Abstract [sv]

Bakgrund: Datortomografi är en komplex modalitet som avbildar kroppen. Filter kan användas för att sortera bort fotoner som endast ger stråldos. Röntgenstrålar är joniserande vilket innebär risker för stokastiska skador. Patienter har dock ingen dosgräns vilket sätter dem i en sårbar position. Röntgensjuksköterskor ska arbeta i enlighet med ALARA-principen för att minska stråldos samtidigt som diagnostisk bildkvalitet bibehålls.

Syfte: Syftet var att jämföra stråldos och bildkvalitet vid konventionell datortomografi med och utan tennfilter.

Metod: Kvantitativa artiklar söktes i Science direct, Scopus, Pubmed och Cinahl. Olika sökord såsom ”ct” och ”radiation dosage” användes för att söka artiklar. Urvalet skedde i fyra steg och genomgick en granskning med en granskningsmall. Åtta artiklar analyserades med en fyrstegsmetodik.

Resultat: Sju av åtta artiklar visade en statistisk signifikant minskning i minst en stråldosparameter vid användning av tennfilter. Bildkvalitetsparametrarna varierade – både statistiskt signifikant minskning och ökning, eller ingen signifikant skillnad. Subjektiv bildbedömning varierade men det diagnostiska värdet bibehölls. 

Konklusion: Tennfilter kan minska stråldos signifikant samtidigt som det diagnostiska värdet behålls. Det kan vara ett användbart verktyg för röntgensjuksköterskor för att upprätthålla god patientsäkerhet samt minska risken för strålningsrelaterade skador.

Abstract [en]

Background: Computed tomography is a complex modality that images the body. Filters can be used to sort out photons that only provide radiation dose. X-rays are ionizing which means an increased risk of stochastic damages. Patients don’t have a set limit for how much radiation they can receive, which puts them in a vulnerable position. Radiology nurses should work in accordance with the ALARA-principle to reduce radiation dosage whilst maintaining the diagnostic quality of the images. 

Aim: The aim was to compare radiation dosage and image quality in conventional computed tomography with and without a tin filter.

Methods: Quantitative articles were searched in Science direct, Scopus, Pubmed and Cinahl. Keywords such as “ct” and “radiation dosage” were used to search for articles. The selection happened in four stages and were later quality-controlled with a quantitative checklist. Eight articles were analysed using a four-step method.

Results: Seven of eight articles showed a statistically significant decrease in at least one radiation dosage parameter when using a tin filter. Image quality parameters varied with tin filter use showing statistically significant decreases and increases, or no significant difference. Subjective image quality varied but the diagnostic value was maintained.

Conclusion: Tin filter can reduce radiation dosage significantly whilst keeping the diagnostic value. It can be a useful tool for radiology nurses to ensure patient safety and decrease the risk of injuries related to radiation.
