    Abstract [en]

    Objective: Recent research shows that almost every second woman with breast cancer is depressed or has anxiety; the risk for younger women is even higher. Moreover, research shows that women are at risk for developing depression, also a threat for women with breast cancer. The aim of this randomized controlled clinical trial was to study the outcome of five sessions of art therapy given at a 5-week period of postoperative radiotherapy.

    Methods: The participants were between 37 and 69 years old; six participants in each group were below 50 years of age. Half of the participants (n = 20) received art therapy and the other half (n = 21) were assigned to a control group. At the first measurement, at least 17% (n = 7) of the participants medicated with antidepressants. Data were collected before and after art therapy and at a 4-month follow-up using self-rating scales that measure self-image (the Structural Analysis of Social Behaviour) and psychiatric symptoms (the Symptom Check List–90).

    Results: At follow-up, significant lower ratings of depression, anxiety, and somatic symptoms and less general symptoms were reported for the art therapy group compared to the control group. The regression analysis showed that art therapy relates to lower ratings of depression, anxiety, and general symptoms; chemotherapeutic treatment predicts lower depressive symptoms; in contrast to axilliary surgery and hormonal treatment as well as being a parent predicts higher ratings of anxiety and general symptoms.

    Significance of results: The conclusion suggests that art therapy has a long-term effect on the crisis following the breast cancer and its consequences.

    The entangled butterfly and the blue cactus: two case studies on time-limited psychodynamic art psychotherapyManuscript (Other academic)
    Abstract [en]

    The aim of these two case studies is an in-depth analysis of the practice of scribbling in time- limited psychodynamic art psychotherapy. Our hypothesis is that the answers to "What do you see?" change as therapy proceeds, due to changes in emotions and cognitions expressed in scribbles and amplifications. The treatment is one hour per week for ten weeks, exempli- fied by two women with depression selected for differences in age, demographics and art expression. Analysis of the pictures and words is in two steps. The pictures were first tran- scribed into words using a phenomenological approach and then analysed according to con- tent analysis. Correspondingly, the words of the sessions were first transcribed verbatim and then analysed according to content analysis. The results show that the even if the two women have the same diagnosis at inclusion according to DSM IV, the symptoms of depression are expressed differently both in how they are manifested and in latent messages in the content analysis. The two women’s therapies can be divided into three phases each marked by turning points in the pictures – hence the pictures rather than the words set the theme for the phase. On leaving therapy the patients are able to use their drawings, their amplifications are in a complete form, they symbolise in words what they have expressed in pictures.

    The study’s conclusion is that the practice of scribbling seems to give a better capacity to regulate affects and, hence, a better regulation of the self. The women used not only their intellects but also their bodies in making latent communications manifest; their self-esteem improved as therapy proceeded, and they regained a sensitivity of "me – not me", i.e. better boundaries. The clinical implications are that the time-limited model of art psychotherapy is well suited to process emotions connected with depression, and that the patient’s problem should determine the length of the time limit.

    Abstract [sv]

    Under tidsperioden vt 2000 - vt 2001 prövades psykoterapihandledning via videokonferens på Kurs IX, moment 4: ”Tillämpning/psykoterapi - klientarbete och klientrapport” vid Psykologprogrammet, Umeå universitet. Syftet med denna utvärdering var att undersöka om denna typ av handledning har samma pedagogiska värde som handledning förmedlad av handledare som befinner sig i samma rum som sina handledda. Under den aktuella tidsperioden förekom fyra olika handledningstyper: psykodynamisk, kognitiv beteendeterapi, traditionell kognitiv och kognitiv handledning med videokonferens. 35 psykologstudenter som påbörjade momentet klientarbete vt 2000 fyllde i en enkät som handlade om egen professionell utveckling respektive hur handledningen fungerade vid två tillfällen. Handledarna (n=7)ombads besvara samma enkätfrågor vid två tillfällen utifrån deras syn på studentens professionella utveckling samt hur handledningen hade fungerat. Resultaten visade att de studenter som fått handledning via videokonferens inte skilde sig märkbart på någon punkt från de studenter som fått traditionell handledning. Majoriteten av psykologstudenterna, oberoende av typ av handledning, upplevde att de hade utvecklats professionellt, att de hade lärt sig att arbeta i psykoterapeutiska samtal och i grupphandledning. Deras handledare gjorde likartade bedömningar. Denna utvärderingsstudie anger attdet är möjligt att arbeta med videokonferenshandledning varvat med handledning som ges i rummet inom Psykologprogrammet.

    Utvärdering av handledning inom ramen för PUPPA-projektet2003Report (Other academic)
    Abstract [sv]

