Abstract [sv]
We have shown that anti-citrulline antibodies predict onset of rheumatoid arthritis (RA) by years. The relative risk is highly increased by having PTPN22 1858T variant and HLA-DRB1 0404/0401 and a number of pro-inflammatory cytokines were up-regulated years before onset of symptoms. We have reported increased morbidity and mortality in patients with RA, in cardiovascular disease (CVD) and ischaemic heart disease. We hypothesise that an initiating process gradually leads to immunological activity, measured as autoantibody production and up-regulated cytokines, and development of RA in genetically predisposed individuals. In this project we will include individuals with pre-dating samples in the Medical Biobank and other serological banks, 1st degree relatives from multi-case RA families and RA-patients to analyse 1) markers of immune activation (antibody production using multiples assays, gene expression profiling and genetic factors) in plasma and frozen cells i) before disease onset, and ii) after onset of RA, and 2) by using epidemiological and physiological methods the development of co-morbidity. A prospective cohort is continuously recruited, technical equipment is available and collaborations established. Knowledge will be gained about the aetiology-pathogenic process leading to RA, and disease progression. The interaction between autoantibodies and genetic markers in relation to inflammatory activity, destruction, and premature atherosclerosis will be explored.
