Perceptions of intimacy and integrity in formal home care: [Föreställningar om intimitet och integritet i hemtjänst och personlig assistans]2023Inngår i: European Journal of Social Work, ISSN 1369-1457, E-ISSN 1468-2664, Vol. 26, nr 5, s. 828-839Artikkel i tidsskrift (Fagfellevurdert)
    Abstract [en]

Sweden has a long tradition of providing social care and support to its citizens in their own homes through formal home care, delivered either by home care services or personal assistance. A majority of people given support by formal home care need assistance with personal and intimate care. The focus of this interview study was on exploring care recipients’, care workers’, personal assistants’ and care unit managers’ perceptions and experiences of intimate and personal care in the context of formal home care in Sweden. In total, 57 interviews were conducted with 42 persons. Three themes emerged in the analysis: Personal Hygiene, Personal Sphere, and The Contextual Variability of Intimacy. Interviewees described intimate care as being inseparable from a person’s service needs as a whole. Highlighted was how caring for and washing intimate body parts, intrusion into recipients’ personal spheres, and the need to preserve integrity vary depending on situational, temporal and relational aspects. To safeguard the care recipient’s influence, integrity and dignity in the reception/provision of care work, home care services and personal assistance, it is important to raise awareness of the variation in perceptions of intimate and personal care in education and inhouse training.

Methodological challenges in the implementation and evaluation of social welfare policies2012Inngår i: International Journal of Social Research Methodology, ISSN 1364-5579, E-ISSN 1464-5300, Vol. 15, nr 1, s. 69-80Artikkel i tidsskrift (Fagfellevurdert)
    Abstract [en]

As social reality is quite elusive, even regarding seemingly well-recognized everyday concepts and objects, there are always methodological challenges underlying assessments and evaluations of implementation policies. The present article addresses this area of concern by presenting the results of a rereading of an empirical study of elderly home care services. Our results reveal the emergence of a dissolution of common and professional key concepts and objects in these welfare services to a degree that challenges both the implementation policy and the evaluation of policy. We claim that this has methodological implications for evaluation of implementation policies in general.

Strategies to handle the challenges of intimacy in nighttime home care services2017Inngår i: European Journal of Social Work, ISSN 1369-1457, E-ISSN 1468-2664, Vol. 20, nr 2, s. 219-230Artikkel i tidsskrift (Fagfellevurdert)
    Abstract [en]

The provision of intimate and personal care constitutes a challenge for both careworkers and care recipients and is still a neglected area of research. An observational study of the interaction between the careworkers and care recipients in night-time home care services was conducted in a large municipality in Sweden. The results were analysed in light of previous research and theorising on strategies for handling intimacy in intimate care. The study highlights what appears to be a tension between the ways in which the recipient of care is conceptualised as an active consumer of care in present-day guidelines and the strategies chosen on the part of both caregivers and care recipients, when intimacy and integrity is most at stake, and framed as it is by the care recipients' situation of dependency and vulnerability. Home care services night-time was shown to be a case that markedly differs from many other settings of intimate care, but in the interactional routines intimate care came forth as a smooth and minimally obtrusive activity. The careworkers and care recipients engaged in strategies such as disattention, eye-discipline, middle-distance orientation, and objectification, thereby serving the purpose of balancing the transgressions of thresholds of intimacy.

Empathetic attuning: 'How would i feel if i had to expose myself all the time?'—strategies for managing personal and intimate care in swedish formal home care2023Inngår i: British Journal of Social Work, ISSN 0045-3102, E-ISSN 1468-263X, Vol. 53, nr 2, s. 921-938Artikkel i tidsskrift (Fagfellevurdert)
    Abstract [en]

