Abstract [sv]
We have reported increased morbidity and mortality in patients with rheumatoid arthritis (RA), in cardiovascular disease (CVD) and ischaemic heart disease in particular. We have shown that anti-citrullin antibodies predict the onset of RA by years. The relative risk was highly increased by having PTPN22 1858T variant and HLA-DRB*1 0404/0401. The hypothesis is that an initiating process gradually leads to immunological activity, measured as autoantibody production, and development of RA in genetically predisposed individuals. Autoantibody profiles delimit the disease severity, therapeutic response and development of co-morbidity, particularly CVD. The development of CVD is favoured by inflammation. We will analyse 1) markers of immune activation (antibody production, epitope spreading, isotypes, gene expression profiling and genetic factors) in plasma and frozen cells from individuals before disease onset, 2) after onset of RA, in relation to disease progression, 3) and development of co-morbidity. 4) Analyse the interaction between inflammatory, metabolic and genetic factors, predisposing for CVD. A prospective cohort is continuously recruited, technical equipment is available and collaborations established. Knowledge will be gained about the aethio-pathogenetic process leading to RA, and progression. The interaction between autoantibodies, and genetic markers and the interplay between inflammatory activity, destruction, premature atherosclerosis and CVD will be explored.
