(Swedish)
Abstract [en]

Introduction: Neck pain is a global problem. The role of muscle strength in its treatment needs further investigation, necessitating valid clinical measurement methods. Aims: The aim of this cross-sectional study was to examine the convergent and discriminant construct validity and acceptability of measuring maximum voluntary isometric contraction (MVIC) of the neck muscles, in primary care patients with chronic neck pain. Methods: MVIC of the neck flexors and extensors was measured using a commercially available digital dynamometer and construct validity examined. Using hypotheses testing, we studied correlations with Swedish versions of the 11 item Tampa scale of kinesiophobia (TSK 11-SV) and pain catastrophizing scale (PCS-SW) for discriminant and with a modified version of the cervical flexion endurance test (CFET) for convergent construct validity. Acceptability of the MVIC test was assessed using a theory based, self-developed questionnaire. Adverse events were monitored. Results: Spearman rank correlation coefficients between MVIC, TSK 11-SV and PCS-SW were 0.288 and 0.260 respectively in extension and 0.153 and 0.185 in flexion. This was above our hypothesized r ³ -0.29, supporting divergent construct validity. In contrast correlation between flexion MVIC and CFET was r = 0.206, falling short of our hypothesis and not supporting convergent construct validity. Acceptability of the test was good with no serious adverse events. Conclusion: Neck strength measurement with an affordable digital dynamometer in primary care was safe, showed good acceptability and support for discriminant but not construct validity of its measurements. Due to small sample size, future studies need to confirm these preliminary results.
