, test av gånghastighet på svart matta och Stroops test under gång. Slutligen fick de besvara några frågor om hur de upplevde testerna.

    Resultat/Konklusion: Resultaten visade att inga signifikanta samband fanns mellan de olika testvärdena. Stroops test under gång var genomförbart för äldre personer över 70 år. Testet var enkelt att genomföra och deltagarna förstod lätt instruktionerna. Fler studier behöver dock göras för att utveckla testet vidare, bland annat för att granska vilken patientgrupp som testet är lämpligt att utföras på samt för att finna normalvärden.

    This study investigated the use of robotic technology for promoting attention, communication and social skills in adolescents with autism spectrum disorder (ASD). Attention, communication and social skills were measured while participants played a memory card matching game (Face Match) using (a) a humanoid robot, (b) a Smart Board and (c) playing cards. Three participants with ASD and three with other cognitive impairments were recruited from a secondary school with a special needs unit. Participants were paired such that one of each pair had a diagnosis of ASD and Face Match was played in these pairs for similar to 15 min, with a game organizer present. On 3 separate days, video recordings were made as the participants played Face Match; a different game mode (robot, Smart Board, playing cards) was used each day. A system for categorizing attention, communication and social skills was developed that described 16 subcategories of interactions and intra-actions. In general, participants with ASD showed highly individualized patterns of behaviour in the three different modes. However, repetitive behaviour was reduced in participants with ASD when using both the robot and the Smart Board compared with playing cards. We show that it is feasible to use a robot to assist teaching of social skills to adolescents with ASD, but suggest that the robot features could be further explored and utilized.

    Introduktion/Syfte: Fallolyckor skapar mycket stora  kostnader för samhället. Forskning visar att träning av balans och benstyrka har god effekt på att förebygga fall. Dagens tekniska utveckling ger nya möjligheter för applikationer att sprida och motivera till träning. Mobilapplikationen Säkra steg har tagits fram i syfte att göra fallpreventiv träningen mer tillgänglig och vara ett stöd för äldre att genomföra träningen. Uppsatsens syfte är att undersöka skillnader med avseende på ålder, kön, tidigare fall, fallrädsla, tidigare yrke, mobilvana, fysisk aktivitet, balans och benstyrka, mellan en grupp seniorer som valt att träna med Säkra Steg och en grupp som valt att träna med hjälp av modifierad Otagoträning i broshyrformat.

     

    Material/Metod: Studiepopulationen består av 41 individer efter bortfall av 10 individer, alla över 70 år.  Deltagarna rekryterades från pensionärsorganisationer och har själva fått välja träningsgrupp, 20 i Otagogruppen och 21 i Säkra Steg gruppen. Data samlades in från en telefonintervju gällande ålder, kön, tidigare fall, fallrädsla, tidigare yrke, mobilvana, fysisk aktivitet samt fysiska test;  Short Physical Performance Battery och 30 second chair stand test. 

     

    Resultat: Grupperna uppvisade signifikanta skillnader i könsfördelning och tid för 5 uppresningar. I Säkra steg gruppen var männen överrepresenterade och den genomsnittliga tiden för 5 uppresningar var kortare. I övrigt sågs inga signifikanta skillnader mellan den grupp som valt Otagoträning i broschyrformat och Säkra steg mobilapplikation. Säkra Steg gruppen tenderade att vara mer mobilvana.

     

    Slutsats: Männen i studien valde i större utsträckning Säkra Steg och det kan finnas skäl att tro att individer med större mobilvana väljer Säkra Steg. Inga andra skillnader sågs mellan grupperna. Därmed kan fritt val av träningsstöd i framtida studier av Säkra steg resultera i jämförbara grupper med undantag för könsfördelning trots att deltagarna ej randomiserats men vidare forskning är önskvärt.

    Bakgrund och syfte: Rekommenderad behandling för patienter med höftartros är artrosskola och träning. Manuell terapi kan vara ett komplement till denna behandling. Syftet med denna studie var att undersöka om kaudal traktion med fixering samt behandling av Muskulus Quadriceps coxa, gav effekt för patienter med mild-moderat höftartros som har genomgått artrosskola och handledd träning.

    Metod: En Single Subject Experimental Design med A1-B-A2 design genomfördes med 3 forskningspersoner (fp) under totalt 9 veckor, 3 veckor/fas. Interventionen i B-fasen bestod av kaudal traktion med fixering, tvära friktioner och töjning av M. Quadriceps coxa 2 ggr/ vecka. Höftrelaterad livskvalitet skattades med HOOS totalt 3 ggr, vid första mättillfället i varje fas. I A1-fasen mättes höftrörlighet 5 ggr och smärta skattades dagligen med NRS. I B-fasen mättes rörighet innan varje behandling samt daglig smärtskattning. I A2-fasen mättes rörlighet 4 ggr och smärta skattades dagligen. Data bearbetades visuellt och statistiskt med 2 SD metoden.