    Denna utvärderingsstudie undersökte värdet av handledning för vårdpersonal inom primärvård, tandvård och sjukhusvård i Västerbottens läns landsting. Handledningen tillskapades inom ramen för PUPPA-projektet(Projektgruppen för Utveckling av ett Praktiskt Psykosomatiskt Arbetssätt), utifrån vårdpersonalens önskemål att få tillfälle att utveckla bättre verktyg i bemötande och behandling av patienter som har psykosomatiska besvär. För att undersöka utfallet av denna handledningsinsats genomfördes en utvärdering, vars huvudsyfte var att undersöka om handledningsinsatsen har påverkat personalens upplevelse av professionell effektivitet och kompetens i arbetet med patienter som har psykosomatiska besvär. Ett annat syfte var att undersöka graden av utbrändhet samt personalens känsla av sammanhang. I denna studie deltog 38 personer från åtta handledningsgrupper. Före och efter handledningens genomförande fyllde samtliga deltagare i självskattningsformulär och nio av dessa deltagare medverkade också vid djupintervjuer. Efter det att handledningen avslutats, fyllde en majoritet av deltagarna (n =32) i en mindre enkät med frågor om bl.a. deltagarnas uppfattning om handledningen. Resultaten visade att deltagarnas upplevelse av att vara kompetenta i sitt arbete (Professionell effektivitet [PE]) stärktes signifikant under handledningsperioden. Den genomsnittliga gruppdeltagaren hade en stabil Känsla av sammanhang (KASAM), som var helt i nivå med andra studier. Resultaten av mätningarna av utbrändhet var likaså i linje med tidigare studier; graden av utmattning och trötthet förändrades inte efter den korta handledningsperioden (cirka 1 år). Två delskalor i Maslach Burnout Inventory (MBI) indikerade att den genomsnittliga deltagaren var sliten och utmattad inför handledningen, detta resultat var oförändrat efter avslutad handledning. I kontrast så visade The Burnout Measure (TEDIUM) inte på någon påtaglig leda, varken före eller efter handledningen. Resultaten antyder att vårdpersonalens utmattning och slitenhet inte i första hand är relaterad till patientarbetet. Det generella intryck som deltagarna förmedlade i den enkät som besvarades efter handledningen var att handledningen upplevdes mycket värdefull, något som bekräftades i analysen av de individuella intervjuerna. Från enkät och intervjuer framkom också att landstingets organisationsformer och organisationsförändringar upplevdes resultera i påfrestningar i många situationer. Slutsatsen av denna utvärderingsstudie är att grupphandledning är ett välkommet och potentiellt värdefullt komplement för vårdpersonal som upplever ett växande behov av stöd och hjälp att hantera ökande krav och komplexitet i arbetet med patienter.

    Utvärdering utifrån klientens perspektiv av studerandeterapier vid psykologmottagningen, Institutionen för psykologi, Umeå universitet2002Report (Other academic)
    Abstract [sv]

    Denna utvärdering undersökte värdet, sett ur klientperspektiv, av utbildningsterapier som ges av studenter vid Psykologprogrammet, Umeå universitet. Samplet bestod av 52 klienter som började i studerandeterapi under ett år och medverkade i utvärderingen före och efter behandlingen. Hälften av klienterna var studenter, de övriga förvärvsarbetande. Vanligaste orsaker till att söka terapi var nedstämdhet, identitetsproblem och relationssvårigheter. Majoriteten fullföljde terapin (n = 43, M = 24 samtal) medan ett fåtal avbröt (n = 9, M = 4 samtal). Analyser av initiala självskattningar visade att gruppen som avbröt skattat att de var signifikant mer kallsinniga och socialt undvikande före terapin jämfört med gruppen som fullföljde. Vid eftermätningen kvarstod skillnaden mellan grupperna då det gäller interpersonella problem. Båda klientgrupperna uppvisade en mer kärleksfull och mindre självanklagande självbild efter terapin. Resultaten visade att utbildningsterapier har ett värde för klienterna. Resultaten kan användas vid val av instrument vid intakebedömningar respektive studier av behandlingsresultat.

    Change and stability of attachment from childhood to early adulthood2002Report (Other academic)
    Abstract [en]

    The main objective of this study was to examine continuity and change inattachment classification from childhood to early adulthood. The relationsbetween childhood attachment, stressful life events, and young adultattachment were examined based on data from observations of thirty-fourSwedish middle-class 3 years old children who were invited to a play sessiontogether with their mothers. At 23 years, the same participants wereinterviewed with an attachment interview and they also filled out Paykel’s LifeEvent Scale. Results showed that the stability of secure and insecure attachment classification was 74 %. Classification of childhood attachment (secure versus insecure) together with stressful life events before 18 years were strong predictors of adult attachment. In conclusion, the present study supports the relevance of childhood experience to development later in life.

    The shape of alliance in psychotherapy and supervision using growth curve modeling was examined for clinically inexperienced trainee therapists, who were engaged in long-term cognitive behavioral - or psychodynamic individual psychotherapy at a Psychology Clinic in Sweden. Trainee therapists rated their view of the alliance with their clients and their supervisors on the Working Alliance Inventory at five time points. The alliance to the client show a consistent, positive development throughout the therapy. The alliance to the supervisors show initially a decrease, followed by an increase, and then again, the rate of increase in the supervisory alliance ratings slowed down indicating a more complex view of the supervisory alliance compared to the view of the alliance in psychotherapy.

    Abstract [en]

    Two different admission procedures (high school grades/scholastic tests and high school grades/scholastic tests/interview) to the five-year Master's Program in Professional Psychology at Stockholm University were examined. The analyses were based on data from students who were admitted on a combination of high school grades/scholastic test and interview, and students who were admitted based only on high school grades/scholastic test. In the final semester of the five-year Psychologist Program, the students and their supervisors rated the student's development of psychotherapeutic knowledge and skills, and their relation to the supervisor and the supervision group. A self rating scale was used. The results showed that students who were admitted upon interview and their supervisors, rated significantly higher on all factors. Moreover, students admitted upon interview rated their development of psychotherapeutic skill significantly higher than students admitted traditionally. The corresponding difference between supervisor ratings was not significant. Both student and supervisor ratings indicated that the students admitted by an interview had a better relation to their supervisor and to their supervision group.
