Low energy fractures is common in the elderly and associated with pain, fear, increased dependency, reduced quality of life, and high cost for society. With an increasing number of elderly the next 30 years the problems associated with fractures will increase. Therefore, effective preventive measures that target both the fragile bone and the fall will be of highest importance. The overarching aim of the present project is to increase the knowledge about the fall as a risk factor for fractures, and study the effects of bone-specific treatment on fractures in the elderly. We will investigate risk factors for falls using different strategies and cohorts ranging from nationwide register studies utilizing Sweden?s unique quality registers, to a large cohort study. The cohort study is set up with the pre-specified aim to study falls and fractures in elderly men and women, including detailed information of bone structure, balance, gait variability, muscle properties, and genomic information. The effects of bone-specific treatment will be studied in nationwide cohorts with respect to the outcome of both clinical fractures and possible side effects. The focus will be on the effects of bisphosphonates in elderly individuals, where there is lack of knowledge concerning the effects on clinical fractures. In summary, within this project we aim to increase improve the possibilities for effective prevention of fractures in the elderly by targeting both the fall and the fragile bone.