    Resultat: Två av fp ökade rörligheten statistiskt signifikant i flexion, inåt och utåtrotation, de skattade högre livskvalitet i HOOS samt reducerade smärtan. Den tredje fp ökade rörligheten statistiskt signifikant i flexion. Övriga rörelseriktningar, smärtskattning och skattad livskvalitet var oförändrade för denna fp.

    Slutsats: Traktion med fixering, tvära friktioner och töjning av Muskulus Quadriceps coxa kan ha klinisk betydelse för patienter med mild-moderat höftartros som har genomgått artrosskola och handledd träning. Fler studier behövs för att kunna dra slutsatser om behandlingseffekten samt undersöka effekten av varje enskild teknik.

    Bakgrund: Trots att Sveriges befolkning ökar i medellivslängd finns det få fungerande preventiva insatser som, ur ett samhällsekonomiskt perspektiv, har huvudfokus på att effektivisera träningen för äldre. Preventiva insatser med målet att öka äldres muskelstyrka och på så sätt minska den degenerativa processen används i dagsläget i för liten utsträckning. Därför blir behovet av anpassade fysioterapeutiska insatser större då andelen äldre väntas öka.

    Syfte: Undersöka genomförbarheten av tio veckors mentorledd träning i ett seniorboende för 55+ år.

    Metod: En genomförbarhetsstudie med kvantitativ och kvalitativ karaktär. Trettiotvå personer rekryterades och delades in i fyra träningsgrupper. Fyra personer värvades som mentorer till var sin träningsgrupp som tränade tre gånger i veckan under tio veckor. Testning av timed up and go (TUG), chairstand 30 sekunder och sex minuters gångtest genomfördes före och efter träningsperioden, samt fokusgruppintervjuer som hölls mot slutet av interventionen. Cirkelträningsprogrammet bestod av åtta övningar som inkluderade stora delar av kroppen. Övningarna utfördes i bestämd ordningsföljd med inledningsvis 3x30 sekunders arbete per station och 15 sekunders vila mellan varje set. Upplägget innefattade styrkeövningar som på olika sätt medförde balans- och konditionsträning.

    Resultat: Interventionen hade en närvaro på 83%, med tre avhopp. Samtliga tester visade på signifikanta förbättringar (p <0,001). I fokusgruppintervjuerna framkom det att deltagarna upplevde fysiska samt psykiska förbättringar som förbättrad balans och ökat välmående. Dessutom gav gruppträningen samhörighet bland deltagarna.

    Slutsats: Resultaten utifrån fysiska tester och fokusgruppintervjuer tyder på goda förutsättningar för genomförbarheten av mentorledd träning för äldre samt möjlighet till fortsatt användning som preventiv metod.

    Abstract [en]

    Introduction: The major paediatric triage systems are primarily based on flow charts involving signs and symptoms for orientation and subjective estimates of the patient's condition. In contrast, the 4-level Paediatric Triage Instrument (PETI) is primarily based on vital parameters and was developed exclusively for paediatric triage in patients with medical symptoms. The aim of this study was to assess the inter-rater reliability of this triage system in children when used by nurses.

    Methods: A design was employed in which triage was performed simultaneously and independently by a research nurse and an emergency department (ED) nurse using the PETI. All patients aged <= 12 years who presented at the ED with a medical symptom were considered eligible for participation.

    Results: The 89 participants exhibited a median age of 2 years and were triaged by 28 different nurses. The inter-rater reliability between nurses calculated with the quadratic-weighted kappa was 0.78 (95% CI 0.67 to 0.89); the linear-weighted kappa was 0.67 (95% CI 0.56 to 0.80) and the unweighted kappa was 0.59 (95% CI 0.44 to 0.73). For the patients aged <1, 1-3 and >3 years, the quadratic-weighted kappa values were 0.67 (95% CI 0.39 to 0.94), 0.86 (95% CI 0.75 to 0.97) and 0.73 (95% CI 0.49 to 0.97), respectively. The median triage duration was 6 min.

    Conclusions: The PETI exhibited substantial reliability when used in children aged <= 12 years and almost perfect reliability among children aged 1-3 years. Moreover, rapid application of the PETI was demonstrated. This study has some limitations, including sample size and generalisability, but the PETI exhibited promise regarding reliability, and the next step could be either a larger reliability study or a validation study.

    Introduktion: Efter stroke sker reorganisering i hjärnan som kan leda till maladaptiv plasticitet och motoriska nedsättningar. Transkraniell magnetisk stimulering (TMS) är en mätmetod med vilken man kan mäta kortikal excitabilitet. TMS är också den vanligaste metoden för att modulera kortikal excitabilitet och används bland annat som behandling för att förbättra motorisk funktion hos strokepatienter.