Sweden, as a welfare state, has a long tradition of providing formal home care and support to their citizens in their own homes, either through home care services or personal assistance. A large percentage of frail elderly and persons with disability who receive formal home care require personal and intimate care, such as help with eating, showering, getting dressed and personal hygiene. Managing intimacy and safeguarding the care recipient’s integrity pose particular challenges for staff. The aim of this qualitative interview study is to describe and analyse care workers’ (CWs) and personal assistants’ (PAs) strategies for managing situations and challenges related to provision of personal and intimate care in the context of formal home care. Semi-structured interviews with eleven CWs and nine PAs were conducted. Our analysis reveals a complex repertoire of relational and communicative strategies, within an overall approach - which we labelled 'empathetic attuning'—of relating to the current situation and task at hand whilst safeguarding integrity. These strategies were intertwined with dimensions of time. The possibility to accomplish satisfactory personal and intimate care rests on structural and organisational conditions that promote sustainable working conditions, where relations characterised by continuity, integrity and respect can be realised.

The emotional politics of research collaboration2013Collection/Antologi (Annet vitenskapelig)
    Abstract [en]

Research collaboration in the form of networks, projects and centers has become one of the dominant modes of engaging in research, especially funded research, across all academic domains. However, there has been little research on the processes of such collaborations, particularly their affective dimensions. These, as this volume demonstrates and as researchers know well, are highly important, yet mostly not directly engaged with when scientists work together, even though they are experienced by everybody involved. This volume is the first to consider questions such as how the naming of projects impacts on their accompanying “affect-scapes,” the policing or disciplining of emotions in research collaborations, their accompanying tensions and how these might be managed, and the challenges to trust between scientists that such collaborations present. Drawing on theories of affect and literature on collaboration, as well as on the contributors experiences of being involved in large-scale research projects, the volume also importantly deals directly with some of the key emotions that occur during research collaborations such as blame, elation, frustration, alienation and belonging, and suggests some ways in which one might engage productively with the affective dimensions of research collaboration.

    Abstract [en]

This article reports on a study the aim of which was to sketch a tentative model for illuminating variations in the professional role of a social worker, based on Swedish social work students' own experiences of knowledge use and its premises in client meetings during field practice. Social work students voluntarily submitted narratives depicting problematic and unproblematic situations experienced during field practice in one of three broad occupational positions of social work: casework; counselling; and social assistance. Three different formative aspects were identified as constituting premises for knowledge use in social work. Knowledge use from differing epistemological domains are set in relation to the identified formative aspects, all together comprising a role that we label 'the Compassionate Bureaucrat'.

Dependency and vulnerability in the 21st century2016Konferansepaper (Annet vitenskapelig)
    Abstract [en]

A common trend in Scandinavian countries today is that policies on the formation of elderly care is framed by concepts such as freedom of choice and marketization, where ‘clients’ are implicitly conceptualized as active and alert customers, whereas the levels of dependency amongst those living in the community is increasing. E.g. the 1992 reform of elder care in Sweden prioritized enabling older people to continue living in their homes in spite of conditions such as disability, frailty, illness or dementia. One result of this reform is a 25% reduction in the number of beds for the care of older people in residential homes over the past years despite an increase in the number of citizens aged 65 and over. This to the effect that those now receiving home care services are on average in worse condition than before, and that only those in need of a great deal of help receive such services.

Disability and the frailty of old age put many people in need of help with intimate care in their homes: e.g. undressing, using the toilet, changing adult diapers or incontinence pads, showering, washing intimate parts of the body, brushing teeth, shaving, etc. — still little attention is being paid to this hidden ‘off the radar world’. Set betwixt and between home and institution, private and professional, the situation and relation of intimate care in home care services is a particularly precarious one.

Whilst legislation and guidelines emphasise participation and influence, our recent observational studies of intimate care in home care services show strategies of objectification, distancing and routinization to be used in an interaction between care workers and care recipients, as ways to balance the intimate, and safeguard the integrity of the person. These results challenge professed ideals, and show how dependency in others may neutralise users’ power.

    Abstract [en]

"Dependency and Vulnerability in the Twenty-First Century: The Swedish Case" by Hildur Kalman illustrates the impact of regulations on the provision of care for older adults. Even if Sweden is perceived as having generous welfare policies, Kalman's paper is concerned with the noticeable move toward the marketization of care and its impact on the care recipients, more specifically, older adults who require intimate care.

