Title [sv]
Visualisering av tyst åderförkalkning för optimerad kardiovaskulär prevention. En pragmatisk befolkningsbaserad randomiserad kontrollerad intervention i rutinsjukvård.
Title [en]
Direct visualization of asymptomatic atherosclerotic disease for optimum cardiovascular prevention. A population based pragmatic randomised controlled trial within ordinary care
Abstract [sv]
This randomized controlled intervention study aims at high precision stratification of cardiovascular disease (CVD) risks, improved communication about CVD risk and accurate risk perception, and as a consequence optimum compliance to prevention with improved CVD risk profile and reduction of premature CVD deaths. It is integrated into the existing population-based screening and risk factor intervention program in Västerbotten County. Subjects with at least one of six conventional risk factors will undergo a carotid ultrasound with detailed exploration of intima media thickness and plaques, and randomly be assigned to two groups: 1/ they and their doctors will get, or 2/ will not get imaging results of the ultrasound. All participants will be treated according to current clinical guidelines for risk factor control. In subgroups added very intensive pharmacological and/or life style intervention will also be evaluated The ultrasound will be repeated after three years and the results disseminated to the two groups. Primary outcome variables are changes of CVD risk factor levels and life style. Changes in ultrasound results, prescriptions of preventive medications and hard endpoints within 5 years (MI, stroke, revascularization, all-cause and CVD related deaths) will also be analysed. Both quantitative and qualitative research methods will be used. The results are expected to be highly relevant for clinical settings and ready to implement in both primary and hospital care.
