Abstract [sv]

Psykisk ohälsa utgör idag ett stort samhällsproblem, och i ljuset av begränsade vårdresurser framstår studentledd behandling som ett möjligt alternativ för att öka tillgången till psykologisk behandling. Syftet med denna studie var att undersöka behandlingseffekter på grupp- och individnivå för studentledd individuell kognitiv beteendeterapi respektive studentledd gruppbehandling för stress och sömn, samt om dessa behandlingsformer kan ge liknande behandlingseffekter. Studien har en praktikbaserad design och baseras på enkätdata som insamlats från psykologmottagningen vid Umeå universitet samt studentledd stress- och sömnbehandling i grupp i primärvården i Umeå mellan 2020–2023. Deltagarna i studien var patienter med lindrig till medelsvår psykisk ohälsa som genomgått individuell studentledd KBT-behandling (n = 109) eller studentledd gruppbehandling för stress (n = 208) respektive sömn (n = 83). Studiens utfallsmått var upplevd stress, sömnbesvär, ångest och depression och mättes med självskattningsinstrumenten Perceived Stress Scale (PSS-10), Karolinska Sleep Questionnaire (KSQ) och Hospital Anxiety and Depression Scale (HADS). Data analyserades med deskriptiv statistik, repeated measures ANOVA och analyser av klinisk signifikans. Resultaten visade signifikant symtomreduktion efter studentledd individuell behandling på samtliga utfallsmått med genomgående stora effektstorlekar. Störst andel förbättrade och återställda observerades för ångestsymtom (31 %), följt av depressionssymtom (22 %), upplevd stress (20 %) och sömnbesvär (10 %). Vidare visade resultaten att individuell behandling och studentledd gruppbehandling i huvudsak gav likartade behandlingseffekter såväl på gruppnivå som individnivå. Sammantaget tyder resultaten på att både studentledd individuell och gruppbaserad KBT kan utgöra resurseffektiva behandlingsalternativ för personer med mild till medelsvår psykisk ohälsa.

Abstract [en]

Mental ill-health constitutes a major societal challenge, and given limited healthcare resources, student-led treatment may increase access to psychological care. This study aimed to examine treatment effects at group and individual levels for student-led individual cognitive behavioral therapy (CBT) and group-based interventions for stress- and sleep problems, and whether these treatments yield comparable effects. The study used a practice-based design with questionnaire data collected before and after student-led treatments conducted at the psychology training clinic at Umeå University as well as stress and sleep treatments in primary care in Umeå between 2020-2023. Participants were patients with mild to moderate mental ill-health who received student-led individual treatment (n = 109) or group-based interventions for stress- (n= 208) or sleep problems (n = 83). Outcome measures included perceived stress, sleep problems, anxiety and depression, assessed using self-report instruments Perceived Stress Scale (PSS-10), Karolinska Sleep Questionnaire (KSQ), and Hospital Anxiety and Depression Scale (HADS). Data were analyzed using descriptive statistics, repeated measures ANOVA, and Reliable Change Index. Results showed statistically significant symptom reduction following individual treatment across all outcome measures, with large effect sizes. The highest proportion showing reliable improvement and recovery was observed for anxiety symptoms (31 %), followed by depressive symptoms (22 %), perceived stress (20 %) and sleep problems (10 %). Furthermore, the findings suggest that student-led individual and group-based interventions produced largely comparable treatment effects. Overall, the results indicate that both student-led individual and group-based CBT may serve as resource-efficient treatment options for individuals with mild to moderate mental ill-health.