    Syfte: Syftet med denna studie är att genom en systematisk litteraturgranskning kartlägga vilka moduleringsmetoder, förutom TMS, som använts i syfte att modulera kortikal excitabilitet i motoriska kortex efter stroke, samt se vilken omedelbar effekt de har på kortikal excitabilitet.

    Metod: Mellan den 12 november fram till den 6 december genomfördes en systematisk litteratursökning efter PRISMA-modellen i databaserna; Medline, Pubmed, Cinahl och PEDro. Sökorden som användes var stroke, transcranial magnetic stimulation, rehabilitation och motor cortex excitability. Artiklarna skulle vara publicerade inom de fem senaste åren.

    Resultat: Sökningen resulterade i sju studier med olika moduleringsmetoder. Fem studier visade moduleringsmetoder som gav en ökning av kortikal excitabilitet i motoriska kortex och fyra studier visade moduleringsmetoder som inte gav någon effekt på kortikal excitabilitet i motoriska kortex.

    Konklusion: Resultatet visade att det går att få en omedelbar effekt på kortikal excitabilitet i motoriska kortex vid använding av vissa moduleringsmetoder. En ökning av kortikal excitabilitet skulle kunna bidra till förbättrad motorisk funktion efter stroke. I denna studie valdes de metoder som gav en omedelbar effekt på kortikal excitabilitet vilket gör att studien inte kan uttala om dessa moduleringsmetoder ger effekt även på lång sikt.

    Introduktion Glenohumeralleden är kroppens rörligaste led och en stor del av befolkningen får skulderproblematik där orsaken är förändrad biomekanik i glenohumeralleden. Den förändrade biomekaniken kan bero på styva eller korta muskler kring leden vilket eventuellt även kan leda till nedsatt rörelseomfång. Vid denna typ av problematik är det vanligt att behandling utförs av fysioterapeuter.

    Syfte Syftet med studien är att undersöka hur fysioterapeuter kan bistå vid längdförändring av glenohumeralledens utåtroterande muskler.

    Metod Systematisk litteraturgranskning enligt rekommendationer från Statens beredning för medicinsk utvärderings (SBU) handbok.

    Resultat Tretton artiklar kvalitetsgranskades varav 11 ansågs ha låg eller medelhög risk för systematiskt bias. Resultatet beskrivs utifrån utfallsmått, intervention och resultat av interventioner. Stretching, manuell mobilisering och massage var interventioner som användes. Många studier hade signifikanta resultat på att interventionerna skapade en längdförändring i främst inåtrotation- och adduktion av glenohumeralleden.

    Konklusion Denna litteraturgranskning har brister i kvalitén och därför kan inga långtgående slutsatser dras. Antydningar visar att range of motion är det utfallsmått som används mest, mätt med goniometer eller inclinometer. De interventioner som användes för att öka ROM och skapa längdförändring var stretching, manuell mobilisering och massage. Bakre deltoideus, teres minor och infraspinatus var de muskler som interventionerna riktade in sig på i samband med posterior shoulder tightness (PST) och den vanligaste behandlingsmetoden för PST var cross-body stretch och/eller sleeper stretch. Samtliga interventioner gav resultat på längdförändring och det är därför svårt att avgöra vilken metod som har störst effekt. Fler metoder bör ställas mot varandra och jämföras för att kunna dra säkra slutsatser.

    Abstract [en]

    Objective: To evaluate if Geriatric Interdisciplinary Home Rehabilitation could improve walking ability for older people with hip fracture compared with conventional geriatric care and rehabilitation. A secondary aim was to investigate the postoperative length of hospital stay (LOS).

    Design: Randomized controlled trial.

    Setting: Geriatric ward, ordinary housing, and residential care facilities.

    Participants: People operated on for a hip fracture (n = 205), aged 70 or older, including those with cognitive impairment, and living in the north of Sweden.

    Intervention: Home rehabilitation with the aim of early hospital discharge that was individually designed and carried out by an interdisciplinary team for a maximum of 10 weeks. Special priority was given to prevention of falls, independence in daily activities, and walking ability both indoors and outdoors.

    Measurements: Walking ability and the use of walking device was assessed in an interview during the hospital stay. These assessments were repeated along with gait speed measurements at 3- and 12-month follow-up. The length of the hospital stay after the hip fracture was recorded.

    Results: No significant differences were observed in walking ability, use of walking device, and gait speed at the 3- and 12-month follow-up between the groups. At 12 months, 56.3% of the intervention group and 57.7% of the control group had regained or improved their prefracture walking ability. The median postoperative LOS in the geriatric ward was 6 days shorter for the intervention group (P = .003).

    Conclusion: Participants receiving Geriatric Interdisciplinary Home Rehabilitation regained walking ability in the short-and long-term similar to those receiving conventional geriatric care and rehabilitation according to a multifactorial rehabilitation program. The intervention group had a significantly shorter postoperative LOS in the hospital.
