Abstract [en]
The primary treatment of colorectal cancer (CRC) is surgery but only half of the operated patients are cured. Tumor cell spread to lymph nodes is the single most important indication for risk of recurrence and cancer-death. The method used for detection of tumor cells has serious limitations and patients are misclassified. To save lives and hospital costs there is urgent need for a better method. Based on our research we have developed an mRNA test utilizing five biomarkers that define key properties of the disseminated tumor cells. Using this test in a patient material of 160 CRC patients we identified patients with different risk of recurrence (short-, intermediate- and long disease-free survival). Additionally we found a number of patients that had large numbers of tumor cells in their nodes that were missed by the standard method. To confirm these results and to investigate whether increasing the number of analyzed nodes improves the prognostic value of the biomarker test we have initiated a multicenter, prospective study (350 patients) in which the present routine method and the biomarker mRNA test are compared. Moreover we explore the possibility of using the biomarker test as a blood plasma test in follow-up studies after surgery, by analyzing extracellular micro-vesicles released by tumor cells. The research group at Immunology, Umeå University will perform the analyses, construct a data bank and sample collection. A RUO biomarker mRNA kit is under development.