    Faking orgasms and the idea of successful sexuality2013Inngår i: Janus Head, ISSN 1524-2269, E-ISSN 1521-9194, Vol. 13, nr 1, s. 97-118Artikkel i tidsskrift (Fagfellevurdert)
    Abstract [en]

In the Nordic countries, at a time when women have only recently won the right to their own bodies and to a sexuality of  their own and for themselves, women nevertheless fake orgasms. Moreover, a common question posed to the Swedish Association for Sexuality Education (RFSU) deals with lack of desire. Not only are women faking and complaining of lack of desire, but men as well. It seems that contemporary ideals surrounding sexuality converge with quests for not only pleasure and love, but also for fitting in and experiencing what is conceived of as normal and “successful” sexuality. This essay examines the contemporary and prevalent phenomenon of faked orgasms from the perspectives of feminist theory and phenomenology.

Loss and grieving: selves between autonomy and dependence2012Inngår i: PhaenEx: Journal of Existential and Phenomenological Theory and Culture, E-ISSN 1911-1576, Vol. 7, nr 2, s. 1-27Artikkel i tidsskrift (Fagfellevurdert)
    Abstract [en]

A recurrent theme in contemporary narratives of grieving is that there is a gap between the griever’s more or less consciously chosen expression of, and acting out of, grief and loss and other people’s seeming lack of acceptance. Starting from the view that the social context of feelings and emotions are constitutive in making an emotional experience what it is, this article explores what is done and experienced in acts of grief. A phenomenological perspective is applied to analyze the conditioning of these experiences, and what the acts of grieving may accomplish in relation to various aspects of the self, indeed selves—that is, in relation to the surviving relative as well as the deceased.

The structure of knowing: Existential trust as an epistemological category1999Doktoravhandling, monografi (Annet vitenskapelig)
    Abstract [en]

This thesis investigates the structure of knowing, and it argues that existential trust is an epistemological category.

The aim of the dissertation is to develop a view according to which all human activity is seen as an activity of a lived body, and in which the understanding of the structure of such activity is regarded as central for the solution even of epistemological problems. This view is not rooted in any one philosophical tradition, but circles around activity of the lived body. It connects thinkers who in other respects belong to different "isms" in philosophy. Central to the dissertation are Aristotle, Dewey, Merleau-Ponty, Wittgenstein, Ryle, Anscombe, Polanyi, and Grene.

Michael Polanyi's concept of tacit knowledge, and connected concepts like attend to, attend from, and subsidiary awareness, are presented. Different kinds of subsidiary awareness, not noted by Polanyi, are distinguished. It is also argued that Polanyi has not seen all the implicatons of his view that instruments can be interiorized and be part of the lived body. Conversely, parts of the normally lived body can be exteriorized. Nor has Polanyi seen that one has subsidiary awareness of oneself as a certain kind ofperson. This fact, in turn, is shown to have implications for the way we constitute ourselves as agents. Since we are engendered agents, we always attend from gender.

In the last decade, the concept of trust has definitely entered epistemology. Mostly, it has been in terms of trusting testimony and/or testifier. This thesis wants to deepen that account. There is a more fundamental kind of trust, namely trust in oneself and trust in the world. It is called existential trust, and it is of epistemological importance, too. Existential trust, it is claimed, is necessary in all acts of knowing. Probably, this fact is hard to discover without having recourse to a distinction like that between attending from and attending to. Existential trust is shown in the way we attend from something. Observation and thinking are central epistemological categories, of course, but they should be supplemented by trust. Without trust they cannot perform any epistemological work.

Linguistically, 'to know' is a state verb. Superficially, it describes only a state, not an activity. This fact, however, does not contradict the view of knowledge put forward in the dissertation. To know is to have a disposition to perform successfully either some kind of action (= knowing how), or to perform some kind of true assertive speech act (= knowing that). Basically, knowing is an activity.
